One burr-hole craniotomy: Upper retrosigmoid approach in helsinki neurosurgery.

BACKGROUND: In this video-abstract, we present a one burr-hole craniotomy for the upper retrosigmoid approach developed in Helsinki Neurosurgery to access the lateral cerebellar hemisphere, the cerebellopontine angle, and lateral skull base (e.g. including the posterior petrous bone). This approach may be utilized to manage tumors of the lateral posterior fossa and to perform microvascular cranial nerve decompression of the V or VII cranial nerves. The upper portion of the vertebral-posterior cerebral artery complex, and the anterior inferior cerebellar artery may also be accessed with this te

One burr-hole craniotomy: Modified presigmoid approach in Helsinki Neurosurgery.

BACKGROUND: In this video abstract, we present a one burr-hole craniotomy for a modified presigmoid approach developed in Helsinki Neurosurgery to access the space extended to both middle and posterior fossa. Thus, indications for this approach are lesions that extend to both middle and posterior fossa, petroclival tumors, basilar tip aneurysms located extremely low below the posterior clinoid process, trunk basilar aneurysms, and bypass procedures from the P2 segment of the posterior cerebral artery. The procedure is composed by three stages: a temporal and presigmoid craniotomy, a partial pe

Prone position craniotomy in pregnancy without fetal heart rate monitoring.

A pregnant patient in second trimester scheduled for posterior fossa craniotomy in prone position is a challenge for the anesthesiologist. Things to consider are physiological changes during pregnancy, non-obstetric surgery in pregnant patients, neuroanesthetic principles, effects of prone positioning, and need for fetal heart rate (FHR) monitoring. We have described the anesthetic management of this case and discussed intra-operative FHR monitoring including controversies about its role, indications, and various options available as per fetal gestational age. In our case we attempted intermit

Conservative treatment of severe limited mouth opening after transtemporal craniotomy.

Limited mouth opening (LMO) is a common complication of the transtemporal craniotomy that is often involved in the temporalis and temporomandibular joint. The occurrence of prolonged LMO was rare. Risk factors for such limitation after these neurosurgical procedures are defined, and the strategies of treatment and prevention are further discussed. This article presents 12 patients with severe LMO after transtemporal neurosurgical procedures. Four patients received myofunctional training combined with the neuromuscular blocking in the temporalis, which methods may be more effective for LMO withi

Critically ill cancer patients are not consistently hypercoagulable after craniotomy.

INTRODUCTION: Recent reports using thrombelastography have suggested that neurosurgical patients develop a hypercoagulable state in the postoperative period. Since venous thromboembolism is a potentially life threatening complication in these patients, we studied a similar population in our institution. METHODS: We conducted a prospective pilot study to evaluate postoperative coagulation changes in critically ill cancer patients after craniotomy. Data collected included demographics, diagnoses, severity of illness, all hematological information (coagulation tests included conventional and TEG

Pterional craniotomy without keyhole to supratentorial cerebral aneurysms: technical note.

BACKGROUND: Patients who have pterional craniotomy occasionally complain of scalp deformity at the frontotemporal area because of craniotomy site. Especially, this occurs as a result of inappropriate repair of the bony defect at the keyhole with the complex curvature of the surrounding bone, although burr holes buttons are used. The author presents results of pterional craniotomy that is performed without keyhole to supratentorial cerebral aneurysms. METHODS: The temporal muscle was incised a few millimeters before its insertion at the superior temporal line, leaving a small fascial cuff for

Vasoactive modulators during and after craniotomy: relation to postoperative hypertension.

Hypertension after craniotomy is frequent. To establish an association between vasoactive modulators and postoperative hypertension, we followed the arterial blood pressure and plasma concentrations of selected substances in patients undergoing craniotomy. Twelve consecutive patients scheduled for operation of a supratentorial brain tumor were anesthetized with thiopental, fentanyl, isoflurane, and pancuronium. None of the patients had a history of arterial hypertension or were hypertensive before the operation. Arterial blood pressure and heart rate measurements were obtained preoperatively,

Scalp nerve blocks decrease the severity of pain after craniotomy.

UNLABELLED: Up to 80% of patients report moderate to severe pain after craniotomy. In this study, we assessed the efficacy of scalp block for decreasing postoperative pain in brain surgery. Thirty patients scheduled for supratentorial craniotomy were enrolled. They were randomly divided into two groups: Ropivacaine (scalp block with 20 mL of ropivacaine 0.75%) and Saline (scalp block with 20 mL of saline 0.9%). Anesthesia was standardized. The scalp block was performed after skin closure and before awakening. Postoperative pain was assessed at 4, 8, 12, 16, 20, 24, and 48 h by using a 10-cm vi

Pseudoaneurysm of the superficial temporal artery following craniotomy--case report.

