Bilateral cryptorchidism treatment results are often shadowed by the majority of unilateral cases. We report the long-term follow-up results of boys treated for bilateral cryptorchidism during childhood. Patients treated in two main paediatric surgery centres were selected from medical registries and invited for a clinical examination including scrotal ultrasound, salivary testosterone measurement and a semen sample. The term cryptorchidism is related to the failure of the migration of the testis to the scrotum. In most cases, testis are retained along the physiological route through the inguinal canal. In 1% of cases the gubernaculum testis is abnormally fixed (testicular ectopy).In 20% of cases, one testis is not clinically palpable. The US has a sensitivity of 45% and a specificity of 78% in detecting intra-abdominal testis. Artificial cryptorchidism or local testicular heat treatment can induce reversible oligospermia or azoospermia in monkeys and rats via germ cell apoptosis. Local warming of monkey testes in water at 43°C for 2 consecutive days (30 min per day) decreased the number of sperm in the semen by up to 80% on d 28, and the effect was completely reversed on d 144. BACKGROUND: It has been suggested that maternal smoking during pregnancy is a risk factor for low sperm counts and testicular cancer in the offspring. Cryptorchidism is associated with both of these disorders and might share causal mechanisms.

A retrospective study was performed 2.60 +/- 1.14 years after ochidopexy on 124 patients with cryptorchidism. Complex spermatological, hormonal, and histomorphological investigations were carried out which demonstrated the existence of disturbances in the exo- and endocrine functions of the testes of these patients. The results juxtaposed to the ages when the orchidopexy was accomplished revealed the necessity of early surgical intervention prior to 5 years of age. After this age there were severe changes in the histomorphological characteristics of the testicular tissue.
