A novel infraclavicular brachial plexus block: the lateral and sagittal technique, developed by magnetic resonance imaging studies.

UNLABELLED: A new infraclavicular brachial plexus block method has the patient supine with an adducted arm. The target is any of the three cords behind the pectoralis minor muscle. The point of needle insertion is the intersection between the clavicle and the coracoid process. The needle is advanced 0 degrees -30 degrees posterior, always strictly in the sagittal plane next to the coracoid process while abutting the antero-inferior edge of the clavicle. We tested the new method using magnetic resonance imaging (MRI) in 20 adult volunteers, without inserting a needle. Combining 2 simulated need

The musculocutaneous nerve and its branches to the biceps and brachialis muscles.

The musculocutaneous nerve and its motor branches to the biceps and brachialis were dissected and studied under the operating microscope in 24 fresh-frozen cadaveric specimens. The motor branch to the biceps exits from the musculocutaneous nerve at 119 mm distal to the coracoid process. Anatomic variations were seen in the innervation of the two heads of the biceps. A common primary motor branch that bifurcates to supply the two heads was seen in 20 specimens (type I). Two specimens were observed to have two separate primary branches originating from the main musculocutaneous nerve trunk to in

The results of the Boytchev procedure for treatment of recurrent dislocation of the shoulder.

Boytchev's operation for recurrent dislocation of the shoulder, in which three muscles attached to the coracoid process are re-routed deep to the subscapularis muscle and immediately adjacent to the gleno-capsular rent, is described. Seventeen patients have been reviewed after an average post-operative interval of six years. No recurrence of the dislocation had occurred and all patients were graded as excellent or good on objective examination although on subjective assessment one patient was considered only fair and the remainder good or excellent.

Quadruple Disruption of Superior Shoulder Suspensory Complex With Proximal Humerus Open Fracture-Dislocation: A Case Report.

CASE: A 34-year-old man was a restrained passenger involved in a high-speed rollover motor vehicle crash. The patient sustained a type 5 AC joint separation, severely comminuted intra-articular glenoid fracture with extension to the coracoid process base, displaced open scapular body fracture, a posterior shoulder dislocation of the glenohumeral joint, and a 2-part proximal humerus fracture. CONCLUSION: To our knowledge, this is the first report describing this injury pattern involving the superior shoulder suspensory complex with an associated open proximal humerus fracture-dislocation.

The rotator interval and long head biceps tendon: anatomy, function, pathology, and magnetic resonance imaging.

The rotator interval is an anatomically defined triangular area located between the coracoid process, the superior aspect of the subscapularis, and the anterior aspect of the supraspinatus. It is widely accepted that the rotator interval structures fulfill a role in biomechanics and pathology of the glenohumeral joint and long head biceps tendon. However, there is ongoing debate regarding the biomechanical details and the indications for treatment. A better understanding of rotator interval anatomy and function will lead to improved treatment of rotator interval abnormalities, and guide the in

Arthroscopic fixation of type III acromioclavicular dislocations.

Type III Acromio-Clavicular Joint dislocations can be treated successfully by surgical stabilisation in situ, with or without reconstruction of the coracoclavicular ligaments. The authors describe a simple and reliable mode of fixation, performed arthroscopically. The technique can be used for in situ fixation, or as part of an arthroscopically assisted Weaver and Dunn procedure. Using a metallic anchor loaded with a braided polyfilament suture, a strong and reliable fixation of the clavicle to the coracoid process is obtained. No hardware removal is necessary. Concomitant glenohumeral patholo

Musculocutaneous nerve: histotopographic study and clinical implications.

Surgical reconstruction of severe brachial plexus injuries includes nerve grafting and neurotization techniques of the musculocutaneous nerve (MCN) to recover elbow flexion. In treating recurrent anterior shoulder instability, knowledge of the topography of the MCN is important for the margin of safety available during dissection. The present study evaluates the origin and course of the MCN and its branches, and their relationships to bone landmarks. Twelve unembalmed cadavers (50-82 years old) were dissected. A histological study of the MCN and the coracobrachialis muscle (CB) was also carrie

How to salvage the fractured coracoid during the Latarjet Procedure? An empirical approach.

The Latarjet procedure is a frequently used stabilization procedure in case of anterior shoulder instability with critical glenoid bone loss and/or off-track Hill Sachs lesions. Although uncommon, intra-operative graft fractures do occur. When confronted with this potentially challenging intra-operative complication, having a secondary solution is paramount to achieve a successful outcome. This technical note provides a treatment algorithm that may function as a useful guideline to assist surgeons that experience this potentially complex unintended event during a Latarjet procedure. The Effects of Latarjet Reconstruction on Glenohumeral Kinematics in the Presence of Combined Bony Defects: A Cadaveric Model.

