Enhancing family caregiving through support from community health workers: A pathway towards a more integrated healthcare system.

PurposeThis commentary examines the growing burden on family caregivers in the United States and argues for the strategic integration of community health workers (CHWs) into caregiver support models as a practical, equity-centered solution.Major FindingsFamily caregivers provide the majority of long-term care in the United States, but receive limited formal support despite escalating care demands and increasing complexity. As caregiving continues shifting into community and home settings, CHWs are uniquely positioned to address this gap through care navigation, coordination, education, and cul

The public healthcare sector of Mauritius: knowledge of and attitudes toward hearing loss among community health workers.

Mauritius does not have community health workers trained in identifying risk factors for hearing loss or in referring patients for diagnostic testing. It is crucial to gather information about the knowledge of and attitudes toward hearing loss among community health workers before involving them in the identification and intervention of hearing loss in Mauritius. To describe the knowledge of and attitudes toward hearing loss among community health workers in Mauritius. A descriptive survey design with quantitative analysis was used. Using non-probability purposive sampling, 125 community healt

Engaging community health workers in maternal and infant death identification in Khayelitsha, South Africa: a pilot study.

BACKGROUND: Engaging community health workers in a formalised death review process through verbal and social autopsy has been utilised in different settings to estimate the burden and causes of mortality, where civil registration and vital statistics systems are weak. This method has not been widely adopted. We piloted the use of trained community health workers (CHW) to investigate the extent of unreported maternal and infant deaths in Khayelitsha and explored requirements of such a programme and the role of CHWs in bridging gaps. METHODS: This was a mixed methods study, incorporating both q

Health extension workers improve tuberculosis case detection and treatment success in southern Ethiopia: a community randomized trial.

BACKGROUND: One of the main strategies to control tuberculosis (TB) is to find and treat people with active disease. Unfortunately, the case detection rates remain low in many countries. Thus, we need interventions to find and treat sufficient number of patients to control TB. We investigated whether involving health extension workers (HEWs: trained community health workers) in TB control improved smear-positive case detection and treatment success rates in southern Ethiopia. METHODOLOGY/PRINCIPAL FINDING: We carried out a community-randomized trial in southern Ethiopia from September 2006 to

Assessing the value-added contributions of community health workers and communities to early child development: a longitudinal study in a low-income setting.

BACKGROUND: Early child development (ECD) programs in low-resource settings can be effective when delivered through community health workers (CHWs), but there are significant challenges when moving to scale. This analysis aimed to determine the value-added, or relative effectiveness, of CHWs and communities on ECD outcomes within a home-visiting trial and examine associations between observable characteristics of the CHW or community and value-added. METHODS: We analysed data from the four treatment arms of a cluster-randomised trial conducted in 100 communities in rural Madagascar from 2014

Novel community health worker strategy for HIV service engagement in a hyperendemic community in Rakai, Uganda: A pragmatic, cluster-randomized trial.

BACKGROUND: Effective implementation strategies are needed to increase engagement in HIV services in hyperendemic settings. We conducted a pragmatic cluster-randomized trial in a high-risk, highly mobile fishing community (HIV prevalence: approximately 38%) in Rakai, Uganda, to assess the impact of a community health worker-delivered, theory-based (situated Information, Motivation, and Behavior Skills), motivational interviewing-informed, and mobile phone application-supported counseling strategy called "Health Scouts" to promote engagement in HIV treatment and prevention services. Efficacy of Communication Training of Community Health Workers on Service Delivery to People Who Inject Drugs in Vietnam: A Clustered Randomized Trial.

OBJECTIVES: To evaluate the efficacy of an intervention targeted to commune health workers (CHWs) who deliver services to people who inject drugs (PWID) in Vietnam. METHODS: From 2014 to 2016, we conducted a cluster randomized controlled trial of 300 CHWs and 900 PWID in 60 communes in 2 provinces of Vietnam. Intervention CHWs participated in training sessions to enhance their communication skills. Trained CHWs were asked to deliver individual sessions to PWID. We assessed the outcomes at baseline and at 3-, 6-, 9-, and 12-month follow-ups. RESULTS: Intervention CHWs showed greater improveme

The potential role of peripheral health workers and community key informants in the rapid assessment of community burden of disease: the example of lymphatic filariasis.

