Attitude change during behavioural treatment of sexual inadequacy.

Attitudes towards 'self' and 'partner' were studied in couples undergoing three different behavioural treatments for sexual inadequacy: systematic desensitization with counselling; guided practice with counselling; and practice with minimal counselling. Factor analysis of semantic differential scales identified five components--general evaluation, anxiety, and three factors relevant to sexual evaluation designated as 'loving', 'sexually attractive' and 'easy to arouse'. Differences in derived factor scores were found which related to sex of rater, identity of complainant, and treatment receive

A randomized controlled trial of intranasal oxytocin as an adjunct to exposure therapy for social anxiety disorder.

In humans, oxytocin nasal administration reduces social-threat perception and improves processes involved in communication and the encoding of positive social cues. The aim of this study was to determine whether oxytocin given as an adjunct to exposure therapy improves treatment for social anxiety disorder (SAD) as indicated by a comprehensive set of symptom outcome measures. In a randomized, double-blind, placebo-controlled trial, we administered 24 IU of oxytocin or a placebo in combination with exposure therapy to twenty-five participants who met primary diagnosis for SAD. Participants admi

The Canadian Forces Airsickness Rehabilitation Program, 1981-1991.

Airsickness is a significant obstacle in the training of some student pilots. When conventional therapy fails, desensitization therapy may be indicated. Using experience gained by the RAF and USAF, the Canadian Forces (CF) began such a program in 1981. This paper reports program results from 1981-1991. Following subject identification, treatment consists of three phases: biofeedback relaxation therapy, ground-based desensitization training and in-flight desensitization therapy with a pilot-flight surgeon. Employing a definition of cure used by the RAF, success was compared with that of the RAF

Agoraphobia, compulsive behaviours and behaviour completion mechanisms.

Anxiety is identified with a state of high arousal. Agoraphobia is differentiated from specific phobias which are inherent responses to situations which threatened primitive man. In agoraphobia, attacks of high arousal are produced by situations which delay ongoing activity. It is hypothesised that such delays produce arousal by activating behaviour completion mechanisms. Evidence is reviewed which indicates desensitization has a lasting effect in agoraphobia but not in specific phobias. It is suggested that desensitization reduces the arousal produced by behaviour completion mechanisms. Avers

Psychotherapeutic Interventions Used in Psychological Treatment Studies With Syrian Refugees: A Systematic Review.

The ongoing conflicts in the Middle East have led to a substantial influx of Syrian refugees, exposing them to severe traumatic experiences and contributing to a range of mental health issues. This systematic review examines psychotherapeutic interventions employed in psychological treatment studies with Syrian refugees, focusing on 22 articles identified across Scopus, PubMed, Web of Science, and ScienceDirect. The review highlights the need for psychotherapeutic intervention for Syrian refugees due to the high prevalence of post-traumatic stress, depression, anxiety, grief, and loss which re

Meta-analysis of psychological treatments for posttraumatic stress disorder in adult survivors of childhood abuse.

Posttraumatic stress disorder (PTSD) is highly prevalent in adult survivors of childhood sexual and/or physical abuse. However, intervention studies focusing on this group of patients are underrepresented in earlier meta-analyses on the efficacy of PTSD treatments. The current meta-analysis exclusively focused on studies evaluating the efficacy of psychological interventions for PTSD in adult survivors of childhood abuse. Sixteen randomized controlled trials meeting inclusion criteria could be identified that were subdivided into trauma-focused cognitive behavior therapy (CBT), non-trauma-focu

Usefulness of a trauma-focused treatment approach for travel phobia.

Despite its prevalence and potential impact on functioning, there are surprisingly little data regarding the treatment responsiveness of travel phobia. The purpose of this non-randomized study was to evaluate the usefulness of a trauma-focused treatment approach for travel phobia, or milder travel anxiety arising as a result of a road traffic accident. Trauma-focused Cognitive Behavioural Therapy (TF-CBT), and Eye Movement Desensitization and Reprocessing were used to treat a sample of 184 patients, who were referred to a psychological rehabilitation provider. Patients in both treatment groups

Empirically supported psychological treatments for adult acute stress disorder and posttraumatic stress disorder: a review.

BACKGROUND: Acute stress disorder (ASD) predicts the development of posttraumatic stress disorder (PTSD), which in some sufferers can persist for years and lead to significant disability. We carried out a review of randomized controlled trials to give an update on which psychological treatments are empirically supported for these disorders, and used the criteria set out by Chambless and Hollon [1998: J Consult Clin Psychol 66:7-18] to draw conclusions about efficacy, first irrespective of trauma type and second with regard to particular populations. METHODS: The PsycINFO and PubMed databases

Targeting Shame and Guilt in Psychotherapy: From Conceptual Understanding to Diverse Therapeutic Approaches.

