Neuropathy patterns differ in patients with diabetic complications.

PURPOSE: Diabetic polyneuropathy (PNP) is an important risk factor for foot ulcers. Diabetic dermopathy is more frequent in patients with diabetic neuropathy. We compared clinical and electrophysiological characteristics of PNP localizations/recurrences of foot ulcers, and diabetic dermopathy (DD) between sexes. METHODS: Eighty-eight diabetic patients (44 men, 44 women) had an evaluation regarding detailed history of their diseases, lesion-related data, and clinical examination. Nerve conduction velocities (NCV), compound motor action potentials (CMAP), distal latencies (DL), and sensory nerv

The impact of diabetic autonomic neuropathy on the incretin effect.

BACKGROUND: The aim of this study was to determine the effect of diabetic autonomic neuropathy (AN) on the incretin effect in patients with type 2 diabetes mellitus (DM2). MATERIAL/METHODS: Forty patients with DM2 (20 with and 20 without AN) and 10 healthy controls were studied. The subjects underwent an oral glucose tolerance test (OGTT) and 7-14 days later an intravenous infusion of 25 g glucose. Blood samples were drawn for glucose, insulin, C-peptide, glucose-dependent insulinotropic polypeptide (GIP), and glucagon-like peptide-1 (GLP-1) determination during the tests. The incretin effect

[Cardiovascular impact of the autonomic neuropathy of diabetes mellitus].

The neuropathic complications related to Diabetes may affect the somatic, sympathetic and parasympathetic nervous system. As a result, there are several clinical manifestations of diabetic neuropathy. They can be related to nervous system lesions of the genital, urinary, gastro-intestinal, skin and cardiovascular tissues. The results of these alterations are loss in the quality of life as well as increase of mortality indexes related to sudden death with cardiac arrhythmias and other causes. The cardiovascular autonomic neuropathy probably contributes to the bad prognosis of the coronary heart

Pregabalin: in the treatment of painful diabetic peripheral neuropathy.

Pregabalin, the pharmacologically active S-enantiomer of 3-aminomethyl-5-methyl-hexanoic acid, has a similar pharmacological profile to that of its developmental predecessor gabapentin, but showed greater analgesic activity in rodent models of neuropathic pain. The exact mechanism of action of pregabalin is unclear, although it may reduce excitatory neurotransmitter release by binding to the alpha2-delta protein subunit of voltage-gated calcium channels. Oral pregabalin at fixed dosages of 300 and 600 mg/day, administered three times daily, was superior to placebo in relieving pain and improvi

Management of neuropathy and foot problems in diabetic patients.

Diabetic neuropathy (DN) is a complex set of clinical syndromes that affect distinct regions of the nervous system, either singly or combined. DN is the most common form of neuropathy in the developed countries of the world and is responsible for 50% to 75% of nontraumatic amputations. It is also the most life damaging--once autonomic neuropathy sets in, the mortality rate approximates 25% to 50% within 5 to 10 years. Distal symmetric polyneuropathy, the most common form of DN, usually involves both small and large nerve fiber damage. Small nerve fiber neuropathies occur early and are often pr

[Polyneuropathy index-revised in the evaluation of diabetic neuropathy].

The polyneuropathy index-revised(PNI-R), based on 8 electrophysiological parameters(conduction velocities and F-latencies), was constructed to obtain an overall estimation of peripheral nerve conduction in diabetic patients, taking PNI as a model. PNI was calculated as a mean percentage of the normal on 12 velocity or latency parameters on motor nerve conduction studies. PNI-R is composed of 8 parameters; motor nerve conduction velocities in the forearm or leg segment and F-wave latencies after wrist or ankle stimulation concerning to the median, ulnar, peroneal and posterior tibial nerves. F-

[Relation between cardiovascular and gastrointestinal neuropathy in diabetics].

BACKGROUND: The presence of autonomic neuropathy impairs the quality of life (orthostatic hypotension, impotence, gastroparesis) or endangers the life of diabetics (sudden death, unawareness of hypoglycemia). The purpose of the investigation was: 1. To assess the presence of the autonomic neuropathy of the cardiovascular and the gastrointestinal systems and their mutual relationship. 2. To assess the relationship of found autonomic neuropathy and the subjective symptoms which are typical to affected particular systems (cardiovascular, gastrointestinal, genitourinary, sudomotor systems and the

Impairment of the respiratory system in diabetic autonomic neuropathy.

Diabetic autonomic neuropathy (DAN) may affect up to 30% of the diabetic population. Sometimes DAN becomes clinically manifest causing specific symptoms and signs; more often, however, DAN is responsible for subtle alterations detectable only by functional tests, as in the case of the respiratory system. At first, abnormalities both in the bronchomotor tone and aspecific airway responsiveness to different stimuli were recognised in diabetic patients with DAN, indicating a defective control of mechanisms which regulate the bronchial calibre in these subjects. Subsequently, peculiar changes in b

Uncommon early-onset neuropathy in diabetic patients.

