Delusion of oral parasitosis in a patient with major depressive disorder.

Delusion of parasitosis is a rare condition in which the patient has a strong conviction that he or she is infested by small organisms. The condition may exist as an isolated phenomenon, or may occur in association with other psychiatric syndromes. We report a single case of a rare form of this phenomenon. A middle-aged woman who met forth edition of diagnostic and statistical manual of mental disorders criteria for major depressive disorder with psychotic features is described. She had delusions of oral parasitosis. She felt lizards and small organisms in her mouth. To the best of our knowled

Selective attention to threatening faces in delusion-prone individuals.

INTRODUCTION: Selective attention to threat-related information has been associated with clinical delusions in schizophrenia and nonclinical delusional ideation in healthy individuals. However, it is unclear whether biased attention for threat reflects early engagement effects on selective attention, or later difficulties in disengaging attention from perceived threat. The present study examined which of these processes operate in nonclinical delusion-prone individuals. METHODS: A total of 100 psychologically healthy participants completed the Peters et al. (1999) Delusions Inventory (PDI). T

Delusions of theft in dementia of the Alzheimer type: a preliminary report.

Delusions of theft are commonly found in patients with dementia of the Alzheimer type (DAT). This report describes the frequency, onset, and characteristics of delusions of theft in DAT patients. The sample consisted of 54 geropsychiatric inpatients with DAT; delusions of theft were found in 30 (55.6%) patients. Two thirds of these patients had delusions of theft within 1 year after onset of illness. There were no significant differences in age, age of onset, duration of disease, years of education, or the Mini-Mental State Examination (MMSE) score between patients with and without delusions o

[Spatial delusion: sign of lesions of the non-dominant hemisphere].

Two cases with respectively an ischemic and a tumoral lesion of the right hemisphere showing a striking and isolated delusional belief to be relocated elsewhere are reported. Delusion had the remarkable intrinsic coherence of a paranoid delirium but was limited to the localization in space. In contrast these patients had no mental clouding or hallucinations and were well orientated for time and persons. Associated findings were a misrepresentation of space in drawing in the first case, and a left hemiparesis, a left sensory extinction, a left hemianopsia, a left visual neglect, a constructiona

Association Between Delusions of Infestation and Prescribed Narcotic and Stimulant Use.

OBJECTIVE: Patients diagnosed with delusions of infestation (DOI) at a psychodermatology clinic appeared to have a higher incidence of being prescribed narcotic or stimulant medications compared with the general dermatologic clinic population with chronic pruritic conditions. A retrospective study was conducted examining the correlation between patients with DOI and prescribed psychoactive medications. METHODS: Ninety-two patients with a diagnosis of DOI, seen at our University Psychodermatology Clinic, served as the study population. The comparison group (N=354) included dermatology patients

Cognitive factors maintaining persecutory delusions in psychosis: the contribution of depression.

Depression is common in people with schizophrenia, but how it might directly contribute to the persistence of psychotic symptoms has rarely been tested. The key aim of the present study was to test whether depression and associated cognitive processes predict the maintenance of persecutory delusions. Three groups of participants were tested at baseline: 60 patients with persecutory delusions in the context of a schizophrenia spectrum diagnosis, 30 patients with depression, and 30 nonclinical controls. They completed interviewer and self-report assessments of depression and paranoia, and measur

Delusions, illusions and hallucinations in epilepsy: 2. Complex phenomena and psychosis.

In this second paper the clinical features and electrophysiological underpinnings of more complex psychotic states associated with epilepsy are reviewed. (a) Complex partial status epilepticus, in particular of temporal lobe origin, may result in mental states remarkably similar to those seen in the primary psychoses. This non-convulsive state is associated with prolonged epileptic discharges on intracranial stereoelectroencephalography (SEEG) in hippocampal and other mesial temporal structures, sometimes without abnormalities on the scalp EEG. Where hallucinatory or psychotic symptomatology d

Delusions associated with elevated muscarinic binding in dementia with Lewy bodies.

