[Short history of cranial trauma in western country (part 1)].

The treatment of cranial traumatism has been the subject of numerous publications since the " corpus hippocraticum ". Following this period, the school of Alexandria will only be known thanks to Celsus. After him, Galen will determine the therapeutic attitude in accordance with the hippocratic theory. This theory will have an influence for more than 1.500 years. A better knowledge of anatomy will shake the galenic system for the first time at the Renaissance. The decisive progress will arise in the XIX century, with the emergence of experimental medicine, Pasteur's discoveries, and the greater

Factors that influence health-seeking behaviors of patients experiencing acute stroke.

Stroke is the third leading cause of death in the United States and the leading cause of serious, long-term disability. Patients who sustain a sudden ischemic stroke can benefit from the administration of intravenous recombinant tissue plasminogen activator (rt-PA). rt-PA may reduce disability, yet few patients arrive in emergency departments in time to be eligible for rt-PA administration. The purpose of this study was to discover factors that influence patients' decision-making process when seeking treatment for acute-stroke symptoms. Qualitative inquiry using grounded-theory methods was use

Dyslexics Exhibit an Orthographic, Not a Phonological Deficit in Lexical Decision.

Dyslexia is theorized to be caused by phonological deficits, visuo-attentional deficits, or some combination of the two. The present study contrasted phonological and visuo-attentional theories of dyslexia using a lexical decision task administered to adult participants with and without dyslexia. Homophone and pseudo-homophone stimuli were included to explore whether the two groups differed in their reliance on phonological encoding. Transposed-letter stimuli, including both TL neighbors and TL non-words, measured potential orthographic impairment predicted by visuo-attentional deficit theorie

Factors Associated with Parental Intent not to Circumcise Daughters in Enugu State of Nigeria: An Application of the Theory of Planned Behavior.

This study explored intention of parents not to circumcise daughters in Enugu State, Nigeria using theory of planned behavior (TPB) as a framework. A survey of 1345 parents was carried out using structured questionnaire with FGM question items based on TPB constructs of attitude, subjective norm (SN), perceived behavioral control (PBC), and intention. Intention was dichotomized into two categories and logistic regression analysis was performed to examine the association between the constructs of TPB while controlling for some socio-demographic factors. 70% of respondents did not intend circumc

Democracy under uncertainty: the wisdom of crowds and the free-rider problem in group decision making.

We introduce a game theory model of individual decisions to cooperate by contributing personal resources to group decisions versus by free riding on the contributions of other members. In contrast to most public-goods games that assume group returns are linear in individual contributions, the present model assumes decreasing marginal group production as a function of aggregate individual contributions. This diminishing marginal returns assumption is more realistic and generates starkly different predictions compared to the linear model. One important implication is that, under most conditions,

Planning treatment of ischemic heart disease with partially observable Markov decision processes.

Diagnosis of a disease and its treatment are not separate, one-shot activities. Instead, they are very often dependent and interleaved over time. This is mostly due to uncertainty about the underlying disease, uncertainty associated with the response of a patient to the treatment and varying cost of different diagnostic (investigative) and treatment procedures. The framework of partially observable Markov decision processes (POMDPs) developed and used in the operations research, control theory and artificial intelligence communities is particularly suitable for modeling such a complex decision

Reading the family: A constructivist grounded theory on organ donation conversations.

Approaching families to discuss deceased organ donation authorisation is considered one of the central stages of the organ donation process. In many countries, specialist nurses lead the organ donation process, approach and support families. However, how these encounters occur is not yet fully understood. A constructivist grounded theory methodology was used to conceptualise the process of approaching families from the perspectives of healthcare professionals and families. Data collected included six months of observations across two large hospitals in Chile, documents, interviews and focus gr

Discrepancy between social and nonsocial decision-making under uncertainty following prefrontal lobe damage: the impact of an interactionist approach.

Deficits in decision-making are thought to contribute significantly to socio-behavioral impairments of patients with frontal lobe damage. The purpose of this study was to test the hypothesis of whether the inappropriate social behavior of patients with frontal lesions can be viewed as the product of a general failure of decision-making ability or as the result of socio-cognitive impairment. We studied a group of patients with prefrontal lesions (FL patients, n = 15) and a group of matched healthy controls (n = 30) on the Iowa Gambling task (IGT) of nonsocial decision-making, environmental depe

Social cognitive predictors of intention to test for variant Creutzfeldt-Jakob disease in those affected by haemophilia and other clotting disorders.

Individuals with bleeding disorders are at increased risk of variant Creutzfeldt-Jakob disease. This study explored social cognitive predictors of screening intentions. Ninety men and women with bleeding disorders, recruited through the Haemophilia Society, completed an extended Theory of Planned Behaviour questionnaire to predict intention to screen for variant Creutzfeldt-Jakob disease. Extended Theory of Planned Behaviour variables accounted for 57 per cent of the variance in intention. Self-efficacy and anticipated affect predicted intention directly, while attitudes were mediated by antic

Do cardiologists and cardiac surgeons need ethics? Achieving happiness for a drug user with endocarditis.

