Contraception for midlife women.

Women wish to continue to be sexually active long after their desire to bear children. The risks associated with unintended pregnancy easily can be avoided by providing women with information about, and access to, contraceptives that are safe and effective. Because fertility naturally declines with age, different contraceptives may be more suitable for women toward the end of their reproductive years than were appropriate at periods of peak fertility. This article discusses the risks and benefits of various contraceptives for midlife women and the implications of this information on nursing pr

Intrauterine devices in adolescents.

PURPOSE OF REVIEW: The stubborn epidemic of teen pregnancy remains a major public health problem in the United States. This review discusses the use of intrauterine devices (IUDs) in United States teens as a potential strategy to reduce teen pregnancy. Traditionally, the IUD was not thought of as an appropriate teen contraceptive method. RECENT FINDINGS: Recently, IUDs have become a recommended contraceptive option for adolescent females. This change reflects an understanding of the safety of modern IUDs, the potential for this highly effective method to reduce teen pregnancy, and the promoti

A comparison between the vaginal ring and oral contraceptives.

OBJECTIVE: To investigate whether contraceptive vaginal ring use results in similar estimated genital symptoms, signs, examination, and laboratory findings compared with oral contraceptive use. METHODS: Women were randomly assigned to either contraceptive vaginal ring or a 20 microg ethinyl estradiol oral contraceptive pill use for 3 consecutive 28-day cycles, directly followed by 3 cycles of the study drug not initially assigned. Subjects scored genital symptoms on a daily diary using a 0-4 scale and underwent a baseline, cycle 2, cycle 4, and exit pelvic examination including vaginal discha

Racial and ethnic differences in men's knowledge and attitudes about contraception.

BACKGROUND: Little is known about racial/ethnic differences in men's contraceptive knowledge and attitudes. STUDY DESIGN: We used multivariable logistic regression to examine racial/ethnic differences in contraceptive knowledge and attitudes among 903 men aged 18-29 in the 2009 National Survey of Reproductive and Contraceptive Knowledge. RESULTS: Black and Hispanic men were less likely than Whites to have heard of most contraceptive methods, including female and male sterilization, and also had lower knowledge about hormonal and long-acting reversible methods. They were less likely to know t

Expanding Contraceptive Access for Teens-Leveraging the Pediatric Emergency Department.

PURPOSE: Adolescents have limited access to sexual healthcare services, and the emergency department (ED) may be the only place some will seek care. We implemented an ED-based contraception counseling intervention to assess intervention feasibility, and adolescent intention to initiate contraception, contraception initiation, and follow-up visit completion. METHODS: This prospective cohort study trained advanced practice providers in the EDs of two pediatric urban academic medical centers to deliver brief contraception counseling. A convenience sample of patients enrolled from 2019 to 2021 in

Contraception in the teenager. A comparison of four methods of contraception in adolescent girls.

The continuation rates, side effects and use-effectiveness of four types of contraceptive were studied during a two-year follow-up period in a group of 160 teenage girls of high socioeconomic standing who hd requested contraception advice. The method of contraception was selected by the girls after a discussion of the alternatives. Results showed good rates of continuation among girls selecting oral contraceptives, the intrauterine device (IUD) and condoms. Side effects and complications preclude the widespread use of oral contraceptives and the IUD in adoescents. There were no serious complic

An Educational Intervention to Raise Awareness of Contraceptive Options Among Young People.

Background: Young people in the United States know little about contraceptive options available to them, although method use is sensitive to individual preferences, and method switching is common. For young people to gain reproductive autonomy, a first step is to be aware of different contraceptives, including hormonal and nonhormonal methods. We tested whether an educational intervention delivered on community college campuses was effective in increasing contraceptive awareness. Materials and Methods: We developed a low-cost educational intervention featuring youth-friendly visual tools and t

The IUCD-associated incidence of Actinomyces israelii in the female genital tract.