Data for planning disease control programs in most developing countries is often not available because they are usually expensive to gather. This study explored the potential use of peripheral health staff and community key informants in gathering community-level data about lymphatic filariasis. Agreement between findings of health workers and those of physicians was very high (kappa 0.66-0.87) for the clinical conditions examined. The prevalence of hydrocele was found to be a good predictor of communities at risk of filariasis. Community key informants provided very useful qualitative and qua

Performance of community health workers managing malaria, pneumonia and diarrhoea under the community case management programme in central Uganda: a cross sectional study.

BACKGROUND: Lay community health workers (CHWs) have been widely used to provide curative interventions in communities that have traditionally lacked access to health care. Optimal performance of CHWs managing children with malaria, pneumonia and diarrhoea in communities is desired if a reduction in childhood morbidity and mortality is to be achieved. This study assessed factors influencing performance of CHWs managing malaria, pneumonia and diarrhoea under the Integrated Community Case Management (iCCM) programme in Wakiso district, central Uganda. METHODS: A cross sectional study was conduc

Best Practices for Integrating Community Health Workers Into Care Teams and Organizations: Lessons From the Front Line.

This qualitative case study examined community health worker (CHW) integration within the CHW for COVID Response and Resilient Communities (CCR) program across 7 CCR recipient sites in the United States. The study questions addressed how CHWs are integrated into health organizations and care teams and facilitators and barriers to the integration of CHWs. From July to August 2023, we conducted 25 interviews with CHWs (n = 11) and program staff or partners (n = 14). Guided by the Consolidated Framework for Implementation Research (CFIR), we analyzed data thematically to identify the impact of fa

Psychological Agency Attenuates the Link Between Discrimination and COVID-19 Burnout in Latine-Serving Community Health Workers.

Community health workers that serve low income Latine families (Latine-serving CHWs) play a crucial role in reducing health disparities, yet frequently encounter discrimination, which may elevate their risk for burnout. Burnout not only harms these CHWs' wellbeing but also deprives communities of the full benefit of CHWs' service. Identifying factors that buffer against these effects, such as psychological agency, can offer valuable insights for supporting CHWs. This study examines (1) the association between discrimination and burnout among Latine-serving CHWs and (2) the role of agency as a

Evaluation of an obstetric and neonatal care upskilling program for community health workers in Papua New Guinea.

BACKGROUND: 60% of women in Papua New Guinea (PNG) give birth unsupervised and outside of a health facility, contributing to high national maternal and perinatal mortality rates. We evaluated a practical, hospital-based on-the-job training program implemented by local health authorities in PNG between 2013 and 2019 aimed at addressing this challenge by upskilling community health workers (CHWs) to provide quality maternal and newborn care in rural health facilities. METHODS: Two provinces, the Eastern Highlands and Simbu Provinces, were included in the study. In the Eastern Highlands Province

Perceptions of HIV transmission and pre-exposure prophylaxis among health care workers and community members in Rwanda.

There are too many new HIV infections globally with 1.8 million persons infected in 2016 alone. Pre-exposure prophylaxis (PrEP) holds potential to decrease new infections and is synergistic with efforts currently in place to achieve an end to the AIDS epidemic in Sub-Saharan African, but uptake is limited. Given its novelty, assessing the beliefs and attitudes of healthcare professionals and members of the community towards HIV transmission and PrEP will be helpful to inform implementation efforts. Study was a random survey of 201 community members and 51 healthcare providers, carried out at m

Antiretroviral therapy initiation and adherence in rural South Africa: community health workers' perspectives on barriers and facilitators.

South Africa has the largest global HIV/AIDS epidemic, but barriers along the HIV care continuum prevent patients from initiating and adhering to antiretroviral therapy (ART). To qualitatively explore reasons for poor ART initiation and adherence rates from the unique perspective of community health workers (CHWs), we conducted focus groups during May-August 2014 with 21 CHWs in rural Msinga, KwaZulu-Natal. Interviews were audio-recorded, transcribed, and translated from Zulu into English. Hybrid deductive and inductive analytical methods were applied to identify emergent themes. Multiple psyc