Self-conscious emotions, specifically shame and guilt, are pivotal factors in the onset, maintenance, and treatment of various mental health disorders. While shame involves a devalued sense of the global self and internal/external stigmatization, guilt typically pertains to specific behaviors and the desire for reparation, however it can become problematic when there are inappropriate and excessive attributions of control and responsibility. This virtual issue explores the complex roles these emotions play as both clinical symptoms and barriers within the therapeutic relationship. Through a se

Neuroimaging in post-traumatic stress disorder: a narrative review.

Post-traumatic stress disorder (PTSD) is a psychiatric pathology arising from severe trauma exceeding an individual's adaptive capabilities. Clinical presentations include flashbacks, nightmares, emotional detachment, avoidance of trauma-related stimuli, and heightened vegetative arousal. High-risk groups encompass veterans, survivors of sexual assault, and those impacted by warfare, terrorism or calamities. Therapeutic approaches involve pharmacological and psychological interventions, particularly cognitive-behavioral modalities and prolonged exposure therapy. Eye movement desensitization an

Treatment of PTSD by eye movement desensitization reprocessing (EMDR) improves sleep quality, quality of life, and perception of stress.

The impact of posttraumatic stress disorder (PTSD) on the sleep of patients is widely reported. However, the parameters that can be altered are not the same for all patients. Some studies report an impairment of sleep maintenance and recurrent nightmares, while others failed to find such alterations. Among the many treatments, the eye movement desensitization reprocessing (EMDR) is a therapy used specifically to treat PTSD and general trauma. The purpose of this study was to examine whether EMDR treatment can improve PTSD symptoms, such as sleep, depression, anxiety, and poor quality of life.

Disentangling eye movement desensitization and reprocessing mechanisms of action: The impact of eye movements in the eye blink conditioning task.

OBJECTIVES: Eye movement desensitization and reprocessing (EMDR) is an effective evidence-based treatment for post-traumatic stress disorder. However, the therapeutic mechanisms underlying eye movements (EM) remain unclear. This study aimed to investigate the effect of horizontal EM on fear extinction learning in healthy individuals, using an Eye Blink Conditioning (EBC) task. This experimental paradigm has been widely used to explore associative fear learning and memory as a form of classical conditioning. METHODS: Healthy participants were included to the study protocol and divided randomly

An evaluation of the effectiveness of exposure and response prevention on repetitive behaviors associated with Tourette's syndrome.

Exposure and response prevention (ERP) was evaluated as treatment for three repetitive behaviors in an 11-year-old boy using a multiple baseline across behaviors design. The repetitive behaviors and associated self-reported distress were eliminated. At 3-month follow-up, the frequency for two of the three behaviors returned to baseline levels. This study demonstrates that ERP may be a useful treatment for repetitive behaviors, although booster sessions may be needed to maintain the treatment effects.

Applied relaxation, exposure in vivo and cognitive methods in the treatment of panic disorder with agoraphobia.

Forty-five psychiatric outpatients with DSM-III-R diagnosis of panic disorder with agoraphobia were assessed with a battery of self-report, behavioral and cognitive measures before and after therapy, and at a 1 yr follow-up. They were randomly assigned to either Applied Relaxation (AR; n = 15), Exposure in vivo (E; n = 15) or Cognitive Treatment (CT; n = 15) and received 12 individual therapy sessions, once weekly. All patients also had self-exposure instructions. The three treatments yielded significant improvements that were maintained at follow-up. One criterion of clinically significant im

Self-monitoring of panic attacks and retrospective estimates of panic: discordant findings.

An event sampling method was used to study the frequency of panic attacks during treatment of agoraphobics. Results revealed a much lower incidence of panic attacks in agoraphobics according to self-monitoring than was expected on account of their retrospective estimation. When more stringent criteria for panic attacks are applied, retrospective overestimation becomes even more apparent. The implication of this finding for the classification of panic disorder patients is discussed.

Applied tension, exposure in vivo, and tension-only in the treatment of blood phobia.

Thirty patients with phobia for blood, wounds and injuries were treated individually with applied tension (AT), exposure in vivo (E), or tension-only (T) for 5 sessions. They were assessed on self-report, behavioral and physiological measures before and after treatment, and at a 1 yr follow-up. All groups improved significantly, and the improvements were maintained at follow-up. Applying stringent criteria, 90% of the AT-, 80% of the T-, and 40% of the E-patients were clinically improved at the end of treatment. The corresponding figures at follow-up were 100, 90 and 50%, respectively. AT diff

One hundred and three phobic women not suitable for insight-oriented psychotherapy were randomized to either therapist-directed prolonged exposure therapy (PE), supportive therapy of dynamic type (ST), relaxation therapy (R), or basal therapy (B). Immediately after seeing their therapist for history taking and beginning of treatment, the patients were asked to report their experience of the therapist, of themselves in relation to the therapist, and of the therapy (Experience of Therapy Inventory, ETI). The patients were informed that the therapists would not see the ETI. The patients were rate
