Compartment syndrome of the foot--implications for military surgeons.

INTRODUCTION: Compartment syndrome of the foot is a rare complication of injury to the foot. Treatment by decompression of the compartments is debatable. The debate surrounding decompression stems from the rarity of the condition, the lack of consensus regarding the anatomy of the foot compartments and whether to accept the inevitable contractures by not decompressing. The aim of this paper is two fold; firstly to sample current military orthopaedic experience and secondly establish if there exists a consensus of opinion in how and if to perform fasciotomy of the foot thereby providing guidanc

Orbital compartment syndrome: the ophthalmic surgical emergency.

Orbital compartment syndrome is an uncommon, ophthalmic surgical emergency characterized by an acute rise in orbital pressure. When intraorbital tension rises, damage to ocular and other intraorbital structures, including irreversible blindness, may occur if not promptly treated. The diagnosis of orbital compartment syndrome is completely clinical and early recognition and emergent orbital decompression (even prior to imaging) is essential in preventing permanent vision loss. Lateral canthotomy and inferior cantholysis remain the mainstays of management. More extensive incision of the orbital

Diagnostic techniques in acute compartment syndrome of the leg.

OBJECTIVES: To review the efficacy of the current diagnostic methods of acute compartment syndrome (ACS) after leg fractures. DATA SOURCES: A Medline (PubMed) search of the English literature extending from 1950 to May 2007 was performed using "compartment syndromes" as the main key word. Also a manual search of orthopaedic texts was performed. STUDY SELECTION AND EXTRACTION: The results were limited to articles involving human subjects. Of 2605 primary titles, 489 abstracts limited to compartment syndromes in the leg and 577 articles related to the diagnosis of compartment syndromes were id

Myositis ossificans mimicking compartment syndrome of the forearm.

Myositis ossificans is a benign disorder of heterotopic bone formation occurring in response to soft tissue trauma. It most commonly presents in young adults and children as a painful, enlarging mass of the arm or thigh. Since there is frequently no significant history of injury, the lesion may be mistaken for a malignant bone or soft tissue tumor or infection. This article presents a case of a child in whom myositis ossificans presented with local signs and symptoms suggestive of a rapidly progressive compartment syndrome following trauma.

Compartment syndrome of the foot after calcaneal fractures.

BACKGROUND: A compartment syndrome is a condition in which high pressure within a closed fascial space causes reduced blood flow with nerve and muscle ischaemia. Untreated compartment syndrome leads to ischaemia with necrosis, contractures, myofibrosis and irreversible functional impairment in the involved part of the limb. Calcaneal fractures and other kinds of foot trauma may cause acute compartment syndrome of the foot. This condition may lead to foot deformities, including clawing of the toes, weakness of the foot muscles and, ultimately, a gait disorder. MATERIAL AND METHOD: We studied 1

[The abdominal compartment syndrome - widely unknown in gastroenterology].

A sustained increase in intra-abdominal pressure may derange cardiovascular haemodynamics, respiratory and renal functions and finally lead to multi-organ failure. It is primarily seen in surgical intensive care units and is most frequently associated with small and large bowel surgery, vascular surgery, and abdominal trauma. An expert panel has defined elevated intra-abdominal pressure > 20 mmHg in conjunction with newly occurring organ dysfunction as "abdominal compartment syndrome" (ACS). This entity is not well recognised in gastroenterology, although ACS may occur as a complication of end

Desflurane, malignant hyperthermia, and release of compartment syndrome.

We describe a case of suspected malignant hyperthermia in a healthy 20-yr-old man. The patient underwent urgent release of upper extremity compartment syndrome as a result of traumatic vascular injury. After 3 h of general anesthesia with desflurane, he developed a hypermetabolic state (hypercarbia, hyperthermia, hyperkalemia, and metabolic acidosis), consistent with the diagnosis of malignant hyperthermia. Cardiovascular instability coincided with reperfusion of the injured extremity. Treatment with dantrolene and supporting measures restored cardiovascular stability. Three days later he unde

Intra-abdominal hypertension and the abdominal compartment syndrome.

