The effect of preoperative lumbar epidural corticosteroid injection on postoperative infection rate in patients undergoing single-level lumbar decompression.

BACKGROUND CONTEXT: Lumbar epidural corticosteroid injections (LECIs) are frequently used in the treatment of lumbar intervertebral disc herniation with radiculopathy and lumbar spinal stenosis. Although widely used, their effect on the outcomes and complications of subsequent surgery is unclear. Postoperative infection can be a morbid complication following spine surgery, and recent literature has suggested that the risk may be increased in patients undergoing lumbar spinal surgery who had previously received LECIs. PURPOSE: The purpose of this study is to define the overall postoperative in

Editorial Commentary: Bio-Inductive Collagen Implant Augmentation Shows Long-Term Cost-Effectiveness, But Clinical Patient Outcomes and Careful Patient Selection Must Guide the Path Forward.

Arthroscopic rotator cuff repairs (ARCR) are fraught with low healing rates despite improvements in surgical techniques and constructs. Several studies have emerged showing significant improvements in failure to heal rates when incorporating bioinductive collagen implants (BCI) in the short term. Structural integrity following ARCR is paramount, as retear places exorbitant costs on the health care system and long-term studies have established that clinical outcomes are significantly worse in patients with structural retear. The up-front costs of biologic augmentation is cost-prohibitive in amb

Surgical Margin Mapping of Melanoma In Situ Using In Vivo Reflectance Confocal Microscopy Mosaics.

BACKGROUND: Melanoma in situ (MIS) can have poorly defined borders and subclinical extension that makes margin control challenging. Reflectance confocal microscopy (RCM) is a promising noninvasive technique that can be used to assess subclinical spread. OBJECTIVE: To optimize surgical margins of histology-proven MIS using RCM mosaics. MATERIALS AND METHODS: Prospective review of 22 patients with histology-proven MIS who underwent RCM margin mapping prior to staged excision, between August 1, 2018, and August 13, 2020, at the Department of Dermatology, University of New Mexico, School of Medi

Treatment of the ventral intermediate nucleus for medically refractory tremor: A cost-analysis of stereotactic radiosurgery versus deep brain stimulation.

INTRODUCTION: Medically refractory tremor treatment has evolved over the past half-century from intraoperative thalamotomy to deep brain stimulation (DBS) of the thalamic ventral intermediate nucleus (VIM). Within the past 15years, unilateral radiosurgical VIM thalamotomy has emerged as a comparably efficacious treatment modality. METHODS: An extensive literature search of VIM DBS series was performed; the total cost of VIM DBS was calculated from hospitals geographically representative of the entire United States using current procedural terminology and work relative value unit (RVU) codes.

Mapping Nerve Transfer Adoption in Brachial Plexus Birth Injury Reconstruction, 2005-2024.

PURPOSE: Brachial plexus birth injury (BPBI) affects up to 3 per 1,000 live births, with many children experiencing persistent disability. Although autologous nerve grafting has traditionally served as the primary reconstructive strategy for BPBI, nerve transfers are being used with increasing frequency. We analyzed national trends in nerve reconstruction for BPBI using the Pediatric Health Information System administrative database from 2005 to 2024. METHODS: We used Current Procedural Terminology (CPT) codes associated with nerve autografting or nerve transfer to identify patients in the Pe

Medicare billing and utilization trends in vascular surgery.

OBJECTIVE: Within the past decade, Medicare Part B reimbursements for various surgical procedures have been declining, whereas health care expenses continue to increase. As a result, hospitals may increase service charges to offset losses in revenue, which may disproportionately affect underinsured patients. Our analysis aimed to characterize Medicare billing and utilization trends across common vascular surgical procedures. METHODS: The 2017 to 2021 Medicare Physician and Other Practitioners by Provider and Service dataset was queried for Current Procedural Terminology (CPT) codes for common

Association of COVID-19 Vaccination With Changes in Smell and Taste.

Since the introduction of vaccines for severe acute respiratory syndrome coronavirus 2 in the United States, there has been significant vaccine hesitancy, in part due to fear of adverse effects. We sought to investigate the rates of smell and taste changes after COVID-19 vaccination compared to other common vaccines. Our study cohort included individuals identified by Current Procedural Terminology code in the TriNetX database receiving the COVID-19 first series, COVID-19 booster, influenza, tetanus, diphtheria, pertussis (TDAP), or pneumococcal vaccines between December 15, 2020, and August 1

Brachial Arterial Pressure Monitoring during Cardiac Surgery Rarely Causes Complications.

BACKGROUND: Brachial arterial catheters better estimate aortic pressure than radial arterial catheters but are used infrequently because complications in a major artery without collateral flow are potentially serious. However, the extent to which brachial artery cannulation promotes complications remains unknown. The authors thus evaluated a large cohort of cardiac surgical patients to estimate the incidence of related serious complications. METHODS: The institutional Society of Thoracic Surgeons Adult Cardiac Surgery Database and Perioperative Health Documentation System Registry of the Clev
