STUDY OBJECTIVE: The purpose of this trial was to assess if tramadol wound infiltration is superior to intravenous (IV) tramadol after minor surgical procedures in children because tramadol seems to have local anesthetic-like effect.
DESIGN: Randomized double-blind controlled trial.
SETTING: Postanesthesia care unit.
PATIENTS: Forty children, American Society of Anesthesiologists physical status I or II, scheduled to elective inguinal hernia repair.

In patients undergoing coronary bypass grafting, we noticed that low-dose aspirin failed to inhibit platelet aggregation, potentially elevating the risk of thrombotic bypass occlusion. This "high on-treatment platelet reactivity" was reproducible in vitro and could be transferred with patient plasma or urine to aspirin-sensitive donor platelets, suggesting a drug/drug interaction. Loss of aspirin efficacy was associated with analgesia by dipyrone (metamizol) and initiated further study of the interaction between aspirin and other nonopioid analgesics.

The load capacity of two excipients, a large particle size grade of microcrystalline cellulose (Avicel PH-200) and a coprocessed material composed of lactose and pulverized cellulose (Cellactose) was determined using a model mix with Dipyrone (metamizol sodium) and experiments planned by design of experiments. A multiple-regression model was proposed, and as a result, a new parameter, called specific load capacity SLC was obtained. This constant can be determined for other excipients and drugs; it depends only on the excipient and the drug used.

We report a patient who developed agranulocytosis following exposure to three drugs: amodiaquine, pyrimethamine and dipyrone. The combination of amodiaquine with the patient's serum, obtained during the agranulocytosis, inhibited in vitro granulocyte-monocyte colony-forming unit (CFU-GM) growth of autologous and allogeneic marrow. These results support the view that amodiaquine-induced agranulocytosis is immune in nature. This in vitro approach may be used to study the mechanism of drug-induced agranulocytosis, especially when patients are exposed to multiple drugs.

Twenty-three episodes of drug-induced agranulocytosis admitted to the Beilinson Medical Center between 1960-1972 are reported. An increased incidence in patients admitted with this condition was noted over the years. Dipyrone (6 episodes in 4 patients) and chloramphenicol (5 episodes in 5 patients) were the drugs most commonly associated with this condition. Four patients died in septic shock. The increased incidence of hospital-admitted patients with drug-induced agranulocytosis is related to the increased use of drugs in the Israeli population.

The use of metamizole is controversial, as reflected in the very different national regulations affecting it. The striking disparities between policies for metamizole made us try to determine if there were reasons to ban, limit or liberalize its utilization. An acid-dye colormetric method was described for the determination of chlorpromazine hydrochloride in the compound preparation. The method was based on the reaction of chlorpromazine hydrochloride with methyl orange, to form a yellow complex, which then was extracted by chloroform and exhibited a maximum absorption at 424 nm.