An acute neuropathy rarely occurs early in the course of diabetes mellitus. Five cases are described of adult patients who developed a peripheral neuropathy at the time or shortly after the onset or discovery of diabetes mellitus. Patient 1, an 80-year-old woman who developed a subacute tetraparesis with proximal and distal muscle weakness with normal cranial nerves, proved to have insulin-requiring diabetes mellitus. In the other patients, all men aged 23-34 years, symptomatic neuropathy occurred simultaneously (patient 2) or 1-6 months after the onset of insulin-dependent diabetes mellitus (

Mexiletine in the symptomatic treatment of diabetic peripheral neuropathy.

OBJECTIVE: To evaluate the efficacy and safety of mexiletine in the symptomatic treatment of diabetic peripheral neuropathy (DPN). METHODS: In this prospective, double-blind study, 29 patients were randomized to receive mexiletine 600 mg/d or matching placebo for 3 weeks. A four-item symptom score (FIS), which rated pain, dysesthesias, paresthesias, and nightly exacerbations of symptoms, and a 100-mm visual analog scale (VAS), which rated pain intensity, were completed by patients before and after treatment. At the end of treatment independent patient and investigator global assessments were

[Clinical aspects, diagnosis and therapy of diabetic neuropathy].

Diabetic neuropathy is regarded as the most common form of neuropathy in the Western industrial countries. At least one third of the diabetic patients is affected by a clinically manifest peripheral neuropathy, which can be diagnosed by the clinical neurological examination without a great deal of effort. The involvement of the autonomic nervous system may be associated with an increased mortality. It may result in erectile impotence as well as severe physical disability from orthostatic hypotension, fecal incontinence or bladder dysfunction and life-threatening complications such as silent my

Quantitative measurement of cutaneous perception in diabetic neuropathy.

To determine the diagnostic value of various cutaneous sensory modalities in diabetic neuropathy, we studied cutaneous perception at the dominant hallux of 113 subjects (32 normal healthy controls and 81 diabetic subjects). The cutaneous sensory perception tests included warm and cold thermal perception, vibration, touch-pressure sensation, and current perception testing (CPT). The sensitivity of each modality when specificity is held greater than 90% was as follows: warm = 78%, cold = 77%, vibration = 88%, tactile-pressure = 77%, 5-Hz CPT = 52%, 250-Hz CPT = 48%, and 2000-Hz CPT = 56%. Cold pressor test in diabetic autonomic neuropathy.

The tests for evaluating sympathetic dysfunction seen in diabetic patients are few and insensitive. For this reason, there are some difficulties in diagnosing sympathetic dysfunction and additional tests are required. The cold pressor test causes a strong sympathetic stimulus and this study investigated whether this test could be helpful in diagnosing sympathetic dysfunction. The cold pressor test was applied to a group of diabetic patients (n = 33) and a control group (n = 15). The mean systolic cold pressor response in diabetic patients was found similar to controls (9 +/- 1.4 vs. 10.6 +/- 1

Mortality in diabetic patients with cardiovascular autonomic neuropathy.

Cardiovascular autonomic diabetic neuropathy (CADN) may carry an increased risk of mortality. However, in previous studies the prognosis of patients with CADN seemed to be influenced by life-threatening macro- and microvascular complications which had already been present at the start of the study period. Between 1981 and 1983, 1015 diabetic patients have been examined for CADN (abnormal heart rate variation at rest and during deep respiration) at the Diabetes Research Institute, Düsseldorf. Thirty-five patients (28 with Type 1 diabetes, 7 with Type 2 diabetes) with CADN have been retrospectiv

[Possibilities of early diagnosis of autonomic neuropathy in diabetics].

The possibilities of the diagnosis of autonomous neuropathy were investigated by the authors in 71 insulin-treated diabetes mellitus patients with the use of dynamic esophageal scintigraphy and determination of cardiac reflex tests. It has been found that the extent and severity of the lesion can be earlier and more exactly recognized with the help of these two methods. Important information can be obtained by the determination of esophageal transit time about the functional condition of vagal nerves supplying gastrointestinal organs. In patients suffering from autonomous neuropathy the esopha

Silent ischaemia in diabetic men with autonomic neuropathy.

Autonomic neuropathy is associated with an increased incidence of silent myocardial infarction and sudden death. The purpose of this study was to investigate the prevalence of silent myocardial ischaemia in diabetic patients with autonomic neuropathy and without. Five standard autonomic function tests were performed on 41 men with diabetes: postural change in blood pressure, postural change in heart rate, heart rate response to deep breathing, heart rate response to Valsalva's manoeuvre, and blood pressure response to sustained handgrip. There were 17 patients with autonomic neuropathy (group

Peripheral neuropathy profile in various groups of diabetics.

The results of applying a battery of peripheral nerve function tests in three groups of diabetics are reported. The tests examined the integrity of all the major subgroups of nerve fibres. The diabetics were grouped according to the clinical severity of their neuropathy: Group I--11 patients with long standing diabetes but clinically insignificant neuropathy, Group II--27 patients with mild neuropathy and Group III--23 patients with neuropathic foot lesions. As expected the incidence of abnormality of all functions increased from Group I to III, but within a group there was no clear pattern of
