PURPOSE: To evaluate whether PCDS placement is safe and effective for nonsurgical patients with acute cholecystitis and cystic duct (CD) anatomy predicts technical success.
MATERIALS AND METHODS: Retrospective analysis of 118 consecutive patients who underwent PCDS (double pigtail plastic stent) attempts after percutaneous cholecystostomy tube (PCT) placement between September 2018 and September 2024 was performed. The primary endpoint was technical success, defined as successful deployment of a CD stent. Secondary endpoints included clinical success (symptom resolution without recurrence), a

INTRODUCTION: Even though cholecystectomy relieves symptoms in the majority of cases, a significant percentage suffer from 'post-cholecystectomy syndrome'. Cystic duct/gall bladder remnant calculi is a causative factor. We present our experience with the laparoscopic management of cystic duct remnant calculi.
PATIENTS AND METHODS: We managed 15 patients with cystic duct remnant calculi from 1996 to 2007 in our institute. All these patients had earlier undergone laparoscopic subtotal cholecystectomy at our centre. They were successfully managed by laparoscopic excision of the remnant.

BACKGROUND: Cystic duct stones (CDS) are occasionally encountered during laparoscopic cholecystectomy (LC). They may be noticed during the dissection of the cystic pedicle or seen to extrude from the cystic duct (CD) when it is divided or opened to perform the intraoperative cholangiogram (IOC). The procedures for dealing with CDS range from the simple removal of stones that fall out when the duct is opened to incising the duct over an impacted stone to facilitate its removal or converting to open surgery due to a large stone in a CD adherent to the bile duct (e.g., Mirizzi syndrome). Therefor

The cystic dilatation of the common bile duct is a rare disease in Europe and in the USA; even rarer in adults. In fact less than 30% of cases are described in the over 20's. There are some reports in the elderly. When observed in adults the diagnosis is usually due to the onset of symptoms of associated pathologies or to complications such as cholangitis or cancer. Ultrasound, CT and ERCP allow, in the majority of cases, an adequate pre-operative characterization of the lesion. Intra-operative cholangiography verifies the completeness of surgery--always resective--and excludes residual biliar

We report a rare case of double cystic duct in a 74-year-old woman. The patient complained of mild epigastric discomfort, and several stones were discovered by ultrasonography and computed tomography. Any anomalies of the biliary tract were undetectable in the preoperative examinations without direct cholangiography. Laparoscopic cholecystectomy was performed. After clipping the cystic duct close to the gallbladder, as usual, serial intraoperative cholangiography was performed and unexpectedly showed the inflow of contrast medium into the gallbladder via another cystic duct arising from the ri

BACKGROUND: Cystic duct leak is a rare complication of laparoscopic surgery. To study the incidence, presentation, and management of cystic duct leak (CDL) after laparoscopic cholecystectomy (LC) a retrospective study of centers doing large numbers of LC was done.
METHODS: Patient information was obtained by a questionnaire sent to experienced laparoscopic surgeons. This queried demographic information, course of the original operation, presentation, diagnostic studies, and management of CDL after LC.
RESULTS: Some 22, 165 LCs were performed by 24 surgeons; there were 58 cases of CDL (0. 26%

The human genetic disease cystic fibrosis (CF) is characterized by defective epithelial Cl- conductance (GCl). To distinguish the CF-affected GCl from other Cl- channels, we have studied the properties of GCl in normal and CF cells grown from explanted reabsorptive sweat ducts (RD). The cultured cells from normal subjects retained some of the typical duct cell properties. The Na+ conductance inhibitor amiloride hyperpolarized intracellular potentials (Vm) by 10.4 +/- 1.6 mV (n = 12). Substitution of gluconate for Cl- depolarized Vm by 15.5 +/- 1.1 mV (n = 33). The apparent GCl (G'Cl) of normal

Cystic duct cyst (CDC) is a rare biliary anomaly that may occur in isolation or in association with other choledochal cysts. While individual variants have been described, reports focusing on dual biliary pathology managed by a minimally invasive approach are limited. Surgical resection is recommended to prevent complications, including malignant transformation. We report the case of a 68-year-old woman with a remote history of cholecystectomy who presented with acute pancreatitis. Magnetic resonance cholangiopancreatography identified a CDC associated with dilatation of the common bile duct.

