Protocol of a randomized trial of "pathways," a hope intervention for people undergoing treatment for advanced lung cancer.

OBJECTIVE: Advanced cancer and its treatment can disrupt personal goal pursuits and attainment, which can imperil patient quality of life and increase risk for depression. Hope interventions foster the goal-pursuit skills exemplified by high-hope individuals and, based on a pilot trial, are a promising approach to support these patients. The current Phase II trial (NCT06047301) aims to determine the efficacy of a brief, hope intervention, "Pathways," in improving depression symptoms for people undergoing treatment advanced stage lung cancer with elevated distress. METHODS: In this two-arm ran

PSilocybin for psYCHological and existential distress in PALliative care (PSYCHED-PAL): A single arm unblinded clinical trial.

BACKGROUND: Psychological distress is a common problem near the end of life, for which we lack effective, timely and scalable treatments. No previous study has assessed whether microdose psilocybin can improve symptoms in this population. AIM: To determine whether microdose psilocybin is safe, feasible and potentially efficacious in a palliative setting. DESIGN: Open label, single-arm clinical trial of a 3-week oral psilocybin intervention, starting with 1 mg daily in week 1, increased to 2 mg in week 2 and 3 mg in week 3. CLINICALTRIALS: gov NCT04754061. SETTING/PARTICIPANTS: Two-center s

Psychedelic therapy and cultural humility.

IMPORTANCE: As psychedelic-assisted therapies gain clinical legitimacy, their cultural portability remains underexamined. While promising results have emerged from trials on existential distress in life-limiting illness, most protocols reflect Euro-American values. The expansion of these therapies calls for a pivot toward cultural humility and renewed attention to what constitutes a "good death" in cultural worlds. This commentary explores these questions through the case of Chinese palliative care. OBSERVATIONS: Clinical trials of psilocybin and LSD assisted therapy demonstrate significant r

A multi-level analysis of non-significant counseling effects in a randomized smoking cessation trial.

AIMS: To determine, in the context of a trial in which counseling did not improve smoking cessation outcomes, whether this was due to a failure of the conceptual theory identifying treatment targets or the action theory specifying interventions. DESIGN: Data from a randomized clinical trial of smoking cessation counseling and bupropion SR were submitted to multi-level modeling to test whether counseling influenced real-time reports of cognitions, emotions and behaviors, and whether these targets predicted abstinence. SETTING: Center for Tobacco Research and Intervention, Madison, WI. PARTIC

Time Toxicity in Advanced Breast Cancer: Psychological Burden, Psychiatric Vulnerability, and Implications for Patient-Centered Decision-Making.

The paradigm shift toward precision oncology in advanced breast cancer has increased therapeutic complexity, prolonging survival for many patients while simultaneously expanding the temporal burden associated with cancer care. Time toxicity, defined as the cumulative time spent receiving, accessing, coordinating, and recovering from medical interventions, is increasingly recognized as a clinically meaningful but still under-assessed dimension of patient-centered value. Beyond its organizational and economic implications, time toxicity may exert profound psychological and psychiatric effects, p

Comparing the meanings of living with advanced breast cancer between women resilient to distress and women with persistent distress: a qualitative study.

BACKGROUND: Most women with advanced breast cancer (ABC) show little distress, but about one in ten show persistent distress over time. It remains unclear if meanings ascribed by patients to ABC differentiate these distress trajectories. STUDY AIMS: This qualitative study (a) compared illness meanings of ABC between women with persistent psychological distress and those with low/transient distress, and (b) examined how illness meanings might influence coping strategies. METHODS: The sample was drawn from a prior quantitative study exploring psychological distress trajectories following ABC d

Effects of immigration on the mentally ill--does it produce psychological distress?

This report explores psychological distress among immigrants seeking help from psychiatric outpatient clinics as compared with control nonpatient immigrants. Our hypothesis is that nonpsychotic mentally ill immigrants will react to acculturation by psychological distress similarly to healthy individuals. Three questionnaires were used in this survey: Demographic Psychosocial Inventory, Brief Symptom Inventory (BSI), and Psychiatric Epidemiology Research Interview-Demoralization Scale (PERI-D). They were completed by patient and control groups consisting of recent adult immigrants to Israel fro

Thirty Years of the Diagnostic Criteria for Psychosomatic Research (DCPR): A Systematic Review and Discussion of Clinical Implications.

Objective: This is the first systematic review on the prevalence and clinical characteristics of 14 syndromes included in the Diagnostic Criteria for Psychosomatic Research (DCPR) across different settings. Data Sources: A systematic search was conducted using Web of Science to identify all English-language citing articles for DCPR key references (from 1995 to 2025). A manual search of the literature was also performed. Study Selection: Studies were eligible if they reported the prevalence and clinical characteristics of the DCPR syndromes. The initial search identified 604 reports. After remo

Role of Sense of Coherence in the Outcomes of Acute Myocardial Infarction Survivors: A 2-Year Follow-Up Study.

BACKGROUND: Survivors of acute myocardial infarction (AMI) struggle with stressful consequences. Sense of coherence (SOC) seems to be associated with a person's capacity to face life incidents. This study aims to evaluate SOC's correlation with the major adverse cardiac events (MACE) among the AMI survivors. The study was designed and reported following the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines and checklist. METHODS: This study was part of the ST-elevated myocardial infarction cohort study in Isfahan (SEMI-CI) conducted on 724 AMI survivors

Resisting the racialization of medical deservingness: How Latinx nurses produce symbolic resources for Latinx immigrants in clinical encounters.

