Acute subglottic laryngitis. Etiology, epidemiology, pathogenesis and clinical picture.

In about 3% of children, viral infections of the airways that develop in early childhood lead to narrowing of the laryngeal lumen in the subglottic region resulting in symptoms such as hoarseness, abarking cough, stridor, and dyspnea. These infections may eventually cause respiratory failure. The disease is often called acute subglottic laryngitis (ASL). Terms such as pseudocroup, croup syndrome, acute obstructive laryngitis and spasmodic croup are used interchangeably when referencing this disease. Although the differential diagnosis should include other rare diseases such as epiglottitis, di

Bacterial tracheitis.

Bacterial tracheitis is a recently described clinical entity characterized by high fever, stridor, and airway obstruction. Laryngoscopy shows membraneous inflammation with notable subglottic edema and copious mucopus in the trachea. Staphylococcus aureus is the most common bacterium to be cultured. Subglottic narrowing can be seen on lateral soft-tissue roentgenograms of the neck. Bacterial tracheitis should be treated by establishing a controlled airway by endotracheal intubation or tracheotomy, rather than by usual modalities used for the treatment of croup. Bacterial tracheitis seems to be

Infrared methodologies for the assessment of skin temperature daily rhythm in two domestic mammalian species.

To assess the accuracy of infrared methodologies for daily rhythm monitoring of skin temperature, five clinically healthy Italian Saddle gelding horses, and five not pregnant and not lactating Camosciata goats, were monitored every 4 h over a 48 h period. The horses were housed in individual boxes, while the goats in two indoor pens, under natural photoperiod and natural environmental temperature. In each animal, skin temperature was recorded with the use of a digital infrared camera and a non-contact infrared thermometer, in five regions: neck, shoulder, ribs, flank and croup. Recorded values

Clinical manifestations of human coronavirus NL63 infection in children in Taiwan.

Human coronavirus NL63 (HCoV-NL63) is a global respiratory tract pathogen; however, the epidemiology of this virus in subtropical area is not well known. To evaluate the epidemics and disease spectrum of HCoV-NL63 infection in children in Taiwan, we prospectively screened children admitted to the hospital with respiratory tract infection from May 2004 to April 2005. Every enrolled child had a nasopharyngeal aspirate (NPA) sample taken. Quantitative RT-PCR was used to detect 1b gene of HCoV-NL63. A total of 539 NPAs were collected. Seven (1.3%) were positive for HCoV-NL63. All cases were boys y

Fit and seal characteristics of a new paediatric tracheal tube with high volume-low pressure polyurethane cuff.

BACKGROUND: To evaluate a new paediatric tracheal tube (Microcuff, Weinheim, Germany) with an ultrathin high volume-low pressure polyurethane cuff. METHODS: With approval of the Hospital Ethics Committee tracheas of children undergoing general anaesthesia were intubated using a Microcuff tube. Tube sizes were selected according to: internal diameter (mm) = age/4 + 3.5 in children aged > or = 2 years. In newborns (> or = 3 kg) < or = 1 year, ID 3.0-mm tubes, and in children from 1 to 2 years, internal diameter 3.5-mm tubes were used. Tubes were classified too large if no air leakage was obtain

Adult Laryngotracheobronchitis in the Setting of a COVID-19 Infection.

We describe a case of adult croup in an 18-year-old female caused by the SARS-CoV-2 virus. Her complaints started as lower respiratory tract symptoms that evolved into stridor, barking cough, and dyspnea. The patient was diagnosed with SARS-CoV-2 by reverse transcription-polymerase chain reaction (RT-PCR) testing from a nasopharyngeal swab. The patient received multiple doses of nebulized racemic epinephrine with minimal improvement, and later the patient required mechanical ventilation. Intravenous remdesivir was administered for five days. Multiple doses of dexamethasone were required throug

Pulmonary infections with respiratory syncytial virus and the parainfluenza viruses.

Respiratory syncytial virus (RSV) and the parainfluenza viruses (PIVs) are the most important causes of acute lower respiratory illness (LRI) in infants and children under 6 years of age. These enveloped viruses are members of the paramyxovirus family. They infect cells in the epithelium lining the trachea and intrapulmonary airways, and cause croup, bronchitis, bronchiolitis, and bronchopneumonia. RSV causes annual midwinter to early spring outbreaks of respiratory disease in temperate climates; epidemics are heralded by the appearance of increased numbers of cases of bronchiolitis, primarily

Childhood Respiratory Conditions: Stridor.