The relation between disturbances of cholinergic neurotransmission and delusions (DELs) has not been investigated in degenerative dementias such as dementia with Lewy bodies (DLB). A cohort of dementia patients were assessed with standardized clinical evaluations (including the Columbia University Scale for Psychopathology in Alzheimer's Disease), which were repeated annually until death. DLB was confirmed neuropathologically in 21 patients. Neurochemical evaluation included M1 receptor autoradiography (pirenzepine binding), biochemical measurement of choline acetyltransferase (ChAT), and acet

Clinical and Structural Differences in Delusions Across Diagnoses: A Systematic Review.

Delusions are marked, fixed beliefs that are incongruent with reality. Delusions, with comorbid hallucinations, are a hallmark of certain psychotic disorders (e.g., schizophrenia). Delusions can present transdiagnostically, in neurodegenerative (e.g., Alzheimer's disease and fronto-temporal dementia), nervous system disorders (e.g., Parkinson's disease) and across other psychiatric disorders (e.g., bipolar disorder). The burden of delusions is severe and understanding the heterogeneity of delusions may delineate a more valid nosology of not only psychiatric disorders but also neurodegenerative

Two Case Studies of Delusions Leading to Suicide, a Selective Review.

Many questions pertaining to delusional disorder (DD) remain unanswered. It is unclear what syndromes to include under this category of psychotic illness and when to treat with antidepressants, cognitive therapy, or antipsychotic medication. DD is associated with psychiatric comorbidity, especially depression, and rates of suicidal behavior are high when the two conditions co-exist. In this selective review, we present two instances of suicide in the context of DD, one illustrating risks for the somatic subtype and the second, risks for the persecutory subtype. The frequency of suicidal behavi

Relapsing-remitting type multiple sclerosis with olfactory hallucinosis and secondary delusions.

Multiple sclerosis is frequently associated with psychiatric symptoms. In more than 50% of the patients symptoms of affective disorder or cognitive decline are observed. A significantly smaller proportion of these individuals develop psychotic symptoms usually during the course of the disease. In this report the rare clinical case of a woman who developed olfactory hallucinations and delusional disorder after 13 years of relapsing-remitting type multiple sclerosis is described.

From real-world events to psychosis: the emerging neuropharmacology of delusions.

The earliest stages of delusion are characterized by an overabundance of meaningful coincidences impinging on the sufferer's existing worldview. Successive events are seen by him as pointing to, and then confirming, a fundamentally new reality that takes him over and engulfs his everyday life. Research over the last 4 decades has revealed the importance of dopamine (DA), D2 receptors, and the basal ganglia in psychotic thinking. Recent work has implicated the aberrant reward learning initiated by the excess release of striatal DA in the attribution of excessive importance or "salience" to insi

Somatic sexual hallucinations and erotomanic delusions in a mentally handicapped woman.

A case of erotic delusion and concurrent somatic sexual hallucination is described in a 66-year-old mentally handicapped woman. The history, mental state and biological investigations convey the impression of erotomania, or de Clerambault's syndrome. Treatment with Pimozide has resulted in a remarkable improvement such that the patient has been symptom free for 8 months at the time of reporting. In the authors' opinion, the case lends support to the argument that erotomania exists as a mono-delusional disorder and is equivalent to the DSM-III-R classification of Paranoid Disorder, Erotomanic T

Drug-induced delusion: A comprehensive overview of the WHO pharmacovigilance database.

INTRODUCTION: A number of prescribed medicines have been reported in cases of drug-induced delusion, such as dopaminergic agents or psychostimulants. But to this day, most studies are based on a limited number of cases and focus on a few drug classes, so a clear overview of this topic remains difficult. To address this issue, we provide in this article a comprehensive analysis of drug-induced delusion, based on the World Health Organization (WHO) pharmacovigilance database. METHODS: We performed a disproportionality analysis of this database using the information component (IC). The IC compar

Adjunctive Clotiapine for the Management of Delusions in Two Adolescents with Anorexia Nervosa.

Clotiapine is an atypical antipsychotic indicated for the management of a series of acute psychotic disorders. The current literature lacks evidence concerning the tolerability and clinical use of this drug in the management of individuals with anorexia nervosa (AN). In this study, we report two cases of adolescents with AN, treated with clotiapine. The reason for the administration of clotiapine was, for both patients, the manifestation of bizarre delusions concerning food and calories. Patient 1 presented a presyncope after the first dose of clotiapine, and treatment was rapidly discontinued