A 48-year-old male without marked blood coagulation disorder developed a pseudoaneurysm of the superficial temporal artery (STA) following craniotomy. Cerebral angiography revealed a pseudoaneurysm on the parietal branch of the STA. Total extirpation was performed, and the postoperative course was uneventful. Most pseudoaneurysms of the STA develop after trauma. Lesions rarely develop after craniotomy. However, pseudoaneurysm of the STA should be considered as a possible surgical complication.

Acute postoperative hydrocephalus following translabyrinthine craniotomy for acoustic neuroma resection.

Acute mental status changes following craniotomy for acoustic tumors demand prompt evaluation and treatment to avoid serious morbidity and mortality. Two cases of acute obstructive hydrocephalus complicating the postoperative period following translabyrinthine craniotomy are presented. Diagnosis is made with noncontrast computed tomography scanning. Treatment is rendered at the bedside with placement of a ventriculostomy. Diagnosis and management implications for acoustic tumor patients are discussed.

Cerebral healing after craniotomy to evacuate a Coenurus cerebralis cyst.

An 11-month-old ewe developed neurological signs caused by a space-occupying lesion in the left cerebral hemisphere. Successful surgical evacuation of a Coenurus cerebralis cyst was carried out, the neurological signs regressed and the ewe produced twins in the following breeding season. Sixteen months after the surgical treatment the ewe was found dead and necropsy revealed a vegetative endocarditis. In the brain, at the site of cyst evacuation, there was a collapsed subcortical cavity lined by siderotic fibrovascular tissue and surrounded by a modest glial reaction. These observations confir

[A case of multiple superficial temporal artery pseudoaneurysms following craniotomy].

A case of multiple superficial temporal artery (STA) pseudoaneurysms following craniotomy is reported and a review of the literature is made. The patient was a 17-year-old male who was hospitalized as he developed right hemiparesis 4 hours after golf ball injury to the left temporal region. He had been diagnosed as hemophilia A one year prior to the injury. CT scan showed right parietal intracranial hematoma with mass effect when he was submitted to emergency craniotomy. At surgery a linear scalp incision was made over the left temporal area after high factor VIII concentrates (con-VIII), 2,00

Predictors of Prolonged Hospital Stay After Craniotomy for Tumour Resection.

OBJECTIVES: Prolonged length of hospital stay (LOS) after craniotomy for tumour resection is associated with increased healthcare costs, postoperative complications, and resource utilization. Identifying predictors of prolonged LOS may facilitate perioperative optimization and improve patient outcomes. METHOD: A retrospective cross-sectional study was conducted on 390 adult patients who underwent craniotomy for brain tumour resection between January 2019 and December 2023 at a single tertiary care center, Aga Khan University Hospital, Karachi. Data was collected after ERC approval from May to

Lack of Insurance May Lead to Increased Mortality After Craniotomy.

BACKGROUND: Socioeconomic status (SES) directly impacts outcomes in health care. Prior studies have demonstrated the impact of insurance type and status on postoperative outcomes in subspecialty populations, including peripheral tumors and the spine. There is a paucity of literature addressing the impact of SES in craniotomy. This study investigates insurance status' impact on outcomes in craniotomy patients. METHODS: A single-center, retrospective cohort study was conducted on craniotomy patients from 2013-2018. Preoperative demographics, comorbidities, surgical variables, postoperative adve

Improvement in psychiatric symptoms after subdural haematoma and decompressive craniotomy.

This case study reports unexpected improvement in psychiatric symptoms following traumatic brain injury (TBI) in a man in his mid-30s with a complex psychiatric history, including autism spectrum disorder, paranoid schizophrenia and a personality disorder. He sustained a left-sided subdural haematoma requiring decompressive craniotomy. Post-surgery, he demonstrated resolution of longstanding hallucinations and self-harming behaviours, enhanced affective regulation and successful tapering of antipsychotic medications, alongside improvement in social and functional abilities. Although such impro

Post-operative day one discharge following craniotomy for tumour resection.

PURPOSE: Post-operative admission following craniotomy for tumour resection (CTR) remains standard practice. To improve efficiency, optimise patient flow, and reduce length of stay (LOS), a post-operative day one (POD1) discharge pathway was implemented. This study evaluated the feasibility of POD1 discharge and identified factors associated with delayed discharge and 30-day unplanned readmission. METHODS: A retrospective analysis of 263 consecutive patients undergoing CTR between November 2022 and December 2023 was performed. Baseline demographic, clinical, tumour-related, operative, and pos
