Learning from Latent Safety Threats Identified During Simulation to Improve Patient Safety.

BACKGROUND: Latent safety threats (LSTs-characteristics of design, processes, or physical environment in health care compromising patient safety) are commonly revealed during simulation-based training. Methods of collecting, analyzing, and classifying LSTs are underdeveloped and not standardized. Building on a large simulation program in one organization, the authors aimed to collect LSTs systematically and develop a taxonomy to classify them. METHODS: The authors modified the Press Ganey Healthcare Performance Improvement Failure Modes Taxonomy (HPI-FMT), a standardized framework for safety

Evolution of TB programmatic information systems in India.

The development of tuberculosis (TB) programmatic information systems in India reflects the broader evolution of health information systems from paper-based reporting to integrated digital ecosystems. This paper documents three major phases in that evolution: the management information system era led by EpiCentre, the electronic case registry era under Nikshay V1, and the current transaction-based ecosystem represented by Nikshay V2. Each phase emerged in response to changing programme needs, technological capabilities, and expectations for data quality, timeliness, and operational usefulness.

Impact of educational interventions on clinician knowledge, adherence, and satisfaction utilizing a CKD care pathway in primary care.

BACKGROUND: Primary care clinicians are central to early chronic kidney disease (CKD) detection and management, yet often lack structured tools to support guideline-based care. CKD is common, underdiagnosed, and frequently undertreated in primary care, despite guideline-recommended screening and disease-modifying therapies. Structured CKD care pathways may improve clinician knowledge, confidence, and adherence to evidence-based management, yet real-world implementation data remain limited. METHODS: A prospective, multi-site pilot study was conducted at 2 health systems implementing a shared C

Event-based safety governance and the systematic neglect of functional decline: a case for rebalancing quality measurement in the care of older adults.

Falls prevention is a cornerstone of quality improvement in the care of older adults, yet the metrics through which it is operationalised warrant scrutiny. Current quality frameworks in acute and post-acute settings overwhelmingly treat falls as discrete sentinel events while treating functional decline as a background condition unworthy of equivalent measurement. This commentary identifies this asymmetry as a distinct governance problem, which we term event-based safety governance: the systematic privileging of countable adverse events over longitudinal harms in quality oversight. In this con

Development and Validation of the Stent Tracking Algorithm Registry for Monitoring and Retrieving Forgotten Ureteral Stents.

Purpose: Forgotten ureteral stents are associated with safety issues, increased cost, and medicolegal disputes. Tracking ureteral stents is cumbersome because of the variety in placement periods. We developed and validated an electronic medical record (EMR) system-based algorithm for monitoring patients with ureteral stent placements. Materials and Methods: The Stent Tracking Algorithm Registry (STAR) is automatically activated once the physician enters the stent placement or replacement billing code into the EMR billing system. At 120 days, an overdue notification is generated and sent to the

Therapist perspectives on the clinical utility of hand performance information from at-home egocentric video in outpatient neurorehabilitation: a multi-methods evaluation study.

BACKGROUND: Restoring hand function is a primary focus of rehabilitation after neurological injuries, such as stroke and spinal cord injury. However, monitoring hand use outside the clinic remains challenging. This study aims to evaluate how therapists perceive and would utilise information from a clinical decision support system (CDSS) that uses egocentric video to monitor patients' hand use at home. METHODS: Five patient-therapist dyads were recruited. Patients recorded daily activities using head-mounted cameras. Therapists reviewed dashboards of processed video data from their patients an

Indian sickle cell disease registry for surveillance and patient management: Development and implementation.

In India, sickle cell disease (SCD) predominantly occurs in indigenous (tribal) people, who are about 104 million. However, screening and diagnosis seldom happen. This situation necessitates developing a comprehensive SCD care model, including a registry. This paper describes the development and implementation of the Indian SCD registry (ISCDR) in six tribal-dominated districts of India. The ISCDR was created in two components-(i) an Android-based mobile/tablet application, (ii) a dashboard/admin panel for patients' data management and retrieval. Data capture involves two electronic case repor

A Neuro-Informatics Pipeline for Cerebrovascular Disease: Research Registry Development.

BACKGROUND: Although stroke is well recognized as a critical disease, treatment options are often limited. Inpatient stroke encounters carry critical information regarding the mechanisms of stroke and patient outcomes; however, these data are typically formatted to support administrative functions instead of research. To support improvements in the care of patients with stroke, a substantive research data platform is needed. OBJECTIVE: To advance a stroke-oriented learning health care system, we sought to establish a comprehensive research repository of stroke data using the Houston Methodist
