Ring graft technique for microvascular decompression.

INTRODUCTION: Various nerve compression syndromes, such as trigeminal neuralgia (TN), glossopharyngeal neuralgia (GN), and hemifacial spasm (HFS), are caused by compression of the concerned nerve by the adjacent vessel. Patients who do not respond to medical management are usually treated by "microvascular decompression (MVD) of the nerve." Teflon patch graft is the most commonly used material for MVD. This graft has been used in various shapes like a patch, in the shredded form, or as a sling. This is done to prevent recurrence because of graft failure. We used a teflon ring graft to perform

Orbital decompression for thyroid eye disease.

The literature regarding orbital decompression for thyroid eye disease is vast, spanning multiple specialty areas including neurosurgery, head and neck, maxillofacial, and ophthalmic plastic surgery. Although techniques have advanced considerably over the more than 100 years during which this procedure has been performed, the 4 major approaches remain: transorbital, transcranial, transantral, and transnasal. The explosion in literature related to orbital decompression has mostly involved minor technical variations on broader surgical themes. The purpose of this review is to organize the major

Microvascular decompression for tinnitus: systematic review.

OBJECTIVE The objective of this study was to examine operative outcomes in cases of microvascular decompression (MVD) of cranial nerve (CN) VIII for tinnitus through a critical review of the literature. METHODS Forty-three English-language articles were gathered from PubMed and analyzed. In this review, two different case types were distinguished: 1) tinnitus-only symptomatology, which was defined as a patient with tinnitus with or without sensorineural hearing loss; and 2) mixed symptomatology, which was defined as tinnitus with symptoms of other CN dysfunction. This review reports outcomes o

Outcome analysis of cubital tunnel decompression.

BACKGROUND: Cubital tunnel decompression is a commonly undertaken upper limb procedure. Most studies compare the different techniques of decompression; however, only a few have specifically investigated the outcome of ulnar nerve decompression. AIM: The aim of this study was to investigate the outcome of ulnar nerve decompression following cubital tunnel syndrome. METHODS AND RESULTS: A total of 174 ulnar nerve decompression cases were identified from the upper limb surgery database with complete data available for 136 cases. Simple decompression was performed in 110 (80.88%) cases, and in 2

Endoscopically assisted decompression for pronator syndrome.

PURPOSE: Traditional surgical management for pronator syndrome results in a relatively long and possibly disfiguring scar across the antecubital fossa. The purposes of this study were to present an endoscopic technique that facilitates the decompression of the proximal median nerve without extensile incisions, and to evaluate whether this minimally invasive procedure could adequately and safely treat the condition to improve outcome scores. METHODS: We treated 13 patients (14 cases) with isolated pronator syndrome with endoscopically assisted decompression and retrospectively reviewed them. W

Surgical technique for trigeminal microvascular decompression.

BACKGROUND: Microvascular decompression (MVD) is a non-ablative technique designed to resolve the neurovascular conflict responsible for typical idiopathic trigeminal neuralgia (TN). METHOD: With the patient in a supine position, a small elliptical retrosigmoid craniectomy is used to approach the cerebellopontine angle and the trigeminal nerve. After careful exploration of the trigeminal root entry zone, the offending vessel is identified and moved away. Oxidized regenerated cellulose is used to keep the vessel in its new position far from the nerve. CONCLUSION: MVD represents the gold stand

Microvascular decompression for trigeminal neuralgia: update.

PURPOSE OF REVIEW: A recent Cochrane systematic review of surgical interventions for trigeminal neuralgia found not a single trial of what is becoming the most popular surgical intervention, namely microvascular decompression (MVD). With an increasing number of anticonvulsant drugs it is likely that patients may not be offered a surgical option for management of their trigeminal neuralgia for many years. RECENT FINDINGS: Current studies repeat much of what is already in the literature but there is an increasing appreciation of the value of preoperative imaging and the need to be more precise

Bullosis diabeticorum following carpal tunnel decompression.

A 76-year-old gentleman underwent carpal tunnel decompression on his right dominant hand under local anaesthesia. He was diabetic and otherwise healthy. He presented 3 days after with ischaemic blisters of his lateral three and a half fingers on the finger tips. This complication following carpal tunnel decompression or following the use of local anaesthetics at the wrist has not been documented so far in the literature on the PUBMED data base. The authors wish to highlight this rather unusual complication encountered following this common procedure in orthopaedic surgical practice.

[Microsurgical decompression of lumbar spinal stenosis].

Lumbar spinal stenosis in most cases is due to progressive degeneration of the spine, resulting in thickening of facet joints and flaval ligament. Thus the diameter of the lumbar spinal canal is reduced to less than 12 mm in the AP direction. Typically complaints consist in neurogenic claudication. Patients usually experience improvement of pain when bending their back or walking up a hill. Diagnosis of lumbar spinal stenosis is confirmed by MRI. CT myelography may help detect where compression is most pronounced. Surgical treatment should be based on the clinical symptoms of the mostly elderl

Intracranial hemorrhage complicating an orbital decompression.

We report a case of intracerebral hemorrhage in the left frontal lobe following a left medial wall orbital decompression for thyroid-related optic neuropathy. There was no obvious fracture in the orbital roof bone but the hemorrhage was felt to occur from a disruption in the orbito-frontal branch of the anterior cerebral artery. The patient was asymptomatic and remained so during the follow-up period.

Technique of microvascular decompression. Technical note.

Vascular compression of the trigeminal nerve in the cerebellopontine angle is now generally accepted as the primary source or "trigger" causing trigeminal neuralgia. A clear clinicopathological association exists in the neurovascular relationship. In general, pain in the third division of the trigeminal nerve is caused by rostral compression, pain in the second division is caused by medial or more distant compression, and pain in the first division is caused by caudal compression. This discussion of the surgical technique includes details on patient position, placement of the incision and cran

Microvascular decompression surgery for trigeminal neuralgia.

Long-term follow-up data were analyzed to evaluate the surgical results of microvascular decompression (MVD). Among 1,324 patients with trigeminal neuralgia (TGN), who underwent MVD between 1976 and 2000, a consecutive 281 patients were followed and studied for 5-20 years as of the year of 1997. When the data were analyzed and compared in two groups (A: 1976-1986, B: 1987-1991), the postoperative cure rate increased from 92.9 to 96.7%, satisfaction rate with the results of MVD increased from 80.3 to 82.5%, incomplete cure rate decreased from 7.1 to 3.3%, and recurrence rate decreased from 10.2
