[Scintigraphy of the lacrimal ducts].

Scintigraphy of the lacrimal pathways is an examination method which informs us on the function of the efferent lacrimal system. The authors made bilateral scintigraphic examinations in 32 patients with obstruction or stenosis of the lacrimal pathways, 22 times they made concurrent contrast examinations of the lacrimal pathways. In seven suprasaccal and 5 saccal obstructions and stenoses they did not find penetration of activity from the conjunctival sac into the lacrimal pathways. In 16 subsaccal obstructions, 12 times penetration of activity into the lacrimal sac was found. In five patients

Endonasal dacryocystorhinostomy: indications, technique and results.

Dacryocystorhinostomy (DCR) allows an obstructed lachrymal drainage system to be drained into the nasal fossa. Since the development of endonasal microsurgical techniques, the endonasal approach presents itself as an alternative choice to the conventional external approach. It is far less traumatic, yet as efficacious as the conventional approach, and avoids the need for a skin incision and the disruption of the medial canthal structures. Twenty-six patients suffering from lachrymal system obstruction underwent 28 endonasal dacryocystorhinostomies under microscopical (n = 25) or endoscopic con

Frequency of the common canaliculus: a radiological study.

BACKGROUND: It is frequently stated in the literature that there is a common canaliculus (CC) in the lacrimal drainage system in about 90% of individuals; in the remaining 10%, the upper and lower canaliculi enter the lacrimal sac separately. To our knowledge, there is no quantitative study supporting this assumption. OBJECTIVE: To investigate the frequency of the CC and other anatomical types connecting the upper and lower canaliculi to the lacrimal sac. MATERIALS AND METHODS: We presumed that there might be 3 different types of anatomical connection between the lacrimal sac and the canalic

Syringing has limited reliability in differentiating nasolacrimal duct stenosis from functional delay.

PURPOSE: To elucidate the role of syringing in assessing nasolacrimal duct (NLD) stenosis and non-anatomical functional NLD delay. METHODS: Consecutive adult patients with epiphora attending a tertiary lacrimal clinic from June 2011 to March 2021 were reviewed. Cases with evidence of canalicular stenosis or other identifiable causes of epiphora were excluded. Following syringing, patients were investigated with dacryocystography (DCG) and dacryoscintigraphy (DSG). The sensitivity and specificity of syringing were evaluated using the combined findings on DCG and DSG. RESULTS: A total of 289 s

Novel technique for lacrimal duct reconstruction using a tubularized nasomucoperiosteal flap.

The medial canthal region is a common site for skin cancers, and its resection often requires a concomitant lacrimal canaliculi resection. Although conventional reconstruction methods, such as dacryocystorhinostomy with placement of a Jones tube, autologous vascular grafting, and conjunctival flaps, have been used, each has notable drawbacks, including restenosis and conjunctivitis. Herein, we report a case of basal cell carcinoma of the lower eyelid in which the lower canaliculus was resected, followed by lacrimal duct reconstruction using a tubularized nasomucoperiosteal flap. A bony window

Improvements in planning lacrimal surgery using DICOM Horos® viewer 3D images.

INTRODUCTION AND OBJECTIVE: The individual anatomic variation of the course of the lacrimal duct and surrounding structures requires the thorough knowledge of its three-dimensional configuration in order to perform the surgery in the safest and most efficient way. The aim of this study was to consider virtual surgical planning in order to improve dacryocystorhinostomies. METHODS: Horos® was used as a viewer and manager of DICOM-format images for multiplanar, three-dimensional (3D) reconstruction when planning 148 first-time lacrimal operations and 26 reoperations by laser endonasal and endoca

Eponymous "valves" of the nasolacrimal drainage apparatus. I. A historical review.

The nasolacrimal drainage apparatus (NDA) is of interest to anatomists, radiologists, and ophthalmologists alike, owing to its intricate luminal contour, complex surrounding structural morphology, and its clinical relevance. Here, we review the history of anatomical descriptions of so-called luminal "valves" of the NDA, including the numerous historical figures whose eponyms adorn the NDA. By 1908, multiple false "valves" that are likely no more than mounds of mucosa along the NDA had been recorded. In the modern era, these have all been largely considered speculative in nature and function as

[Recent developments in lacrimal duct endoscopy].

BACKGROUND: At the moment digital dacryocystography yields the most exact results in lacrimal diagnostics. The main disadvantage lies in the dependency of high tech X-ray systems and an angiography unit, which raise enormously the costs of the examination. Recently a new diagnostic system of the lacrimal pathway is available. PATIENTS AND METHODS: Miniaturizing of endoscopes and new developments in the fiberoptic technology made it possible to introduce these mini-endoscopes into the canaliculi and perform antegrade examination of canaliculi, lacrimal sac and ductus nasolacrimalis. 86 patient

Diagnostic performance of the three-dimensional fast spin echo-Cube sequence in comparison with a conventional imaging protocol in evaluation of the lachrymal drainage system.

OBJECTIVES: To compare the three-dimensional (3D)-fast spin-echo (FSE)-Cube with a conventional imaging protocol in evaluation of dacryostenosis. METHODS: Thirty-three patients with epiphora underwent examinations using Cube magnetic resonance dacryocystography (MRD) and a conventional protocol, which included 3D fast-recovery fast spin-echo (FRFSE) MRD and two-dimensional (2D)-FSE sequences at 3.0 T. Using lachrymal endoscopic findings as the reference standard, we calculated the sensitivity and specificity of both protocols for detecting lachrymal drainage system (LDS) obstruction and their

[Tumors of the lacrimal drainage system].

BACKGROUND: The prevalence of tumors affecting the lacrimal drainage system is low, thus generating a risk of late diagnosis and treatment in clinical routine. However, these tumors can be potentially life-threatening, which emphasizes the relevance of early diagnosis and treatment. OBJECTIVE: This review focuses on the symptoms, incidence, management, and prognosis of the different tumor entities affecting the lacrimal drainage system. METHODS: The study provides a PubMed-based literature review and presents own clinical results. RESULTS: Alongside detailed medical history taking and compr

[Dacryocystorhinostomy as part of the interdisciplinary treatment of lacrimal duct].

INTRODUCTION AND METHODS: Epiphora, which leads to blurry vision, is the leading symptom for intra- and/or postsaccal lacrimal duct stenosis. Due to the anatomy of the tear duct system, which lies between the fields of ophthalmology and otorhinolaryngology, and due to newly available techniques in interventional radiology to diagnose and treat patients with intra- and postsaccal lacrimal duct stenosis, various methods for diagnosis and treatment are available. We report the results of 107 patients who underwent endonasal dacryocystorhinostomy (DCR) between 2005 and 2011. RESULTS: Prior to the
