BACKGROUND: It has been observed that replacement of an implantable cardioverter-defibrillator generator in response to a device advisory may be associated with a substantial rate of complications, including death. The risk of lead revision in response to a lead advisory has not been determined previously.
METHODS AND RESULTS: Twenty-five implantable cardioverter-defibrillator implantation and follow-up centers from the Canadian Heart Rhythm Society Device Advisory Committee were surveyed to assess complication rates as a result of lead revisions due to the Sprint Fidelis advisory issued in O

An elderly man with severe chronic obstructive pulmonary disease and a history of complete heart block with pacemaker placement was found to have pacemaker lead infection and required device extraction. He had a standard dual chamber pacemaker in place, however the ECG obtained showed paced QRS complexes with presence of R wave in lead V1 and QS in lead I suggestive of left ventricular pacing. Additional imaging with CT scan obtained for confirmation revealed that the heart was displaced to the left posterior hemithorax secondary to pulmonary disease. Due to significant posterolateral rotation

Leadless pacemakers (LPMs) have emerged as a promising alternative to transvenous systems, particularly in patients requiring transvenous lead removal (TLR) due to cardiac implantable electronic device (CIED) infections. However, real-world outcomes following LPM implantation in this setting remain understudied. Using the National Readmissions Database from 2016 to 2021, we identified 41,058 adult patients who underwent LPM implantation, of whom 3,244 (7.9%) received the device following TLR for CIED infection. Compared to patients undergoing de novo implantation, those in the post-TLR cohort

Background: With increasing cardiovascular implantable electronic device (CIED) implantations, growing number of extractions of leads and devices are noted, mainly for complications such as infection and lead dysfunction. The optimal timing for re-implantation remains uncertain. We investigated the time to eventual re-implantation of CIEDs in the University Hospitals Leuven, Belgium. Methods: All consecutive patients, referred for extraction between January 2005 and December 2016, were analysed for the timing of eventual re-implantation.Results: Two-hundred and forty-three patients were includ

The role of cardiac implantable electronic device (CIED)-related tricuspid regurgitation (TR) is increasingly recognized as an independent clinical entity. Hence, interventional TR treatment options continuously evolve, surgical risk assessment and peri-operative care improve the management of CIED-related TR, and the role of lead extraction is of high interest. Furthermore, novel surgical and interventional tricuspid valve treatment options are increasingly applied to patients suffering from TR associated with or related to CIEDs. This multidisciplinary review article developed with electroph

The cardiac implantable electronic device (CIED) is the therapy of choice for management of symptomatic bradyarrhythmias. However, the indication for CIED implantation in the cases of asymptomatic bradycardias should be carefully individualized. Incidental electrocardiographic findings in asymptomatic patients (e.g., low baseline heart rates, higher than first-degree atrioventricular block or longer pauses) may complicate the physician's decision regarding the necessity of CIED implantation. The main reason is the inherit risk of short- and long-term complications with every CIED implantation,

BACKGROUND: Managing non-functional cardiac implantable electronic device (CIED) leads remains controversial. While extraction is standard for infection, it is unclear whether transvenous lead extraction (TLE) or lead abandonment (LA) provides superior outcomes for superfluous leads. Prophylactic TLE may prevent future infection or venous obstruction and avoid complex delayed extraction but carries immediate procedural risk. Conversely, LA avoids upfront risk, though retained leads may predispose to long-term sequelae.
OBJECTIVES: To compare procedural and clinical outcomes of TLE versus LA f

INTRODUCTION: Tooth extraction is one of the clinical internship requirements in Japan. Human posture during tooth extraction is important since poor posture can cause failure of the safe operation and musculoskeletal injuries. Only a few studies aimed to evaluate the posture, but most of them used manual or complex measurement methods which can lead to some inconvenient problems such as subjective biases, quantitativeness, and device availability. Thanks to the recent advancement of computer vision and technology, pose estimation has been widely used for kinematic analysis. However, none of t

Transvenous lead extraction for cardiac implantable electronic devices remains a high-risk procedure among profound evolutions of extraction techniques. Laser-assisted transvenous lead extraction is required with prolonged lead dwell times or failed mechanical extraction. Tertiary subspecialty support is crucial to mediate the small but significant risk of cardiovascular morbidity and mortality. We report a case of laser lead extraction complicated by internal mammary artery perforation and fistulization, salvaged with endovascular intervention. Multidisciplinary subspecialty support, skills,

BACKGROUND: Cardiac implantable electronic device-related infective endocarditis is a serious and increasingly frequent complication, often presenting atypically in frail patients with multiple comorbidities.
CASE SUMMARY: We report the case of an 83-year-old man admitted with fever and heel pain, who developed heart failure symptoms. Investigations revealed Staphylococcus aureus bacteraemia, osteitis, spondylodiscitis, and a large tricuspid valve vegetation (26 mm) attached to the pacemaker lead, complicated by pulmonary embolism. Given his high surgical risk and resistant Gram-positive bact

