Reactive Balance Control Following Selective Dorsal Rhizotomy in Child With Diplegic Cerebral Palsy.

Cerebral palsy (CP) is the most common motor disability in early childhood characterized by impaired selective motor control. Children with CP often exhibit delayed and disorganized muscle activation in response to external perturbations, resulting in a high incidence of falls and decreased participation in activities compared with their peers. We examined changes in reactive balance control post-selective dorsal rhizotomy (SDR) in a child with CP in which the major goal of this surgery is to reduce lower extremity spasticity. A 7-year-old girl with spastic diplegic CP (Gross Motor Function Cl

Inattention, Impulsivity, and Hyperactivity in Deaf Children Are Not Due to Deficits in Inhibitory Control, but May Reflect an Adaptive Strategy.

The present study had two main aims: (1) to determine whether deaf children show higher rates of key behaviors of ADHD (inattentive, hyperactive, and impulsive behaviors) and of Conduct Disorder-CD-(disruptive, aggressive, or antisocial behaviors) than hearing children, also examining whether the frequency of these behaviors in deaf children varied based on cochlear implant (CI) use, type of school (regular vs. specific for deaf) and level of receptive vocabulary; and (2) to determine whether any behavioral differences between deaf and hearing children could be explained by deficits in inhibit

What Is the Relationship of Fear Avoidance to Physical Function and Pain Intensity in Injured Athletes?

BACKGROUND: Fear avoidance can play a prominent role in maladaptive responses to an injury. In injured athletes, such pain-related fear or fear avoidance behavior may have a substantial influence on the recovery process. Specifically, it may explain why some are able to reach their preinjury abilities, whereas others are unable to return to sport. QUESTIONS/PURPOSES: (1) Is fear avoidance in athletes associated with decreased physical function after injury? (2) To what degree is fear avoidance associated with athletes' pain intensity? METHODS: In a cross-sectional study, we recruited injured

Evaluating the use of prior information to individualise start item selection for the EORTC CAT core.

PURPOSE: Computerized adaptive tests (CATs) provide individualised measurement, using score estimates based on the patient’s prior responses to select the next most informative item. However, as no score estimate is available at the outset, the start item is typically not individualised. The European Organisation for Research and Treatment of Cancer (EORTC) CAT Core covers 15 health-related quality of life (HRQoL) domains. We explored whether scores from one domain could be used to obtain initial score estimates and hence, individualised start items for another domain, thereby improving measur

Theta-band oscillatory activity differs between gamblers and nongamblers comorbid with attention-deficit hyperactivity disorder in a probabilistic reward-learning task.

Problemgambling is thought to be comorbid with attention-deficit hyperactivity disorder (ADHD). We tested whether gamblers and ADHD patients exhibit similar reward-related brain activity in response to feedback in a gambling task. A series of brain electrical responses can be observed in the electroencephalogram (EEG) and the stimulus-locked event-related potentials (ERP), when participants in a gambling task are given feedback regardless of winning or losing the previous bet. Here, we used a simplified computerized version of the Iowa Gambling Task (IGT) to assess differences in reinforcement

Real-time surveillance to assess risk of intussusception and other adverse events after pentavalent, bovine-derived rotavirus vaccine.

BACKGROUND: A pentavalent, bovine-derived rotavirus vaccine (RotaTeq, Merck) was licensed in 2006 for use in infants. A previously licensed rotavirus vaccine was withdrawn due to elevated risk of intussusception. We prospectively evaluated the risk of intussusception and other pre-specified adverse events among RotaTeq recipients in the Vaccine Safety Datalink. METHODS: The exposed population included children from age 4 to 48 weeks who received RotaTeq between May 2006 and May 2008. Adverse events over the subsequent 30 days were ascertained from inpatient, outpatient, and emergency departme

Extracorporeal Blood Purification Therapy to Deal a Deferasirox Induced Life-Threatening Hepatic Encephalopathy in a Septic Child With Sickle-Cell Disease: A Case Report.

This report details a rare pediatric case of hyperammonemic encephalopathy caused by the oral iron chelating drug deferasirox (DFR) in a septic patient. It is our contention that this study lends support to the existing literature, as it describes the case of a 15-year-old female patient with a history of sickle-cell disease who presented with a fever and vomiting, rapid development of sleepiness, consciousness disturbances, medium mydriasis, neck stiffness, and trismus with seizure. Her Glasgow Coma Scale (GCS) score was 5. Laboratory tests revealed an increase in creatinine, metabolic acidos

Depression and Pain Interference Correlate With Physical Function in Patients Recovering From Hand Surgery.

