[The treatment of severe dysplasias by CO2 laser: the respective indications for vaporization and conization. 84 cases].

Fifty-nine conisations and 25 vaporizations were carried out using laser CO2 in order to treat a series of 84 cases of severe dysplasias (CIN III). After a follow-up of 4 and 12 months (median period of observation 25 months) it was found that out of 74 patients that were followed up in this way 72 had benefitted from the use of the treatment. There were 5 cases however of recurrence of dysplasia (1 CIN I, 2 CIN II, 2 CIN III). These had further treatment with laser. Four of those were controlled properly, the fifth had to have surgery. Complications of treatment did not occur when vaporizatio

CO₂ laser ablation versus conization for cervical intraepithelial neoplasia grade 2: a prospective cohort study.

This study aimed to compare the efficacy of CO₂ laser ablation versus conization for treating cervical intraepithelial neoplasia grade 2 (CIN2) and to identify predictors of treatment failure. A prospective observational cohort study was conducted on 146 women with histologically confirmed CIN2. Participants were allocated to either CO₂ laser ablation (n = 100) or loop electrosurgical excision procedure (LEEP, n = 46) based on clinical decision. The primary outcome was short-term histopathological treatment response (regression to CIN1 or normal histology) at a 6-month follow-up. Propensity sc

Impact of preoperative conization on treatment outcomes in cervical cancer patients undergoing abdominal radical hysterectomy.

PURPOSE: The optimal surgical treatment for early-stage cervical cancer has been reevaluated in the light of the LACC trial, revealing poorer outcomes with minimally invasive surgery due to inevitable tumor spillage. Preoperative conization contributed to longer disease-free survival, improved overall survival, and reduced recurrence rates in patients undergoing minimally invasive radical hysterectomy (RH). This research evaluated the impact of preoperative conization on recurrence and survival in cervical cancer patients undergoing abdominal RH. METHODS: A study retrospectively analyzed 274

Predictive factors used to justify hysterectomy after loop conization: increasing age and severity of disease.

OBJECTIVE: To determine the interpretability and significance of the traditional factors used to predict residual dysplasia in hysterectomy specimens after loop conization. MATERIALS AND METHODS: Loop electrosurgical cervical conization was performed on 372 patients. Ninety three women had a hysterectomy within 6 months of the loop conization. Residual disease was defined as cervical intraepithelial neoplasia or cancer in the hysterectomy specimen. RESULTS: Of the 93 patients having a subsequent hysterectomy, 36 (38.7%) has residual disease in their hysterectomy specimen. The mean age of the

Research Progress of Immune Mechanisms Related to Persistent HPV Infection in CIN after Cervical Conization.

Persistent human papillomavirus (HPV) infection is associated with the grading of cervical intraepithelial neoplasia (CIN), high-risk HPV infection, multiple HPV infections, high HPV load, HPV infection of surgical margin, and age in CIN after conization. The immune mechanism is complex and is primarily related to vaginal microecology disorders, immune escape, immune response impairment, and the release of regulatory cytokines. Currently, the treatment methods for postoperative persistent HPV infection include surgical treatment, antiviral treatment, vaccination, and other approaches.

Prognostic Value of Human Papillomavirus Genotyping before and after Cervical Conization for Squamous Intraepithelial Lesion.

INTRODUCTION: We previously reported that preoperative human papillomavirus (HPV) type 16/18 positivity and postoperative high-risk (HR)-HPV test positivity are associated with abnormal postoperative cytology. In this study, we further examined whether preoperative and postoperative HR-HPV genotyping could predict cytological abnormalities and the risk of additional surgery. METHODS: Patients who underwent cervical conization at our hospital between July 2009 and June 2018 were enrolled. HPV genotyping was performed preoperatively for all patients with HPV-positive. The association among preo

Effective therapeutic strategy for massive retroperitoneal hematoma after conization: arterial embolization and pigtail catheter insertion.

The loop electrosurgical excision procedure (LEEP) is commonly used to remove cervical intraepithelial neoplasia (CIN) because of its safety profile and likelihood of fewer complications. The authors report a rare case of massive retroperitoneal bleeding combined with hypovolemic shock after LEEP conization. Vessel injury was detected by angiographic computed tomography (CT) and embolization of the uterine artery was successfully performed to achieve hemostasis by an intervention radiologist. A pigtail catheter was subsequently inserted for the drainage of the large retroperitoneal hematoma. T
