Relapsing infiltrates after pneumocystis pneumonia in stem cell transplant patients: think about BOOP!

Bronchiolitis Obliterans Organizing Pneumonia (BOOP) can complicate allogeneic haematopoietic stem cell transplantation. It is associated with prior graft-versus-host disease (GVHD) and the case fatality is 21%. In 22%, diagnosis is preceded by tapering the corticosteroids given as a treatment for GVHD. We report a fatal case of BOOP after tapering the corticosteroids that the patient received for a Pneumocystis jirovecii pneumonia after stem cell transplantation.

Chemotherapy-induced pulmonary complications in cancer: Significance of clinicoradiological correlation.

Chemotherapy while revolutionizing cancer management by improving survival and quality of life; is also associated with several adverse effects. Lung is the most common organ affected in chemotherapy-related complications, due to either drug toxicity or more commonly due to infections caused by immunosuppression and less commonly due to immune-mediated injury. Radiology, when used in combination with clinical and lab data, can help reach the specific diagnosis or narrow down the differentials. The common radiological patterns of drug toxicity include pulmonary interstitial and airway infiltrat

Reversal of Cerebral Glucose Hypometabolism in a Patient with COVID and Refractory B-Cell Lymphoma.

Cerebral glucose hypometabolism (CGHM) involves reduced brain glucose uptake on FDG-PET imaging and occurs in neurological and systemic diseases. Its reversibility remains uncertain. We describe a 40-year-old man with reversible global CGHM associated with cryptogenic organizing pneumonia (COP) and COVID-19. His neurocognitive symptoms improved rapidly following steroid-induced hyperglycemia, with FDG-PET demonstrating complete CGHM resolution after two months. His medical background included primary refractory transformed follicular lymphoma diagnosed 30 months earlier, managed with multiple

Concurrence of nivolumab-induced interstitial lung disease and cancer invasion.

Nivolumab improves overall survival rates of patients with advanced or recurrent non-small-cell lung cancer (NSCLC). Among immune-related adverse events caused by nivolumab, interstitial lung disease (ILD) is a clinically serious and potentially life-threatening toxicity, for which appropriate treatment is needed immediately. However, ILD is sometimes difficult to distinguish from invasive lung adenocarcinoma using only computed tomography (CT) findings. A 71-year-old man was diagnosed with advanced lung adenocarcinoma. The patient developed dyspnoea after eight cycles of nivolumab, when chest

Secondary organizing pneumonia following occupational acute nitrogen oxide poisoning: a case report.

Secondary organizing pneumonia (SOP) is a nonspecific inflammatory response towards acute lung injuries caused by various diseases. However, organizing pneumonia (OP) secondary to occupational acute nitrogen oxide poisoning has been reported rarely. We report a 49-year-old man who suffered from nitrogen oxide poisoning after inhaling mixed gas at work. After pathological examination, he was diagnosed with OP. In the absence of other underlying factors causing OP, he was diagnosed with SOP owing to acute nitrogen oxide poisoning. After systematic treatment, the patient recovered and was dischar

[Bronchiolitis obliterans with organized pneumonia: a rare complication of primary Gougerot-Sjögren syndrome].

INTRODUCTION: Bronchiolitis obliterans organizing pneumonia (BOOP) is characterized by plugs of granulation tissue in bronchioles, alveolar ducts and alveoli. This pulmonary disorder has been reported in some cases in relation to drug consumption (D-penicillamine, amiodarone), with bacterial or viral infections (Mycoplasma pneumoniae, HIV), and with systemic diseases, such as rheumatoid arthritis. To our knowledge, only three cases of association BOOP-Sjögren's syndrome have been reported. EXEGESIS: We report three new cases of BOOP. These patients presented a primary Sjögren's syndrome witho

Exhaled nitric oxide measurements in patients with acute-onset interstitial lung disease.

It is important to identify the underlying cause of acute-onset interstitial lung disease (ILD). This study aims to assess whether there are differences in the exhaled nitric oxide (eNO) level between different subtypes of acute-onset ILD. Forty patients with a combination of illness ≤4 weeks in duration and diffuse radiographic infiltrates were classified into groups based on the etiology. The eNO at a flow rate of 50 ml s-1 (FeNO), the alveolar nitric oxide concentration (Calv), and the systemic inflammatory markers of the groups were compared. The median FeNO value of patients with acute eo

A Prospective Cohort Study Comparing Long-Term Outcomes with and without Palifermin in Patients Receiving Hematopoietic Cell Transplantation for Hematologic Malignancies.

The incidence of debilitating oral mucositis (OM) can be as high as 99% after myeloablative conditioning regimens preparing patients with hematologic malignancies for hematopoietic cell transplantation (HCT). Palifermin (KGF) is a recombinant human keratinocyte growth factor that reduces the incidence and duration of severe OM. The long-term safety of KGF has not been well established, however. In this long-term prospective matched-cohort study, patients who received KGF (cases) and underwent autologous or allogeneic HCT for hematologic malignancies between 2006 and 2013 were matched 1:1 to pa

Long-term outcome of pyogenic liver abscess: factors related with abscess recurrence.

