Assessing Pelvic Organ Prolapse Volume in Patients with Rectal Prolapse via MRI.

INTRODUCTION AND HYPOTHESIS: The utility of magnetic resonance imaging (MRI) in recognizing the severity of pelvic organ prolapse (POP) in patients with rectal prolapse (RP) remains underexplored. Understanding POP volume in these patients may be crucial for targeted interventions, particularly in RP cases where a multidisciplinary approach is often required. This study investigates whether volume measurements via MRI defecography can improve screening for symptomatic POP and provide better guidance for clinical interventions. METHODS: This retrospective cohort study involved 77 women with RP

Dynamic Pelvic Magnetic Resonance Imaging Evaluation of Pelvic Organ Prolapse Compared to Physical Examination Findings.

OBJECTIVE: To compare dynamic magnetic resonance imaging (dMRI) defecography phase findings with physical examination (PE) grading in the evaluation of pelvic organ prolapse (POP). METHODS: We retrospectively reviewed 274 consecutive patients who underwent dMRI with defecography. Baden-Walker grading of POP, absolute dMRI values, and grading by dMRI were collected for anterior, apical, and posterior compartments. Anatomically significant POP on PE was defined as Baden-Walker Grade ≥3 and on dMRI by dMRI Grade ≥2. A Spearman's Rank correlation was performed between absolute dMRI values and res

Defecographic study of rectal evacuation in constipated patients and control subjects.

PURPOSE: To validate a computer-based area calculation method of quantification of rectal evacuation by using defecography videotapes and to use that method to compare evacuation in constipated patients with that in control subjects. MATERIALS AND METHODS: For validation of the method, simultaneous defecography and weight measurements were compared in 36 patients with constipation or incontinence. Evacuation was calculated as the rate of change of the contrast medium-covered rectal area (percentage per second) or of the evacuated amount of contrast medium (percentage per second [relative] and

Fecal incontinence successfully managed by antegrade continence enema in children: a report of two cases.

Two children with intractable fecal incontinence after correction of high anorectal malformations were successfully managed by the daily administration of a glycerin enema into the cecum via an appendicocecostomy or tubularized cecostomy, according to the method of Malone's antegrade continence enema (ACE). Fluoroscopic defecography performed during this procedure in each patient disclosed that the glycerin enema promptly evoked cecal peristalsis, which was transmitted to the distal colon and rectum, and squeezed out almost all the fecal matter, evacuating it from the anus. However, two enemas

Effects on anorectal function of endoscopic submucosal dissection for rectal tumors involving the dentate line.

OBJECTIVE: Endoscopic submucosal dissection (ESD) is a specific procedure that uses an electrosurgical knife for en-bloc resectioning large tumors. However, no study has examined the effect of ESD on the defecation function of patients with rectal tumors. We aimed to investigate the potential effects caused by ESD by analyzing changes in the morphology and movement of the anorectum pre- and post-ESD. METHODS: This prospective observational study included 11 patients with anorectal tumors who underwent ESD between April 2020 and February 2022. Pre-ESD assessments included anorectal manometry a

Enterocele: what is the clinical implication?

PURPOSE: Because of the rarity of diagnosis, the clinical implication of enterocele remains unclear. This study was designed to clarify the characteristic symptoms and the role of surgical treatment for enterocele. METHODS: We reviewed a defecography database prospectively maintained from 1998 to 2004. Defecography was performed with oral barium to opacify the small bowel in all patients. Enterocele was defined as the small bowel falling down beyond the level of the vaginal vault practically, it was <3 cm from the anorectal angle in the lateral view. Patients' clinical information was collect

Internal rectal intussusception--fact or fancy?

PURPOSE: There is still considerable debate whether internal intussusception represents a functional disorder. We have reviewed our results in an effort to define its symptomatology and to assess defecography. METHODS: Rectopexy has been performed for internal intussusception in 37 patients. Eighteen had solitary rectal ulcer syndrome (SRUS), and 31 had anterior rectal wall prolapse. Defecography demonstrated anterior wall prolapse in 13, circular prolapse in 21, and no disorders in 3 patients. Pelvic floor function was normal. Follow-up varied from one to nine years. RESULTS: Twenty-six pat

Transverse coloplasty pouch after total mesorectal excision: functional assessment of evacuation.

PURPOSE: Colon pouch reconstruction after total mesorectal excision is functionally superior to straight colorectal/ anal anastomosis. In the long-term, stool evacuation difficulties could jeopardize the functional benefit. The transverse coloplasty pouch presents an alternative to the standard J-pouch. This study was designed to analyze functional outcome and defecography findings after total mesorectal excision and transverse coloplasty pouch reconstruction. METHODS: Thirty consecutive patients with cancer of the middle and lower third of the rectum underwent a total mesorectal excision and

[Laparoscopic rectocele repair with mesh--a case report].

Relaxation of the pelvic floor causes changes in the anatomy. This results in symptoms, which can be treated with different operations. The use of modern, minimal invasive methods and tension free mesh implants for the reconstruction is widely used in rectocele repair as well. We present a 59 year-old patient undergone both vaginal hysterectomy and cystocele repair previously who complained of progressive constipation. Physical examination and dynamic defecography confirmed the presence of a significant rectocele. Anal manometry showed no functional loss. Mobilisation of the rectum and divisio

Treatment of internal rectal intussusception in patients with chronic constipation.

BACKGROUND: The significance of internal rectal intussusception (rectal invagination) in chronic constipation dominated by symptoms of obstructed defecation is not fully clarified. METHODS: Seventeen consecutive patients with chronic constipation and a history of obstructed defecation with internal rectal intussusception demonstrated by defecography were treated by perineal rectopexy. RESULTS: Of 15 patients followed up for more than 2 years defecography showed total disappearance of the intussusception in 12. Five of these claimed substantial improvement (42%; 17-72%). Internal rectal intussusception: results of surgical repair.

Twenty-four patients with obstructed defecation due to rectal intussusception diagnosed by defecography were treated with rectopexy either by the Wells technique (9 patients) or by Orr's operation (15 patients). After follow-up from one to eight years, defecography demonstrated disappearance of the intussusception in 22 patients. None of the patients were completely relieved of their symptoms. Nine (41 percent; 95 percent confidence limits: 21-64) were improved and 13 were unchanged (59 percent; 95 percent confidence limits: 36-79), with no difference between the two procedures.
