Cholangiocytic adenoma in the hilar bile duct is rare, and elevated IgG4 at the same time is extremely rare. This situation has not been reported in the literature. Testicular and paratesticular cystadenomas arise from an oviduct-like structure, which, morphologically, is almost identical with the ovarian surface epithelium. We report the case of a 45-year-old woman who underwent laparotomy for obstructive jaundice due to an uncommon cystic tumor located in the porta hepatis. Histology of the resected specimen disclosed a cystadenoma of the common hepatic duct. A review of 20 reported cases shows that the preoperative diagnosis is uncommon; wide resection appears to be the treatment of choice since these lesions are susceptible of local recurrence and malignant change.

Papillary cystadenoma of the epididymis is an occasional finding in men with von Hippel-Lindau disease. To date, an analogous lesion has not been described in women with the syndrome. We report a case of an asymptomatic papillary cystadenoma of the mesosalpinx found incidentally during surgery for a renal cell carcinoma in a woman with von Hippel-Lindau disease. This finding supports the notion that similar lesions may arise in both men and women with von Hippel-Lindau disease, perhaps from embryologically related portions of the urogenital tract.