Duplicated gallbladder with double cystic duct is a rare anomaly, with 62 cases reported including this case. We present a 76-year-old man who underwent interval laparoscopic cholecystectomy after previous conservative management of acute cholecystitis. Retrograde dissection of gallbladder was performed due to difficult access. Gallbladder was opened at Hartman's pouch which revealed two bile-flowing structures. Cholangiogram was only successful via one of the ducts with no evidence of leak. Subtotal cholecystectomy was performed after consultation with a Hepatobiliary surgeon. Preoperative co

Hepatobiliary disease causes significant morbidity and mortality in people with cystic fibrosis (CF), yet this problem remains understudied. Previous studies in the newborn CF pig demonstrated decreased bile flow into the small intestine and a microgallbladder with increased luminal mucus and fluid secretion defects. In this study, we examined the intrahepatic bile ducts of the newborn CF pig. We assessed whether our findings from the gallbladder are present elsewhere in the porcine biliary tract and if CF pig cholangiocytes have fluid secretion defects. Immunohistochemistry demonstrated apica

OBJECTIVE: To compare the clinical efficacy of exploration of common bile duct through cystic duct or T type tube in CE patients.
METHODS: One hundred and fifty six patients with chronic CE received decompression of biliary tract from January 2007 to December 2012 were included. Exploration of common bile duct was performed through cystic duct (n=102) or T type tube (n=54). Psychological reactions were monitored including inconvenient position alteration due to carrying of the drainage tube, anxiety of the folding and prolapse of drainage tube, folding of the drainage tube, and any pain durin

We report an unusual case of adenoid cystic carcinoma (ACC) arising in the sublingual gland and extending into the submandibular duct. Our article mainly describes the MR findings. The margin of the mass was well-defined and the mass was divided into two parts - anterior and posterior. The anterior part of the mass showed isointense to surrounding muscle on T(1) weighted images and hyperintense on fat-saturated T(2) weighted images. The posterior part of the mass showed hyperintense both on T(1) weighted and fat-saturated T(2) weighted images. The findings indicated that the cystic space of th

To determine the incidence of common-bile-duct lesions and their relation to liver disease in cystic fibrosis, we performed hepatobiliary scanning in 50 of 61 patients with cystic fibrosis who had hepatomegaly, abnormal liver function, or both and in 31 of 92 patients with cystic fibrosis who did not have hepatomegaly or abnormal liver function. Ninety-six percent of the patients with liver disease had evidence of biliary tract obstruction, which was defined cholangiographically as a stricture of the distal common bile duct in the majority of cases. All the patients without liver disease had n

An 83-year-old man without specific symptoms was referred to our hospital for further evaluation and treatment of apparent double primary tumors of the cystic duct and common bile duct. Computed tomography showed contrast-enhanced solid tumors in the cystic duct and common bile duct. Magnetic resonance imaging showed that the bile duct tumor was isointense on T1-weighted images and had low intensity on T2-weighted images. In addition, the bile duct tumor showed high intensity on diffusion-weighted images. Endoscopic ultrasonography revealed the tumor of the common bile duct and endoscopic retr

We report a case of an anomalous junction of the cystic duct with the left hepatic duct found during laparoscopic cholecystectomy. Only five other patients with this anatomy have been reported. Two of these five patients had left-sided gallbladders, and the remaining patients (including ours) had their gallbladder in its normal location. Although the prevalence of this anomaly associated with left-sided gallbladders is 5.6 to 14.3%, this anomaly appears to be quite rare in patients with gallbladders in the normal position. In four cases, it was accompanied by left-sided gallbladder or low bifu

INTRODUCTION: Confluence stones usually arise when gallstones impacted in the cystic duct migrate toward the biliary confluence. Surgical treatment can be technically demanding because of chronic inflammation and distorted anatomy. Confluence stones at the junction between the cystic duct and an aberrant bile duct are extremely rare. We report a successfully treated case of confluence stones arising at the junction of the cystic duct and an aberrant anterior sectoral bile duct with a communicating accessory bile duct.