Cervical swabs and in many cases also intrauterine contraceptive devices were examined from 973 women for the presence of Actinomyces israelii. It was detected in 11.6% of these women, the majority of whom were asymptomatic. The detection, however, of A. israelii in the female genital tract was associated with an almost 4-fold increase in the incidence of pelvic inflammatory disease (PID), indicating that colonization may progress to an infection in a small number of women. The incidence of A. israelii in the female genital tract of IUCD wearers was more than doubled when the IUCD was worn for

The Impact of Sexual Satisfaction, Functioning, and Perceived Contraceptive Effects on Sex Life on IUD and Implant Continuation at 1 Year.

INTRODUCTION: Contraceptives improve women's lives and public health, but many women discontinue their contraceptive method owing to dissatisfaction. An underexamined aspect of contraceptive discontinuation is sexual acceptability, or how contraception affects sexual experiences. Investigators' aims were two-fold: 1) to document changes in multiple domains of women's sexual experiences with their intrauterine device (IUD) or contraceptive implant over time and 2) to examine whether these sexuality factors were associated with method continuation at 12 months. METHODS: We enrolled 200 eligible

Few clinicians provide a wide range of contraceptive methods to Medicaid beneficiaries.

INTRODUCTION: Access to contraception services is an essential component of reproductive autonomy; however, low-income individuals frequently face barriers to care, including a shortage of providers who both accept Medicaid and offer contraception services. METHODS: We used 2021 Transformed Medicaid Statistical Information System Prescription and Other Service files to identify clinicians who provided contraception (prescription, implant, and/or intrauterine device (IUD)) to Medicaid beneficiaries. RESULTS: We identified 523 077 primary care and women's health specialty clinicians (obstetric

Prevalence, patterns and motives for hormonal contraceptive use among German servicewomen: results from a nationwide survey.

Hormonal contraceptive (HC) use is common among servicewomen and has been discussed in relation to health, performance, and operational readiness. No systematic data exist on HC prevalence among German servicewomen. This study analyzes contraceptive practices and motivations for contraceptive choice in German servicewomen. A nationwide, cross-sectional online survey was conducted among female members of the German Armed Forces. Recruitment occurred via military institutions and a digital flyer. The anonymous questionnaire (40 items, 10-15 min) assessed demographic characteristics and contracep

Intrauterine Device Migrated into the Urachus: A Case Report.

BACKGROUND: The intrauterine device is the most commonly used female contraceptive device, but the related complications of intrauterine devices are also common. Sometimes, intrauterine devices can cause uterine perforation, migrating into the abdominal cavity or other organs. At the same time, the intrauterine device may break into several small segments, migrate to distant organs, and even cause misdiagnosis. OBJECTIVE: This study assessed the role of laparoscopy in treating intrauterine device migration. DESIGN: This was a retrospective study involving in a review of a single case. SETTI

Expression profiles of antimicrobial peptides in the genital tract of women using progesterone intrauterine devices versus combined oral contraceptives.

PROBLEM: Sex hormones can influence the immune defenses of the female genital tract (FGT) and its susceptibility to infections. Here we investigated the effect of different hormonal contraceptives on the production of antimicrobial peptides (AMPs) in different compartments of the female genital mucosa (FGM), secretions and tissue. METHOD OF STUDY: Cervicovaginal secretions (CVS) and ectocervical tissue samples obtained from women using progesterone intrauterine devices (pIUD) (n = 23) and combined oral contraceptives (COC) (n = 23) were analyzed for the expression and in situ localization of

Management of contraceptives and menstrual complaints in patients with Down syndrome.

Enhanced health care for patients with Down syndrome (DS) results in improved overall quality of life and longer life expectancy. The main gynecologic complaints of patients with DS and their caregivers relate to menstrual cycles, hygiene and reproductive issues. Certain aspects, such as age of menarche, menstrual cycles, internal genitalia, and hormone profile are similar to those observed in the general population. However, individuals with DS may have a higher incidence of other disorders related to menstruation, such as hypothyroidism, epilepsy and use of anticonvulsants. Contraceptive mea