BACKGROUND/AIMS: Long-term surveillance of pyogenic liver abscess remains unavailable. We thus aimed to identify the recurrence rates of pyogenic liver abscess among various etiologies and pathogens, and to elucidate the factors related with this recurrence. METHODS: Six-hundred and one patients with pyogenic liver abscess were prospectively enrolled to observe abscess recurrence during a mean follow-up period of up to 6.06 years. On the basis of the etiology of the initial abscess, patients were divided into different subgroups as follows: there were 152 (25.3%) patients classified as crypto

Tuberculosis heralding connective tissue disorder.

Connective tissue diseases and infections are amongst the causes for organising pneumonia. However, organising pneumonia preceding other connective tissue disease manifestations is rare. Mycobacterium tuberculosis is rarely associated with organising pneumonia. We report such a case. A 50-year-old diabetic male, a roadside shop keeper, a current smoker presented with fever, breathlessness, cough and weight loss for four months. Chest radiography demonstrated areas of consolidation with halo signs. Anti-nuclear antibody blot was positive for Scl-70 and Jo-1 suggestive of a syndrome of systemic

Bronchiolitis obliterans with organizing pneumonia associated with acute Coxiella burnetii infection.

We report the case of a 62-year-old man who developed bronchiolitis obliterans with organizing pneumonia (BOOP) associated with acute Coxiella burnetii infection. The diagnosis of BOOP was made by fiber-optic transbronchial biopsy. Treatment with corticosteroids resulted in rapid clinical improvement and complete resolution of airspace opacities. To the best of our knowledge, this is the first well-documented case of BOOP associated with C. burnetii infection. This case suggests that this infection might trigger the development of BOOP.

Non-infectious and unusual infectious mimics of community-acquired pneumonia.

Noninfectious or unusual infectious diseases may present with clinical, radiological and laboratorial characteristics of community-acquired pneumonia (CAP). Usually their presence is only suspected after treatment failure, leading to inappropriate interventions, unnecessary costs and risks related to the untreated potentially life-threatening disease. The present study aimed to assess the noninfectious or unusual infectious diseases that may be misdiagnosed as CAP that progresses with treatment failure. Sixteen hospitalized patients with presumptive diagnosis of CAP and treatment failure were

Asymptomatic pulmonary cryptococcosis presenting with mixed lesions of infiltrative and nodular shadow in an immunocompromised patient.

Pulmonary cryptococcosis (PC) is a rare fungal lung infection that usually occurs in immunocompromised hosts. We report a rare case of PC with asymptomatic and mixed lesions of infiltrative and nodular shadows. A woman in her 50s with a 7-year history of methotrexate treatment for rheumatoid arthritis (RA) presented with abnormal chest shadows on annual examination. Chest CT revealed two tiny nodules and two infiltrative shadows in the right lower lobe of the lung. We suspected lung cancer, cryptogenic organising pneumonia or RA-interstitial lung disease. However, transbronchial lung biopsy an

Primary Sjögren's Syndrome Presenting as Flaccid Quadriparesis.

Primary Sjögren's syndrome is an autoimmune disease presenting classically as keratoconjunctivitis sicca. Renal involvement in Sjögrens's syndrome is less common, and the initial presentation with renal complications without any sicca symptoms is extremely rare. The renal involvement may present with symptoms arising from interstitial nephritis, mainly distal renal tubular acidosis.

Obliterative bronchiolitis in a 13-year-old pre-transplant cystic fibrosis patient.

Cystic fibrosis (CF)-related lung disease is characterized by a broad spectrum of pathologic changes. Most of these changes relate to progressive bronchiectasis and airway destruction due to recurrent infections. Other airway pathologies include but are not limited to nasal polyposis, bronchial hyperactivity, pneumothorax and allergic bronchopulmonary aspergillosis (ABPA). Constrictive obliterative bronchiolitis (COB) in CF is usually seen in post-transplant settings including lung and heart-lung transplants. We report a 13-year-old boy with CF and intractable deterioration in lung function th

[Primary pulmonary cryptococcosis exhibiting the radiological characteristics of bronchiolitis obliterans organizing pneumonia].

A 31-year-old male was admitted to Toranomon Hospital because of a cough and bilateral patchy infiltrates shown on the chest radiograph. He had been well prior to admission. Chest CT scan revealed patchy areas of air-space consolidation with air-bronchogram and adjacent ground-glass opacities, suggestive of bronchiolitis obliterans organizing pneumonia (BOOP). Transbronchial lung biopsy specimen confirmed the formation of epithelioid cell granulomas without necrosis and the coexistence of organizing pneumonia. The titer of serum cryptococcal antigen increased to 1:256. According to these findi

Bronchiolitis obliterans organizing pneumonia cured by standard dose inhaled triamcinolone: the first documented case.

Bronchiolitis obliterans organizing pneumonia (BOOP) is characterized clinically by progressive cough, fever, and dyspnea and pathologically by plugging of the bronchiolar and alveolar lumen with buds of loose connective tissue containing fibroblasts and inflammatory cells. The radiographic appearance of the disease varies. Definitive diagnosis requires the histologic identification of the disease in the appropriate clinical setting. Tissue may be obtained by bronchoscopy, needle biopsy, or open lung biopsy. BOOP is responsive to oral corticosteroids, which are the mainstay of therapy. We repo
