Anticipation of public speaking and sleep and the hypothalamic-pituitary-adrenal axis in women with irritable bowel syndrome.

BACKGROUND: Evidence suggests that subgroups of patients with irritable bowel syndrome (IBS) are hyper-responsive to a variety of laboratory stress conditions. METHODS: This study compared sleep quality and night time plasma adrenocorticotropic hormone (ACTH) and serum cortisol levels in response to anticipation of public speaking between 43 women with IBS and 24 healthy control women. In addition, comparisons were made between subgroups within the IBS sample based on predominant stool patterns, 22 IBS-constipation and 21 IBS-diarrhea. Subjects slept three nights in a sleep laboratory, and on

[Effects of caudal block in pediatric surgical patients: a randomized clinical trial].

BACKGROUND: Surgery generates a neuroendocrine stress response, resulting in undesirable hemodynamic instability, alterations in metabolic response and malfunctioning of the immune system. OBJECTIVES: The aim of this research was to determine the effectiveness of caudal blocks in intra- and postoperative pain management and in reducing the stress response in children during the same periods. METHODS: This prospective, randomized clinical trial included 60 patients scheduled for elective herniorrhaphy. One group (n = 30) received general anesthesia and the other (n = 30) received general anes

ADHD symptoms and diurnal cortisol in adolescents: The importance of comorbidities.

BACKGROUND: Altered regulation of diurnal cortisol has been associated with both dimensional symptoms and clinical diagnoses of attention deficit-hyperactivity disorder (ADHD). Indeed, a recent meta-analysis suggests that lower diurnal cortisol output may be a biomarker of attention deficit-hyperactivity disorder (ADHD); importantly, however, the influence of psychiatric comorbidities on this association has not been characterized. Approximately two-thirds of children with ADHD have at least one co-occurring neuropsychiatric condition, and altered HPA-axis function has been implicated in many

Cortisol is not associated with pre-treatment medial temporal lobe volume or volume changes after electroconvulsive therapy in patients with late-life depression.

Accumulating evidence suggests that late-life depression is associated with reduced hippocampal volume and that cortisol might be related to this volumetric reduction. We explored whether cortisol awaking response (CAR), which is the increase in cortisol after awakening, was associated with volumetric changes in the medial temporal lobe (MTL) after electroconvulsive therapy (ECT) in 41 patients (age ≥ 55) treated for major depressive disorder (MDD) with ECT. Cortisol was measured before the start of the ECT treatment and was related to MTL volumes derived from structural T1-weighted images. Th

Sex-specific association between the 5-HTT gene-linked polymorphic region and basal cortisol secretion.

OBJECTIVE: A key regulator of serotonergic neurotransmission is the serotonin transporter (5-HTT) and a common 5HTT gene promoter polymorphism, termed 5HTTLPR, is associated with phenotypes related to anxiety and depression. Furthermore, the serotonergic system influences hypothalamus-pituitary-adrenal (HPA) axis activity, which, in turn, is related to psychiatric diseases. METHODS: To explore the association between the 5-HTTLPR and HPA axis regulation we performed a detailed endophenotyping in 216 healthy subjects (all 126 females used oral contraceptives). RESULTS: While ACTH and cortisol

Exposure to particulate matter, prenatal depressive symptoms and HPA axis dysregulation.

BACKGROUND: The prevalence of depression during pregnancy is on the rise, affecting women's well-being and their children's health outcomes. Preliminary studies suggest that exposure to air pollution during pregnancy may play a role in development of depressive symptoms. In addition, pollution has been linked to dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, our brain's primary stress response system. The purpose of this study was to examine the association of air pollution exposure during pregnancy to prenatal depressive symptoms. We also evaluated whether cortisol, the hormo

Psychobiological stress and premenstrual symptoms across the menstrual cycle: protocol for an ecological momentary assessment (ISSAC) study.

INTRODUCTION: Premenstrual symptoms occur on a continuum from mild experiences to clinically significant, menstrual cycle-related mood disorders, including premenstrual dysphoric disorder (PMDD). Stress has been implicated as a key pathophysiological factor, yet the temporal coupling and dynamics between psychobiological stress patterns and premenstrual symptom expression across the menstrual cycle remain poorly characterised. The Interactions between premenstrual Symptoms and Stress Across the menstrual Cycle (ISSAC) study investigates how daily and momentary stress relates to premenstrual sy

Deranged emotional and cortisol responses to a psychosocial stressor in anorexia nervosa women with childhood trauma exposure: Evidence for a "maltreated ecophenotype"?

Exposure to trauma in the childhood and abnormal interpersonal stress reactivity are believed to contribute to the pathophysiology of anorexia nervosa (AN), which suggests a possible role of the hypothalamus-pituitary adrenal (HPA) axis. Although an effect of early traumatic experiences on the cortisol awakening response has been proved in patients with AN, the consequences of childhood trauma exposure on HPA axis reactivity to psychosocial stressors has been never investigated in such individuals. Therefore, we have assessed emotional and cortisol responses to an acute psycho-social stress in

Effects of prefrontal transcranial direct current stimulation on autonomic and neuroendocrine responses to psychosocial stress in healthy humans.

