Morphometric anatomy of the middle cranial fossa via the anterior petrosal (Kawase) approach: a bilateral cadaveric study.

BACKGROUND: The anterior petrosal (Kawase) approach is a well-established extradural corridor to the petroclival and upper clival regions; however, comprehensive bilateral morphometric data describing middle cranial fossa anatomy, particularly in Southeast Asian populations, remain limited. OBJECTIVE: To provide a comprehensive morphometric description of the microsurgical anatomy of the middle cranial fossa through the anterior petrosal (Kawase) approach based on bilateral cadaveric dissections. METHODS: A descriptive cadaveric study was performed on 21 formalin-fixed adult Vietnamese cadav

Incidence of intraoperative hearing loss during middle cranial fossa approach for repair of superior semicircular canal dehiscence.

INTRODUCTION: Superior semicircular canal dehiscence is a rare inner ear disorder characterized by an abnormal third opening between the superior semicircular canal and middle fossa. Symptoms include amplification of internal sounds, aural fullness, tinnitus, hearing loss, autophony, sound-induced vertigo (Tullio phenomenon), pressure-induced vertigo (Hennebert sign), disequilibrium, nystagmus, oscillopsia, and headache. While no cure exists for SSCD, surgical treatment has proven to effectively minimize these symptoms. This study reviewed brainstem auditory evoked potentials (BAEPs) that were

Hybrid Treatment Approach for a Rare Middle Cranial Fossa Intracranial Tumor in a Pediatric Patient: A Case Report.

Pediatric central nervous system (CNS) tumors are often classified by distinct histologic and molecular features; however, some tumors remain unclassified, resulting in diagnostic and therapeutic challenges. We report a case of a previously healthy 3-year-old female who presented with right eyelid ptosis and headache. Imaging revealed a right middle cranial fossa mass. Following surgery and histopathologic and molecular analyses, the diagnosis was a malignant neoplasm with mixed neural and myoblastic differentiation, not elsewhere classified based on the current World Health Organization (WHO)

Incidence and morphometry of sellar bridges and related foramina in dry skulls: Their significance in middle cranial fossa surgery.

PURPOSE: The current study investigated the incidence, morphology and morphometry of the ossified ligaments expanding between petrous bone and posterior clinoid processes and in between the anterior, middle and posterior clinoid processes. Side symmetry, gender dimorphism and age influence were also studied. MATERIALS AND METHODS: A total of 123 adult Greek dry skulls were observed. RESULTS: A caroticoclinoid bar (CCB) was found in 60.2%. Partial CCBs appeared more commonly (36.6%) than complete (23.6%). The caroticoclinoid foramen (CCF) was symmetrical on both sides and genders. An anterior

The efficacy of neuroendoscopic treatment for middle cranial fossa arachnoid cysts assessed by MRI 3D segmentation and modeling.

PURPOSE: The purpose of this study was to present a more precise and objective way to assess the effectiveness of neuroendoscopic application in the treatment of middle cranial fossa arachnoid cysts. METHODS: Between March 2009 and December 2012, 28 patients affected by middle cranial fossa arachnoid cysts were initially treated with endoscopic fenestration at the three spaces. The volumes of the cysts on MR images at the time of pre-op and 4 months after surgery were reconstructed by 3D Slicer and quantitatively calculated to compare the volumetric changes. The possible predisposing factors

Localization of the Internal Maxillary Artery for Extracranial-to-Intracranial Bypass through the Middle Cranial Fossa: A Cadaveric Study.

The internal maxillary artery (IMAX) is a promising arterial pedicle to function as a donor vessel for extracranial-to-intracranial (EC-IC) bypass procedures. The access to the IMAX through the anterior portion of the middle cranial fossa floor allows a much shorter interposition graft to be used to create a bypass to the ipsilateral middle cerebral artery and prevents a second incision in the neck. One of the challenges of this technique, however, is the difficulty to find the IMAX through an intracranial approach. The purpose of this cadaveric study is to establish a reliable method to local

Epidermoid cysts of the cerebellopontine angle with extension into the middle and anterior cranial fossae: surgical strategy and review of the literature.

Epidermoid cysts are tumours, which contain keratin, cellular debris and cholesterol, and lined with stratified squamous epithelium. Clinically, epidermoid cysts behave like benign, slow-growing lesions. We present a 63 year-old man with a 6-month history of right periorbital pain and hypaesthesia in the area of the first and second branch of the trigeminal nerve. MRI revealed an epidermoid cyst of the cerebellopontine angle extending into the middle and anterior cranial fossae. Radical surgical removal of epidermoid cysts should be attempted, but a less aggressive surgical strategy should be

[Preoperative embolization of the internal carotid artery in surgical treatment of meningioma in the base of the anterior and middle cranial fossae].

