Osteonecrosis of the jaw in a male osteoporotic patient treated with denosumab.

Osteonecrosis of the jaw (ONJ) is a clinical condition associated with long-term exposure to inhibitors of bone resorption, mainly bisphosphonates. Denosumab (DMab) is a human monoclonal antibody of the receptor activator of nuclear factor kappa-B ligand. It prevents osteoclast-mediated bone resorption and is widely prescribed for the management of postmenopausal osteoporosis. Whereas ONJ has already been reported in women treated with DMab, we report for the first time the development of ONJ, following tooth extraction, in a male patient treated for idiopathic osteoporosis with DMab. Due to t

Cost-effectiveness of denosumab in the treatment of postmenopausal osteoporosis in Canada.

OBJECTIVE: Denosumab is a novel biologic agent approved in Canada for treatment of post-menopausal osteoporosis (PMO) in women at high risk for fracture or who have failed or are intolerant to other osteoporosis therapies. This study estimated cost-effectiveness of denosumab vs usual care from the perspective of the Ontario public payer. METHODS: A previously published PMO Markov cohort model was adapted for Canada to estimate cost-effectiveness of denosumab. The primary analysis included women with demographic characteristics similar to those from the pivotal phase III denosumab PMO trial (F

Denosumab osteonecrosis of the mandible: a new entity? A case report.

In the treatment of osteoporosis, M. Kahler and bone metastases from prostate and breast cancer bisphosphonates play a major role. Not all patients respond well to bisphosphonate treatment. Since a few years adverse effects of these drugs have been reported. A new drug, denosumab, a fully human monoclonal antibody to RANKL, has recently been developed. This case reports a 74-year-old male patient with a medical history of diabetes mellitus, angina pectoris, coronary bypasses, hypertension, and prostate cancer with multiple metastases to lymph nodes, bone and lungs. The prostate cancer was trea

Denosumab for the reduction of bone loss in postmenopausal osteoporosis: a review.

BACKGROUND: Osteoporosis is a prevalent condition that may lead to increased risks for bone fracture and other morbidities and increased health care costs. Treatment modalities for osteoporosis aim to prevent further bone loss and to reduce the risk for fracture. Denosumab is a human monoclonal antibody developed for use in osteoporosis. It inhibits the receptor activator of nuclear factor κB ligand, a cytokine that mediates osteoclast-mediated bone resorption. OBJECTIVE: The intent of this article was to review the clinical pharmacology, pharmacokinetic and pharmacodynamic properties, effica

A review of the efficacy and safety of denosumab in postmenopausal women with osteoporosis.

Denosumab, a fully human monoclonal antibody to RANK ligand [RANK-L]) was approved for the treatment of postmenopausal osteoporosis in June 2010, and is highly effective in reducing the risk of vertebral, nonvertebral, and hip fracture risk. The registration of denosumab was the culmination of the discovery and clarification of the internal bone microenvironment regulation of bone remodeling: the osteoblast-produced competitors: RANK-L and osteoprotogerin; and the osteoclast receptor: RANK. When RANK-L is upregulated in the estrogen-deficient state and exceeds the amount of osteoprotogerin, th

Denosumab in postmenopausal osteoporosis: what the clinician needs to know.

Denosumab is a subcutaneously (SC) administered investigational fully human monoclonal antibody to receptor activator of nuclear factor-kB ligand (RANKL), a cytokine member of the tumor necrosis factor family that is the principal mediator of osteoclastic bone resorption. RANKL stimulates the formation, activity, and survival of osteoclasts, and is implicated in the pathogenesis of postmenopausal osteoporosis and other skeletal disorders associated with increased bone remodeling. Denosumab binds RANKL, preventing it from binding to RANK, thereby reducing the formation, activity, and survival o

Initial Denosumab Versus Sequential Bisphosphonate-to-Denosumab for Prevention of Skeletal-Related Events in Breast Cancer with Bone Metastases: A Retrospective, Single-Center Study.

Background: Skeletal-related events (SREs), including pathological fractures, spinal cord compression, radiotherapy to bone, and bone surgery, substantially worsen quality of life in breast cancer with bone metastases. Denosumab, a monoclonal antibody targeting RANKL, mechanistically differs from bisphosphonates and is not renally cleared, offering potential clinical advantages. In practice, an increasing number of patients transition from bisphosphonates to denosumab. However, the comparative effectiveness of sequential therapy versus initial denosumab remains unclear. Methods: We retrospecti

Denosumab-efficacy, safety, persistence and sequential strategies-a systematic review.

INTRODUCTION: Denosumab (DMAB), a monoclonal antibody targeting receptor activator of nuclear factor kappa-Β ligand (RANKL), is an effective antiresorptive therapy for osteoporosis that increases bone mineral density (BMD) and reduces the risk of vertebral and non-vertebral fractures. However, its effects are rapidly reversible, and treatment delays or discontinuation are associated with bone turnover overshoot, loss of BMD, and an increased risk of vertebral fractures. Despite generally high treatment persistence, DMAB is frequently discontinued, necessitating appropriate sequential therapy t

Two cases of tooth resorption in patients receiving denosumab therapy.

BACKGROUND: Multiple external cervical root resorption (MECRR) is a rare, aggressive form of root resorption affecting multiple permanent teeth at the cementoenamel junction. A possible association between denosumab (DMAb), a receptor activator of nuclear factor-κB ligand (RANKL)-inhibiting antiresorptive medication, and MECRR has been proposed, although evidence remains limited. CASE DESCRIPTION: Two retrospective cases of MECRR were identified, including a 72-year-old woman receiving DMAb for osteoporosis and a 65-year-old man receiving high-dose DMAb for metastatic prostate cancer. In both

Monitoring denosumab therapy using the calcium isotope marker (CIM) technology.

Denosumab provides a well-established therapy for osteoporosis. Dual-energy X-ray absorptiometry (DXA) and bone turnover markers (BTMs) track changes in bone mineral density (BMD) and turnover. Calcium (Ca) Isotope Markers (CIM), which measure naturally occurring variations in stable Ca isotope ratios in serum (CIM_serum) and urine (CIM_urine), offer a potentially more sensitive and individualized approach for monitoring bone health and therapy responsiveness. In this pilot study, 13 postmenopausal women with DXA-confirmed osteoporosis were initiated on denosumab. Over 24 weeks, serial measure

Photobiomodulation in Patients Taking Denosumab: Case Report and Literature Review.

Background: Denosumab is a human monoclonal antibody playing a central role in bone resorption. The impaired bone healing observed in patients on denosumab is linked to the drug's inhibition of osteoclast activity. Photobiomodulation (PBM) has garnered attention as a potential adjunctive therapy for managing oral complications in patients on denosumab therapy. The aim of this study is to provide a review of the literature regarding the benefits of photobiomodulation therapy in patients taking denosumab while providing a case report of a patient treated with this therapy. Materials and Methods:

Hypocalcemia after Denosumab in a Pulmonary Hypertension Patient Receiving Epoprostenol.

We report the case of a 50-year-old woman with anorexigen-induced pulmonary arterial hypertension treated with epoprostenol, who presented with Trousseau's sign, leading to the diagnosis of severe hypocalcemia for which substitution was started (initially orally, followed by intravenous substitution). After further analysis, we assume that epoprostenol-induced diarrhea caused malabsorption (as other reasons were excluded), leading to nutritional osteomalacia with secondary hyperparathyroidism. We discovered that even more severe hypocalcemia was induced by the treatment with the anti-osteoporo