Prolonged or repeated activation of the stress response can have negative psychological and physical consequences. The prefrontal cortex (PFC) is thought to exert an inhibitory influence on the activity of autonomic and neuroendocrine stress response systems. In this study, we further investigated this hypothesis by increasing PFC excitability using transcranial direct current stimulation (tDCS). Healthy male participants were randomized to receive either anodal (excitatory) tDCS (n = 15) or sham stimulation (n = 15) over the left dorsolateral prefrontal cortex (DLPFC) immediately before and d

Cortisol Regulation among Women Who Experience Suicidal Ideation during Pregnancy.

BACKGROUND: Suicidal thoughts occur in up to one third of pregnant women. Suicidal ideation (SI) has been linked to hypothalamic-pituitary-adrenal (HPA) axis dysregulation in other populations and could underlie SI during pregnancy when the HPA axis undergoes gestational transformation. However, no studies have evaluated the HPA axis in prenatal suicide risk, including regulation of cortisol. We examined whether SI is associated with distinct features of cortisol regulation among women during the 3rd trimester of pregnancy. METHODS: Sixty-four women completed measures of SI and provided 8 sal

Sleep quality and the biological stress system during an internet-based intervention for major depressive disorder.

INTRODUCTION: Poor sleep quality is a persistent and debilitating symptom of major depressive disorder (MDD), with dysregulations in the biological stress system constituting a potential underlying physiological mechanism. Accordingly, a psychotherapeutic intervention may affect the interplay between sleep quality, MDD and the biological stress system. We examined how basal cortisol, and alpha-amylase levels correspond to perceived sleep quality during an internet-based intervention for MDD. Furthermore, we investigated how changes in sleep quality during the intervention relate to changes in t

Neuroendocrine signature of ME/CFS: Meta-analytic evidence for bioactive cortisol deficit and exaggerated feedback sensitivity.

Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a major clinical challenge as a complex multisystemic disorder with no well-established pathophysiological mechanism, characterized by persistent fatigue and post-exertional malaise, along with unrefreshing sleep, cognitive impairment, and impaired stress recovery. Despite decades of investigation into the hypothalamic-pituitary-adrenal (HPA) axis, a definitive neuroendocrine hallmark has remained elusive due to inconsistent findings across various cortisol matrices. Therefore, this systematic review and meta-analysis aimed to prov

Peer discrimination and diurnal cortisol output in a rural boarding school in China: Empirical findings and methodological considerations.

Adolescents' experiences of discrimination and their health consequences are understudied in non-Western cultures. Using data from 90 rural Chinese adolescents (Mage = 13.70; 49% female), this study examined cumulative and daily experiences of peer discrimination (based on socioeconomic status, gender, parental migration status, appearance, grades) and their associations with diurnal cortisol output. Data highlighted a high prevalence of peer discrimination, with 85% of the sample reporting any type of cumulative discrimination in the current semester and 56% of the sample reporting any type o

Association between Altered Cortisol Profiles and Neurobehavioral Impairment after Mild Traumatic Brain Injury in College Students.

Mild traumatic brain injury (mTBI) is the most common form of TBI, with more than 2.5 million TBI cases in the United States annually. Identification of easily obtainable biomarkers that track strongly with mTBI symptoms may improve our understanding of biological factors that contribute to mTBI symptom profiles and long-term outcomes. Notably, some individuals with mTBI exhibit circadian disruptions and elevated stress sensitivity, which in other clinical groups often correlate with disrupted secretion of cortisol, a glucocorticoid hormone that coordinates circadian and stress physiology. Association of pain intensity, pain-related disability, and depression with hypothalamus-pituitary-adrenal axis function in female patients with chronic temporomandibular disorders.

Patients with temporomandibular disorders (TMD) commonly experience myofascial and joint pain, pain-related disability, and other pain conditions including depression. The present study was carried out to explore the function of the hypothalamus-pituitary-adrenal (HPA) axis in relation to variables of the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) Axis II and comorbid depression in female patients with TMD. Cortisol and dehydroepiandrosterone (DHEA) levels were determined in saliva samples that had been collected at various periods after waking (0, 30, and 60min) an

Salivary cortisol awakening levels are reduced in human subjects with intermittent explosive disorder compared with controls.

BACKGROUND: The role of the hypothalamic-pituitary-adrenal (HPA) axis in human aggressive behavior is poorly characterized, though some studies report that, unlike depression, circulating or salivary levels of cortisol are low compared with controls. METHODS: In this study, we collected three salivary cortisol levels (two in the morning and one in the evening) on three separate days in 78 adult study participants with (n = 28) and without (n = 52) prominent histories of impulsive aggressive behavior. Plasma C-Reactive Protein (CRP) and Interleukin-6 (IL-6) were also collected in most study pa

Sedentary behavior and physical activity are longitudinally associated with diurnal cortisol rhythms in individuals up to 1 year after colorectal cancer treatment: A prospective cohort study.

BACKGROUND: Less robust diurnal cortisol rhythms are hypothesized to be associated with fatigue, depressive symptoms, psychological distress, sleep disturbances and poorer quality of life. Sedentary behavior and physical activity may influence cortisol rhythmicity, providing mechanistic leads for intervention strategies. We examined longitudinal associations of accelerometer-assessed sedentary behavior and physical activity with diurnal cortisol patterns among patients with colorectal cancer (CRC) up to 12 months post-treatment. METHODS: Among 74 CRC survivors, total and prolonged sedentary t