The most important issue in diagnosing stridor in infants and children is determining whether it is acute or chronic. The most common cause of acute stridor is croup, a viral infection with a straightforward diagnosis when patients have typical signs and symptoms (ie, inspiratory or biphasic stridor, barking cough, hoarse voice or cry, chest wall retractions). Management of mild and severe cases includes steroids. Patients with severe croup should undergo evaluation in an urgent care center or emergency department; hospital admission may be required. When symptoms are atypical for croup, physi

Parainfluenza Virus Infection.

Human parainfluenza viruses (HPIVs) are single-stranded, enveloped RNA viruses of the Paramyoviridaie family. There are four serotypes which cause respiratory illnesses in children and adults. HPIVs bind and replicate in the ciliated epithelial cells of the upper and lower respiratory tract and the extent of the infection correlates with the location involved. Seasonal HPIV epidemics result in a significant burden of disease in children and account for 40% of pediatric hospitalizations for lower respiratory tract illnesses (LRTIs) and 75% of croup cases. Parainfluenza viruses are associated wi

Association between follicular tracheitis and gastroesophageal reflux.

OBJECTIVE: Follicular tracheitis (also known as tracheal cobblestoning) is an entity that is poorly described and of unclear significance. The objective of this study was to better define follicular tracheitis and determine the association between the clinical finding of follicular tracheitis on bronchoscopy and objective evidence of gastroesophageal reflux disease. METHODS: Retrospective chart review of children with recurrent croup having undergone a rigid bronchoscopy and an investigation for gastroesophageal reflux between 2001 and 2013. RESULTS: 117 children with recurrent croup childre

Kawasaki disease associated with parainfluenza type 3 virus infection.

Kawasaki disease (KD) is an acute, febrile and multisystem vasculitis of early childhood with a striking predilection for the coronary arteries. In developed countries, the incidence of KD has replaced acute rheumatic fever as the leading cause of acquired heart disease in children. The etiologic agent of KD remains unknown, although clinical and epidemiologic features strongly indicate an infectious cause. Parainfluenza viruses are the major cause of laryngotracheobronchitis (croup), but they also commonly cause upper respiratory tract infection, pneumonia, or bronchiolitis. Types 1 and 2 vir

The clinical significance of measles: a review.

Forty years after effective vaccines were licensed, measles continues to cause death and severe disease in children worldwide. Complications from measles can occur in almost every organ system. Pneumonia, croup, and encephalitis are common causes of death; encephalitis is the most common cause of long-term sequelae. Measles remains a common cause of blindness in developing countries. Complication rates are higher in those <5 and >20 years old, although croup and otitis media are more common in those <2 years old and encephalitis in older children and adults. Complication rates are increased by

Membranous laryngitis in a child.

The most common etiologies for acute infectious airway obstruction include epiglottitis, croup, and bacterial tracheitis. We present a unique cause of upper airway obstruction in a child not previously described. To our knowledge this is the first case in the literature of membranous laryngitis in a child due to Methicillin-resistant Staphylococcus Aureus (MRSA). The diagnosis was made by endoscopy and culture and treated with culture directed antibiotics and debridement of membranes from the larynx. The patient did not present with clinical symptoms consistent with epiglottitis as the disease

[Diagnosis of pharyngeal and laryngeal diphtheria in adults].

142 adults with pharyngeal and laryngeal diphtheria were registered in Yekaterinburg in 1993. Of them, 136 patients recovered and 6 died. The analysis of the clinical features and diagnostic errors showed that subjects with chronic visceral diseases and alcoholics are more likely to have poor prognosis of diphtheria. Diphtheria manifested primarily with edema followed by infiltration of the mucosa, with appearance of thin transparent furs, tenderness of cervical regional lymph nodes. The typical greyish-brown thick and firm false membranes occurred only in the late stage of the disease. The fe

[Stenosing laryngotracheitis (pseudocroup) and sulfur dioxide content of the air. Results of an 18-year retrospective long-term study of possible spatial and temporal relations].

Based on the immission levels measured in the city of Bochum (approx. 20,000 readings), 1200 hospitalized children with pseudocroup were examined for a possible relationship between croup incidence and sulphur dioxide levels. Evaluation according to geographic location showed no statistically significant correlation between croup incidence and SO2 levels over a test period of 13 years. Only for 3 of 13 years did Spearman's rank correlation coefficient show a weakly significant positive correlation (P less than 0.05). In one test year, however, there was a statistically significant negative cor