The pressure within the abdominal cavity is normally little more than atmospheric pressure. However, even small increases in intra-abdominal pressure can have adverse effects on renal function, cardiac output, hepatic blood flow, respiratory mechanics, splanchnic perfusion and intracranial pressure. Although intra-abdominal pressure can be measured directly, this is invasive and bedside measurement of intra-abdominal pressure is usually achieved via the urinary bladder. This cheap, easy approach has been shown to produce results that correlate closely with directly measured abdominal pressures

[Missed acute compartment syndrome of the lower leg].

After some considerable delay, acute compartment syndrome of the lower leg was diagnosed in a 17-year-old man, a 32-year-old man and a 76-year-old woman. The first patient was an accident victim whose leg was continuously bandaged for three days following treatment of a complicated femur fracture and lower leg wounds, so that inspection was not possible. In the second patient who was also an accident victim, the haemodynamics and the neurological condition initially required so much attention that the compartment syndrome was only noticed after 48 hours. The woman presented with a cold, painfu

Missed acute isolated peroneal compartment syndrome.

This paper outlines 3 cases of acute isolated peroneal (lateral) compartment syndrome following exertion, minor trauma or overuse. Compartment syndromes are usually associated with crush injuries or fractures; they are an uncommon development following minor trauma or overuse. In acute isolated peroneal compartment syndrome the diagnosis is often delayed, resulting in permanent impairment. Persistent or worsening pain following a minor injury or overuse is typical, and the initial physical findings are often nonspecific, although swelling and tenderness out of proportion to the described injur

Chronic exertional compartment syndrome of the legs in adolescents.

We present our experience in the management and treatment of chronic exertional compartment syndrome (CECS) of the legs in 23 adolescents. Twenty-one patients were affected in the anteroexternal compartment, one in the deep posterior compartment, and one in both. Seventeen of the patients were in-line competitive skaters; the rest practiced soccer or athletics. Mean age at the time of diagnosis was 16 (range, 14-18) years, and the age at the onset of symptoms was 15 (range, 13-17) years. After a careful differential diagnosis, the diagnosis was established on the basis of clinical signs and sy

Calcaneal compartment syndrome after tibial fractures.

Compartment syndrome of the newly discovered calcaneal compartment of the foot is a theoretical possibility following tibial fracture due to the communication with the deep posterior compartment of the calf. Forty-nine patients were reviewed at least 18 months after open or closed tibial shaft fractures treated with tibial nailing in order to determine the prevalence of foot deformities secondary to previously undetected calcaneal or leg compartment syndromes. Ankle movements, foot height, length of feet and degree of clawing of the toes were all measured and compared with the unaffected oppos

[Compartment syndrome or gas gangrene? A case report].

A case of forearm compartment syndrome due to a minor trauma is reported. The severe clinical conditions due to a shock state lead to an initial misdiagnosis of clostridial myonecrosis. The patient, 68 y-old woman, was admitted to the intensive care unit of Authors' hospital for a traumatic injury of the right forearm. A diagnosis of gas gangrene due to clostridial myonecrosis was done and forearm amputation suggested. In spite of this indication a decompression fasciotomy of the forearm compartments was performed as well as a hyperbaric oxygene therapy. Since culture of necrotic tissue sample

Abdominal compartment syndrome in a patient with congenital megacolon.

A 13-year-old male with a history of chronic congenital megacolon presented to the emergency department with a 1-day history of increasing abdominal pain, distension, and emesis. The patient was admitted for bowel disimpaction and irrigation. The patient rapidly developed an acute abdominal compartment syndrome because of his massive colonic dilation. Surgical decompression resulted in a reperfusion phenomenon and ultimately resulted in coagulopathy and patient demise. This case presents a unique cause of the abdominal compartment syndrome and discusses the implications to the emergency physic

[The compartment syndrome in injuries of the upper extremity in children].

In the Centre for Child Traumatology in Brno the authors followed up during the last 10 years 22 patients with suspected compartment syndrome of the upper extremity. Incisions of intrafascial spaces were made in 12 patients-9x on the forearm and 3x on the hand. Another 10 patients had conservative treatment. The therapeutic results are with the exception of one female patient with developed Volkmann's contracture very satisfactory, the sequelae are only cosmetic. Compartment syndrome is a serious acute condition in child traumatology. Permanent sequelae of this syndrome can be prevented above