Ethical dilemmas are commonplace in clinical cardiology. There has been a recent focus on ethical behaviour of cardiologists and debate about resource allocation and cost-effectiveness of new technologies. The case of an intravenous drug addict, with native aortic valve endocarditis complicated by a cerebral abscess and severe aortic regurgitation, is presented to illustrate some common ethical and moral dilemmas. The predominant theories in medical ethics, including the "Four-Principles Approach," is discussed, and a model to translate these ethical theories into a clinical decision-making to

Acupuncture for hot flashes: decision making by breast cancer survivors.

PURPOSE: Hot flashes (HFs) are a particularly common and distressing symptom among breast cancer survivors (BCSs). Given its low rate of side effects, acupuncture shows promise as a therapeutic approach for HFs, but little is known about BCS's decision making about the use of acupuncture. This study seeks to identify attitudes and beliefs about using acupuncture for HFs by BCSs. METHODS: Using the Theory of Planned Behavior (TPB) as a conceptual framework, we conducted semistructured interviews among women with stage I-III breast cancer who had finished primary treatment and were currently ex

Telehealth for Pregnant Women With Opioid Use Disorder: A Theory-Based Approach.

Pregnant women with opioid use disorder (OUD) face barriers to receiving substance use treatment and prenatal care, which can lead to adverse health outcomes. The purpose of the current article is to inform practice through the design of an innovative, nurse-led approach derived from King's Theory of Goal Attainment to care for pregnant women with OUD using telehealth technologies. The practice model encompasses King's theory using telehealth by addressing perceptions in the nurse-client interaction, establishing effective communication in nurse-client interactions, building mutual goal settin

Utility-Based Multicriteria Model for Screening Patients under the COVID-19 Pandemic.

In this paper, a utility-based multicriteria model is proposed to support the physicians to deal with an important medical decision-the screening decision problem-given the squeeze put on resources due to the COVID-19 pandemic. Since the COVID-19 emerged, the number of patients with an acute respiratory failure has increased in the health units. This chaotic situation has led to a deficiency in health resources. Thus, this study, using the concepts of the multiattribute utility theory (MAUT), puts forward a mathematical model to aid physicians in the screening decision problem. The model is us

Redefining Risk and Benefit: Understanding the Decision to Undergo Contralateral Prophylactic Mastectomy.

Rates of contralateral prophylactic mastectomy (CPM) among unilateral breast cancer patients are rapidly increasing; however, there are little data documenting the decision-making process of patients with no known BRCA mutations, who elect this more aggressive treatment. We conducted semistructured interviews with nine newly diagnosed patients who elected CPM over other surgical options. Using grounded theory, we analyzed interview data to identify influential decision-making factors by prevalence and intensity across participants. Decision-making factors included subjective evaluations of ris

Investigating Coping Processes and Psychophysiological Responses to Uncertainty in Eleven-Year-Old Children.

This experimental study investigated the cognitive, behavioral, and physiological responses of 11-year-old children when confronted with uncertainty. Using a reward-feedback-based simulation, participants engaged in decision-making tasks involving ambiguous outcomes while their heart rate, decision time, hesitation behavior, and verbalized thought processes were systematically recorded. Findings demonstrated that uncertainty led to increased physiological arousal, longer decision times, greater hesitation, and lower decision accuracy. Regression analysis revealed hesitation as the strongest pr

Dissociation of decision-making from escalated drug intake within a long-access model.

General decision-making deficits induced by chronic drug intake have been suggested to underlie the development of substance use disorder (SUD). To determine the effects of differential drug self-administration (cocaine or fentanyl) history on decision making, a procedure built upon choice theory was run alongside a drug (cocaine or fentanyl) escalation procedure in rats. Isomorphic food choice was measured each day under unpredictable probabilistic conditions that required constant engagement of decision systems, followed by self-administered drug (cocaine or fentanyl) for 1or 6 h. Despite cl

The formation of palliative treatment intentions among patients with advanced gynecological cancer in China: A grounded theory study.

OBJECTIVE: This study investigates the mechanisms underlying decision-making for palliative treatment in patients with advanced gynecological cancer, focusing on their subjective experiences and the interplay of emotional, cognitive, social, and cultural factors that shape these decisions, thereby complementing existing research on the role of palliative care. METHODS: Between April and October 2023, 14 inpatients with advanced cervical, ovarian, or endometrial cancer at a tertiary hospital in Guangdong underwent in-depth, semi-structured interviews. The data were analyzed using Strauss and C

Examining online chat within a domain of uncertainty: the case of Asperger's syndrome.

OBJECTIVE: To determine whether grounded theory can be applied as a cross-disciplinary evaluative framework for assessing health information, especially within domain-specific peer-to-peer networks. METHODS: Using a grounded-theory approach, we seek to identify recurring themes of peer-based interaction, without the ongoing management of clinical experts, as a way to determine stakeholder concerns and interests in a domain of frequent clinical uncertainty and treatment, Asperger's syndrome. RESULTS: We find that users of web-based information in such areas often report reliance on informatio
