Methods of ambulatory detection and treatment of cardiac arrhythmias using implantable cardioverter-defibrillators.

The use of the implantable cardioverter-defibrillator (ICD) for the treatment of ventricular fibrillation, a condition that can lead to sudden cardiac death, is examined. Topics relevant to the development and implementation of ICD technology, such as defibrillation threshold optimization, battery design, lead configuration, arrhythmia-detection algorithms, and pacemakers-ICD interactions, are described. Clinical situations involving the surgical implantation procedures and the quality of life after implantation are also considered. Cost-benefit analysis of ICD treatment as well as an overview

Clinical significance of sleep-related breathing disorders in patients with implantable cardioverter defibrillators.

The prevalence and clinical significance of sleep-related breathing disorders (SRBDs) in patients with cardiac disease and a history of life-threatening ventricular tachyarrhythmias is unclear. Forty consecutive recipients of implantable cardioverter defibrillators (ICDs) with cardiac disease and a documented history of spontaneous, life-threatening, ventricular tachyarrhythmias underwent full night polysomnography. SRBDs were diagnosed if the apnea/hypopnea index was > 10. SRBD were diagnosed in 16 of 40 patients (40%): central sleep apnea (CSA) was present in 9 of these 16 patients (56%), 8

Antitachycardia pacing in patients with implantable cardioverter defibrillators: how many attempts are useful?

The purpose of this study was to determine the termination and acceleration rates for 1 to 6 attempts of antitachycardia pacing [ATP] delivered by ICD in order to terminate spontaneously occurring VTs. Twenty-four ICD recipients with active ATP programs, including a maximum of six ATP sequences and spontaneously occurring VTs during follow-up, were investigated. During a mean follow-up of 42 +/- 15 months (range, 17-63 months) 413 spontaneous VT episodes (17 +/- 14; range, 1-49 per patient) resulting in appropriate ATP delivery by the ICD occurred. ATP successfully terminated 328 episodes (80%

[Inadequate therapies with implantable cardioverter-defibrillators--incidence, etiology, predictive factors and preventive strategies].

Patients with implantable cardioverter defibrillators (ICD) often suffer inappropriate ICD-therapies. The incidence, causes and risk factors of ICD-therapies for a rhythm other than ventricular tachyarrhythmias (VT) were determined retrospectively in 462 consecutive patients (pts). Inappropriate ICD-therapies were identified based on stored R-R intervals and/or electrograms. Eighty-two pts (18%) had inappropriate ICD-therapies. Actuarial rates for inappropriate ICD-therapies were 13%, 20%, 24% and 29% at 1, 2, 3 and 4 years after ICD-implantation, respectively. Atrial fibrillation with rapid v

Implantable Cardioverter-Defibrillator Therapy and Implantable Cardioverter-Defibrillator Complications in Idiopathic Ventricular Fibrillation Patients.

BACKGROUND: Implantable cardioverter-defibrillator (ICD) therapy is used to prevent sudden cardiac death (SCD) in patients who experienced idiopathic ventricular fibrillation (IVF). Only certain patients with IVF develop recurrence of ventricular arrhythmia (VA). However, all patients with IVF are exposed to the risk of complications. Transvenous ICD (TV-ICD) carries a significant complication risk. The subcutaneous ICD (S-ICD) may offer advantages in this young population. OBJECTIVES: The aim of this study was to analyze ICD therapy in patients with IVF and to assess complications of ICD the

The extravascular implantable cardioverter-defibrillator: characterization of anatomical parameters impacting substernal implantation and defibrillation efficacy.

AIMS: The aim of this study is to provide a thorough, quantified assessment of the substernal space as the site of extravascular implantable cardioverter-defibrillator (ICD) lead placement using computed tomography (CT) scans and summarizing adverse events and defibrillation efficacy across anatomical findings. Subcutaneous ICDs are an alternative to transvenous defibrillators but have limitations related to ICD lead distance from the heart. An alternative extravascular system with substernal lead placement has the potential to provide defibrillation at lower energy and pacing therapies from a

The effects of pneumothorax on defibrillation thresholds during pectoral implantation of an active can implantable cardioverter defibrillator.