This paper investigates how Latinx nurses resisted the racialization of medical un-deservingness against co-ethnic immigrants in everyday clinical encounters. Drawing on 26 in-depth interviews and dialoguing with the literature on minority professionals, we find that, as a form of racialized equity work, Latinx nurses produced certain symbolic resources, specifically the interactional signals to counteract Latinx patients' internalization of un-deservingness and other medical staff's open hostility towards these "undeserving illegals." Latinx nurses hybridized neoliberal norms (self-sufficienc

Depression and psycho-existential correlates in cancer outpatients: A longitudinal real-life study.

BACKGROUND: Depression is prevalent and clinically significant among cancer patients, being associated with poorer quality of life, treatment non-adherence, and increased mortality. However, few studies have investigated major depression and its relationship with psycho-existential dimensions using structured diagnostic tools and longitudinal designs in real-world psycho-oncology settings. AIM AND METHOD: This study aimed to assess the prevalence of major depression in cancer outpatients receiving integrated psycho-oncology care and to examine longitudinal changes in psychological and existen

Perspective: Depression in Persons with Parkinson's Disease.

Depression is a prevalent and disabling syndrome characterized by sustained sadness and/or anhedonia, as well as cognitive and physical symptoms. In Parkinson's disease (PD), depression is both common and clinically challenging due to overlapping symptoms and complex etiologic interactions. Major depressive disorder occurs in approximately 17% of PD patients, while clinically significant depressive symptoms affect over one-third. Both PD-specific and general risk factors contribute to this burden, with the latter playing a greater role. Diagnosis is complicated by symptom overlap, prompting re

Predictors of suicide attempt versus isolated suicidal ideation in major depressive disorder: Novel insights from LASSO-boruta screening and binary logistic regression.

OBJECTIVES: Phenotypic heterogeneity between suicidal ideation (SI) only and suicide attempt (SA) in major depressive disorder (MDD) suggests distinct suicide risk profiles, yet key clinical and symptomatic factors distinguishing SA from SI remain incompletely defined. METHODS: A total of 799 MDD patients were allocated into SI only (n = 407) and SA (n = 392) groups. Sociodemographic indicators, clinical characteristics and depressive symptoms were compared. Predictors were selected using combined Boruta and LASSO regression, followed by binary logistic regression to identify independent fact

Suicide Risk in Glioblastoma Multiforme: A Brief Review of Risk Factors, Screening, and Prevention.

PURPOSE OF REVIEW: Glioblastoma multiforme (GBM) is an aggressive primary brain tumor associated with poor prognosis, rapid functional decline, and psychological distress. An updated review of the literature is lacking, despite that patients with GBM appear to be among the highest risk for suicide among cancer patients. This review summarizes up-to-date evidence of suicide risk factors, screening approaches, and suicide mitigation strategies in patients with GBM. RECENT FINDINGS: Suicide risk is the highest in the first year following GBM diagnosis. It is also associated with older age, male

Spiritual wellbeing in psychedelic-assisted therapy with palliative care populations: An analysis of outcome measures.

OBJECTIVES: People with serious illnesses often experience spiritual and emotional pain, manifesting in conditions such as depression, anxiety, and demoralization. Emerging research in psychedelic-assisted therapy has shown efficacy in treating these conditions. Despite evidence that psychedelics frequently occasion mystical/spiritual experiences in participants, there has been little research on support for spiritual, existential, religious, and theological needs, including the use of chaplains on therapeutic teams. Spiritual wellbeing outcomes have been inconsistently used and reported on in

Factor structure and validity of the Inventory of Depression and Anxiety Symptoms-II (IDAS-II) in a chronic back pain treatment-seeking sample.

Many patients who seek treatment for chronic back pain are also at a higher risk of having comorbid anxiety- and depression-related disorders. Measures of mood and anxiety are routinely used in medical settings to screen for depression- and anxiety-related symptoms. However, factor analyses of other measures of mood and anxiety in medical settings often detect a somatization factor which, in turn, limits their discriminant validity for use across medical settings. The Inventory of Depression and Anxiety Symptoms-II (IDAS-II) is a comprehensive self-report inventory that assesses varying aspect

The Opioid/Overdose Crisis as a Dialectics of Pain, Despair, and One-Sided Struggle.

The opioid/overdose crisis in the United States and Canada has claimed hundreds of thousands of lives and has become a major field for research and interventions. It has embroiled pharmaceutical companies in lawsuits and possible bankruptcy filings. Effective interventions and policies toward this and future drug-related outbreaks may be improved by understanding the sociostructural roots of this outbreak. Much of the literature on roots of the opioid/overdose outbreak focuses on (1) the actions of pharmaceutical companies in inappropriately promoting the use of prescription opioids; (2) "deat

New Cancer Pain Treatment Options.

PURPOSE OF REVIEW: Cancer pain is often incapacitating and discouraging to patients; is demoralizing to family members and care takers; and is taxing and difficult to subdue for the pain specialists. The consequences of implementing suboptimal treatment are far-reaching; therefore, effective treatment methods are in a great demand. The face of cancer pain management has changed in considerable ways, and interventional procedures have become an integral part of providing multimodal analgesia in cancer pain treatment. The goals of this review are to draw attention to the critical role that regio