BACKGROUND: Recurrent glenohumeral instability is often a result of underlying bony defects in the glenoid and/or humeral head. Anterior glenoid augmentation with a coracoid bone block (ie, Latarjet procedure) has been recommended for glenoid bone loss in the face of recurrent instability. However, no study has investigated the effect of Latarjet augmentation in the setting of both glenoid and humeral head defects (Hill-Sachs defects). PURPOSE: To evaluate the glenohumeral kinematics of the Latarjet procedure in the presence of combined bony defects. STUDY DESIGN: Controlled laboratory study

Open Anatomic Coracoclavicular Ligament Reconstruction by Modified Conjoint Tendon Transfer for Treatment of Acute High-Grade Acromioclavicular Dislocation.

There is no ideal surgical technique for the treatment of acromioclavicular (AC) dislocations. Reconstruction of the coracoclavicular ligaments (CCLs) for the treatment of AC dislocations is evolving. Many techniques for CCL reconstruction have been described. They differ mainly in the method of fixation, number of tunnels, and graft used. The surgeon should select among hamstring autograft reconstruction, coracoacromial ligament transfer, and conjoint tendon transfer for CCL reconstruction. Early on, conjoint tendon transfer to the lateral clavicle was described for the treatment of high-grad

The treatment of recurrent anterior shoulder dislocation by the modified Bristow-Helfet procedure.

South African orthopaedic surgeons have been in the forefront of the development of reconstructive procedures performed for recurrent anterior shoulder dislocation. In addition to the Johannesburg stapling operation described by G. T. du Toit and D. Roux, Professor A. Helfet of Cape Town was the first to describe the coracoid transplantation operation in 1958. A modified form of this operation was performed on 31 patients between August 1975 and February 1981. Patients were followed up for a mean period of 38 months, and no recurrences were encountered in patients who had had true involuntary

Potential Utility of SPECT/CT for Early Detection of Scapular Stress Reaction Following Reverse Total Shoulder Arthroplasty.

Scapular stress fracture (SSF) after reverse total shoulder arthroplasty is a unique and common complication that may have negative impact for long-term outcomes. Scapular stress reaction (SSR), characterized by pain and tenderness without radiographic evidence of fracture, can be precursor of SSF. We believe that early detection using SPECT/CT with proper management for SSRs including acromion, scapular spine, and coracoid process is crucial for better prognosis without additional complications by preventing SSFs. Therefore, we present 3 typical cases with SSR following reverse total shoulder

[74-year-old man with a complex acromio-clavicular disjunction].

Acromioclavicular dislocation is a frequent pathology commonly encountered in traumatology. Therefore, its management is generally standardized, guided by clinical and radiological evaluation. This can range from conservative treatment by limb immobilization and functional rehabilitation, to surgical treatment by using minimally invasive techniques. We present the particular case of a 74-year-old patient with an acromioclavicular dislocation associated with a non-displaced fracture of the coracoid process as well as of the spine of the scapula. This article aims to describe the diagnostic trap

Recurrent anterior glenohumeral joint dislocation and torsion of the humerus.

Two computerized axial tomographic sections, one taken just below the coracoid process and the other about 2.5 cm proximal to the interepicondylar line, were used to determine glenoid version and humeral torsion in 19 patients with recurrent anterior glenohumeral joint dislocation and in 23 controls. Analysis of interobserver variation revealed the method to be reliable. There was no difference in glenoid version between the two groups. However, humeral torsion was greater in patients (153 degrees) than in controls (144 degrees). It appears that increased humeral torsion may predispose to glen

Osteolysis of the distal clavicle in a woman. Case report and review of the literature.

An unusual case of osteolysis of the distal clavicle in a woman is presented. Although almost 100 cases of osteolysis of the distal clavicle have been reported in the literature, none have occurred in females (Neer and Rockwood, 1984). After acute acromioclavicular dislocation, surgical reduction was carried out by transferring the coracoid process to the clavicle. Three years later the osteolysis of the outer clavicle appeared to be related to pain and functional impairment of the joint. The pain is quite tolerable and surgical excision of the distal clavicle has not yet been necessary.

Moment arms of the coracobrachialis and short head of biceps following a Latarjet procedure: a modeling study.

BACKGROUND: The Latarjet procedure transfers the coracoid process to the anterior glenoid. This prevents recurrent anterior humeral dislocation but alters the origins of the coracobrachialis (CBR) and short head of the biceps (SHB). The impact of this alteration on the moment arms of these muscles has not been examined. METHODS: The Newcastle Shoulder Model was updated with 15 healthy cadaveric bone models to create customized shoulder models. The CBR and SHB muscles were attached to the anterior glenoid via an elliptical wrapping object. Muscle moment arms were calculated for abduction, forw

Morphological data of the superior vena cava predicted by multiple linear regression equations.

This study explores the main surface markers in the hemodialysis puncture catheterization pathway and the relative spatial position of venous vessels in the puncture results, providing data support for venous puncture. Seventy cadavers were studied; angles of vertical axis with superior vena cava, left and right brachiocephalic vein at the confluence and circumference of superior vena cava were the primary parameters. Bivariate correlation analysis was used to determine in relation to other body parameters, and a regression equation was also obtained. The angle of the vertical axis with the su