Preoperative embolization is a widely used technique that may decrease blood supply to rich-vascurized tumors of the brain. Its use allows a neurosurgeon to safely remove a tumor of different histological nature and site. The objects of preoperative embolization are most commonly meningiomas at convexital or parasagittal sites. According to the data available in the literature, the internal carotid or ocular artery was embolized in not more than 10 cases. The described case underwent successful embolization of the involved internal carotid whose branches took an active part in supplying blood

[Treatment of large tumors of the cerebellopontile angle using a combined transtentorial and translabyrinthine approach to the middle cranial fossa].

The Authors have reported on 7 patients with large tumors (larger than 4 cm) of the cerebello-pontine angle operated-on utilizing a subtemporal-transtentorial-translabyrinthine-trans-occipital approach. The technique, the results and complications of this approach to the cerebello-pontine angle are discussed. The advantages are the delineation of the cranial nerves and of the ventrolateral brainstem with its blood supply. This approach has been associated with a low morbidity and no operative mortality in 5 cases of acoustic nerve tumor and 2 meningiomas.

Subdural hemorrhage in middle cranial fossa arachnoid cysts: a report of two cases at two ends of the spectrum.

Arachnoid cysts are usually asymptomatic, benign lesions commonly occurring in the middle cranial fossa. However, the cysts may rupture in rare cases causing intracystic or subdural hemorrhages with significant mass effect. We report two cases of middle cranial fossa arachnoid cyst with subdural hemorrhage with very different clinical course. The first case presented with significant mass effect with cerebral herniation and had significant neurological morbidity post-surgery. The second case had minimal symptoms and was managed conservatively with offer of elective surgery. The report undersco

Endoscopic Transorbital Approaches to Anterior and Middle Cranial Fossa: A Laboratory Investigation on Surgical Anatomy and Potential Routes.

Background  Transorbital neuroendoscopic (TONES) approaches promise to open up new horizons for skull base surgery, offering alternative routes to reach the anterior and middle cranial fossa (ACF and MCF, respectively). Objective  The aim of this anatomical study is to acquire new surgical anatomy knowledge and exploit it for the refinement of TONES approaches, as an alternative to open surgery, to reduce the distance to the target, and the risk of neurovascular lesions in pathological conditions extending beyond the orbital cavity. Materials and Methods  Six head specimens (12 orbits) were st

Intracranial Malignant Nerve Sheath Tumor in the Middle Cranial Fossa: A Rare Case Report with Review of Literature.

Intracranial malignant nerve sheath tumors rarely arise in the central nervous system. They usually arise from the cranial nerves, though rare cases of intraparenchymal lesions have also been reported. We report a case of malignant nerve sheath tumor located in the right middle cranial fossa. Preoperatively, the lesion resembled a meningioma arising from the petrous temporal bone. The lesion was completely excised. Postoperatively, the patient developed righted-sided complete facial nerve palsy. Histopathology and immunohistochemistry revealed the lesion to be a malignant nerve sheath tumor. T

Sphenoid sinus mucocele penetrating to the orbit, anterior and middle cranial fossae and parapharyngeal space: a case report.

Mucocele, though is a common lesion, rarely penetrates to the surrounding intra- and extracranial spaces. We describe the case of 45-year-old male with 2 years history of a chronic left nasal obstruction and a concentric visual field deficit in the left eye as the only manifestations. Diagnostic CT and MRI imaging revealed a mucocele originating from the posterior ethmoid and sphenoid sinuses and penetrating intracranially to the anterior and middle cranial fossae and extracranially to the pterygopalatine fossa and the parapharyngeal space. This extensive localization appears to be an extremel

[A case of ruptured aneurysm of the middle cerebral artery associated with dural arteriovenous fistula in the anterior cranial fossa].

A 66-year-old man suffered from subarachnoid hemorrhage due to the rupture of a right middle cerebral artery aneurysm that was detected by 3D-CTA. He underwent an emergent clipping operation. Incidentally, postoperative DSA demonstrated dural arteriovenous fistula in the left anterior cranial fossa, which was fed by the left anterior ethmoidal artery and drained into the superior sagittal sinus via the cortical vein of the left frontal lobe. We performed electrocoagulation and division of the fistula at the second surgery. Subsequently, he underwent a ventriculo-peritoneal shunt and was discha

Dislocation of the mandibular condyle into the middle cranial fossa: report of a case with 5 year CT follow-up.

A 17-year-old patient suffered a dislocation of the right mandibular condyle into the middle cranial fossa following a motorcycle accident. Reduction was delayed because of serious orthopaedic injuries. The condyle was relocated into the glenoid fossa through a combined neurosurgical and preauricular approach. Five years later the patient presented for removal of wisdom teeth and a CT-scan was done to determine the effects of the trauma and management to the condyle. A fibro-osseous ankylosis had occurred in the right temporomandibular joint. The radiological changes confirm that early managem