Background: Patient-reported outcome measures vary more than expected based on underlying pathology, in part due to the substantial influence of mood and coping strategies. Methods: This study addressed the primary null hypothesis that the Patient-Reported Outcomes Measurement Information System (PROMIS) Physical Function score 1 month (range, 3-8 weeks) after hand surgery is not associated with PROMIS Depression and PROMIS Pain Interference scores prior to surgery, accounting for other factors. Using an institution-wide database of routinely collected patient-reported outcomes, we identified

Elevated Glucose Oxidation, Reduced Insulin Secretion, and a Fatty Heart May Be Protective Adaptions in Ischemic CAD.

BACKGROUND: Insulin resistance, β-cell dysfunction, and ectopic fat deposition have been implicated in the pathogenesis of coronary artery disease (CAD) and type 2 diabetes, which is common in CAD patients. We investigated whether CAD is an independent predictor of these metabolic abnormalities and whether this interaction is influenced by superimposed myocardial ischemia. METHODS AND RESULTS: We studied CAD patients with (n = 8) and without (n = 14) myocardial ischemia and eight non-CAD controls. Insulin sensitivity and secretion and substrate oxidation were measured during fasting and oral

Bright light exposure at night and light attenuation in the morning improve adaptation of night shift workers.

With practical applicability in mind, we wanted to observe whether nocturnal alertness, performance, and daytime sleep could be improved by light exposure of tolerable intensity and duration in a real work place. We also evaluated whether attenuating morning light was important in adaptation of real night shift workers. Twelve night shift nurses participated in this study. The study consisted of three different treatment procedures: Room Light (RL), Bright Light (BL), and Bright Light with Sunglasses (BL/S). In RL, room light exposure was given during the night shift and followed by 1 hr expos

Technical note: Characterization of novel iterative reconstructed cone beam CT images for dose tracking and adaptive radiotherapy on L-shape linacs.

BACKGROUND: Cone-beam computed tomography (CBCT) allows for patient setup and positioning, and potentially dose verification or adaptive replanning prior to each treatment delivery. Poor CBCT image quality due to scatter artifacts and patient motion has been a major limiting factor. A new image reconstruction algorithm was recently clinically implemented for improving image quality through iterative reconstruction (iCBCT). PURPOSE: This study aims to characterize iCBCT image quality, establish image value (HU)-to-relative electron density (RED) calibration curves for dose calculation, and ass

The influence of mental health on Patient-Reported Outcomes Measurement Information System (PROMIS) and traditional outcome instruments in patients with symptomatic glenohumeral arthritis.

BACKGROUND: The Patient-Reported Outcomes Measurement Information System (PROMIS) assessment includes computerized adaptive tests (CATs) that assess function, pain, depression, and anxiety. The influence of mental health on patients' self-reported pain and function has not been explored using PROMIS in patients with symptomatic glenohumeral osteoarthritis. METHODS: This cross-sectional study included 284 shoulders in 276 patients presenting with isolated glenohumeral osteoarthritis. All patients completed the American Shoulder and Elbow Surgeons (ASES) score, Simple Shoulder Test (SST), Visua

National Alzheimer's disease program.

The psycho-behavioral troubles of patients with an Alzheimer's Disease (AD) are such that it is very difficult to keep those patients within the normal family environment, and that it therefore proves necessary to have them placed in a specially adapted residence manned by a highly specialised medical and para-medical staff. Within the frame of such an approach, a new type of residence has been devised "Asclepios Gardens", a one-level ranch-type residence, including 44 individual apartments, each opening, on the one hand, on a little private garden opening itself on a large, strictly enclosed

A mathematical micturition model to restore simple flow recordings in healthy and symptomatic individuals and enhance uroflow interpretation.

We describe a model and report a new method to extract quantified data from the simple analysis of whole uroflow curves in healthy and symptomatic individuals. Recorded flow curves were compared with the curves theoretically predicted from a mathematical micturition model. This model was developed by relating each physiological event occurring during micturition to a set of mathematical equations. Due to improvements in speed of computer calculations, a very fast and adaptable mathematical micturition model became available. A total of 302 uroflow curves from 142 patients (61 men and 81 women)

Cascade Immune Mechanisms of Protection against Mycobacterium tuberculosis (IMPAc-TB): study protocol for the Household Contact Study in the Western Cape, South Africa.

BACKGROUND: Natural immunity against Mycobacterium tuberculosis exists, and > 90% of those infected remain disease-free. Innate and adaptive immune responses required to mediate such protection against tuberculosis (TB) are, however, poorly understood. METHODS: This is an analytical study exploring protective and non-protective pathways of immunity against Mycobacterium tuberculosis. Adults without HIV infection are recruited at community healthcare clinics in high TB incidence areas of the Western Cape Province, South Africa.