Cardiac implantable electronic devices (CIEDs), including cardiac resynchronization therapy-defibrillators (CRT-Ds) and other pacemaker systems, are widely used in patients with varying degrees of heart failure to improve cardiac function and reduce the risk of sudden cardiac death. Despite their life-saving benefits, infections related to CRT-Ds present significant clinical challenges. These infections can lead to serious complications, including endocarditis, pocket infections, and device-associated bloodstream infections, often resulting in prolonged hospitalization and potentially life-thr

INTRODUCTION: Modern cardiovascular implantable electronic devices (CIEDs) have mechanisms that prevent damage from external electric shocks, and malfunction following accidental electrocution is rare. However, the effects of lightning injuries in patients with CIEDs are uncertain.
CASE PRESENTATION: A 74-year-old man with a dual-chamber pacemaker due to complete heart block was struck by a lightning while farming. He had no serious injury at the time and sought medical evaluation 1 month later, when he presented with asymptomatic bradycardia. Device interrogation suggested major battery and

The Japanese Heart Rhythm Society established a nationwide, mandatory, multi-center, prospective, observational registry of transvenous lead extraction (TLE) named the Japanese lead extraction registry (J-LEX) in 2018. As of the end of 2024, a cumulative total of approximately 5400 cases has been registered across 140 participating centers since 2018. The registry includes all consecutive transvenous extractions, excluding surgical-only cases. The indication of TLE was an infection in 54.5%, and the guideline classification was class I in 59.8% and class IIa in 21.8%. As for non-infectious ind

There have been many evolutionary milestones in cardiac implantable electronic device (CIED) therapy over the past few decades. These advancements have created new challenges. Right ventricular pacing was the original (late 1950s) breakthrough, however the risk of pacing induced dyssynchrony has become evident. Biventricular pacing provided a viable way to mitigate dyssynchrony, but only benefits certain patients (primarily left bundle branch block and QRS duration ≥150 ms with depressed left ventricular (LV) function). Areas covered: Recent advances have made His Bundle pacing an option that

Transcatheter extraction of an intracardiac mass is a newer approach that may lead to nonsurgical treatment of complex cardiac masses. We present a case in which thrombectomy devices were combined to extract a right atrial mass, which highlights new frontiers in the treatment of complex transcatheter mass extraction. The combined use of two transcatheter thrombectomy devices (Kong and Godzilla) may provide a powerful addition to the existing armamentarium.

BACKGROUND: Right ventricular (RV) lead perforation is a rare but potentially life-threatening complication of cardiac implantable electronic devices. Although apical lead placement has been proposed as a risk factor, its clinical significance remains uncertain. This study evaluated whether apical RV lead positioning increases perforation risk and characterized the clinical features, diagnosis, and outcomes of affected patients.
METHODS: We retrospectively reviewed data from 1,923 patients who underwent RV lead implantation between January 2002 and June 2023. Patients were categorized into Ap

BACKGROUND: Cardiac implantable electronic device (CIED) implantation is on the rise, accompanied by an increase in its inevitable complications such as different types of CIED infections that require further therapy and potential device extraction. Ensuring efficacy and safety remains paramount in transvenous lead extraction (TLE), given the complex nature of the procedure. The purpose of this study is to assess the outcomes of relatively low-cost mechanical TLE, including mid-term clinical follow-up, and to develop a predictive model for post-TLE survival. This study included all consecutive

AIMS: With the rapid growth of cardiac implantable electronic device (CIED) implantations, lead management has become increasingly challenging. Transvenous lead extraction (TLE) is now a primary approach for addressing lead-related complications. However, data on TLE in patients with abandoned leads remain limited. This study evaluates the safety and effectiveness of TLE in patients with abandoned leads.
METHODS AND RESULTS: Consecutive patients undergoing TLE between January 2013 and January 2021 were enrolled and stratified into two groups: those with abandoned leads presence (Group 1) and

Transvenous lead extraction (TLE) is a complex and technically challenging procedure useful in the management strategy for many complications related to the presence of cardiac implantable electronic devices (CIEDs). The decision to perform lead extraction should take into consideration the clinical indication for the procedure, risks versus benefits, extractor and team experience, and also patient preference for the procedure. A variety of techniques can be used when performing TLE procedures, such as simple traction, traction devices, and various types of sheaths or snares. TLE is a procedur

BACKGROUND: The accurate analysis of extractables and leachables (E&L) from medical devices is crucial for the reliable safety risk assessment of substances to which patients and users may be exposed. The extractable profile of medical devices is often complex and unpredictable, thus improper selection of reference standards can lead to irreproducible chemical analyses between laboratories. ISO 10993-18, the international consensus standard for chemical characterization of medical devices, does not specify a process for selection of appropriate chemical reference standards for non-targeted ana