Pneumothorax has previously been reported to increase defibrillation thresholds and lead impedance in monophasic systems. This article demonstrates a case using an active can system in which the lead impedance between the right ventricular defibrillation coil and the device improved from 70 omega with pneumothorax to 48 omega after resolution. The defibrillation threshold also improved from > 30 J to < or = 10 J after the pneumothorax had resolved. We conclude that acute pneumothorax can increase the transthoracic lead impedance and defibrillation thresholds; however, with resolution both para

Battery longevity of implantable cardioverter-defibrillators and cardiac resynchronization therapy defibrillators: technical, clinical and economic aspects. An expert review paper from EHRA.

In recent years an extension of devices longevity has been obtained for implantable cardioverter-defibrillators (ICDs), including ICDs for cardiac resynchronization therapy (CRT-D) through improved battery chemistry and device technology and this implies important clinical benefits (reduced need for device replacements and associated complications, particularly infections), as well as economic benefits, in line with patient preferences and needs. From a clinical point of view, the availability of this improvement in technology allows to better tune the choice of the device to be implanted, tak

Implantable defibrillator recipients' responses to device implantation and design.

The implantable cardioverter defibrillator (ICD) is a surgically inserted device that follows a well-established pattern of technology innovation and rapid adoption without extensive clinical trials. Few devices are as obviously life-saving in potentially fatal situations and deliver such sudden and sometimes painful therapy. Although patients' quality of life and adjustment to the device both physically and psychologically are relatively well-documented, few studies have explored recipients' responses to the device itself or elicited their thoughts on the technological design. The findings pr

Automatic implantable cardioverter-defibrillator: techniques of implantation and results.

An initial experience with use of the automatic implantable cardioverter-defibrillator (AICD) is described. Twelve patients received the device. One death has occurred during a mean follow-up of 15 months, and it was due to causes other than arrhythmias. Appropriate device discharge terminating a malignant arrhythmia occurred in 9 patients (75%). The observed survival (92%) far exceeds that to be expected in survivors of sudden death treated by conventional means. There have been no operative deaths. Morbidity has been minimal, although three reoperations were required in 2 patients because of

A Scoping Review of Australian Hospitals' Policies on the Deactivation of Implantable Cardioverter Defibrillators.

AIM: More older adults are living with an implantable cardioverter defibrillator (ICD). Deactivation rates for those approaching end-of-life are low, risking undue distress and an undignified death. This scoping review aimed to determine the availability and content of hospital guidance documents regarding ICD deactivation towards end-of-life. METHOD: Guidelines' databases from two Australian states (Victoria and South Australia) were systematically searched between September 2022 to February 2023, to identify all documents that specified guidance for ICD deactivation at end-of-life. Relevant

Predictors of Atrial High-Rate Episodes and Inappropriate Device Therapy in Implantable Cardioverter Defibrillators.

BACKGROUND: Implantable cardioverter defibrillator (ICD) therapy can prevent sudden cardiac death, but poses a risk of inappropriate device therapy (IDT), which is linked to worse outcomes and reduced quality of life. On the other hand, ICDs can function as an early warning system for atrial arrhythmias. PURPOSE: This study aimed to predict which patients are at risk of IDT and who develop atrial high-rate episodes (AHRE) by investigating pre-implant clinical parameters. METHODS: Patients who received an ICD with an atrial lead between 2010 and 2019 were included in a prospective registry.

Effects of Irradiation During Computed Tomography Scanning on the Function of Implantable Cardioverter-defibrillators.

The effect of irradiation during computed tomography (CT) imaging on implantable cardioverter-defibrillators (ICDs) has not been fully evaluated in various settings. The purposes of this study were to evaluate the occurrence of electromagnetic interference (EMI) during CT irradiation in various clinically available ICDs with phantom experiments and to determine the potential risks related to irradiation during CT imaging. Five types of clinically available ICDs from five manufacturers were tested. An ICD was combined with an electrocardiogram (ECG) simulator, mounted in a chest phantom, and su

Subcutaneous versus transvenous implantable cardioverter defibrillators in children and young adults: A meta-analysis.

INTRODUCTION: The implantable cardioverter defibrillator (ICD) has been demonstrated to successfully prevent sudden cardiac death (SCD) in children and young adults. A wide range of device-related complications/malfunctions have been described, which depend on the intrinsic design of the defibrillation system (transvenous-implantable cardioverter defibrillator [TV-ICD] vs. subcutaneous-implantable cardioverter defibrillator [S-ICD]). OBJECTIVE: To compare the device-related complications and inappropriate shocks with TV-ICD versus S-ICD. METHODS AND RESULTS: Electronic databases were queried
