OBJECTIVES: Examine the association between dementia and all-cause 5-year mortality among skilled nursing facility (SNF) residents exposed to Hurricane Sandy flooding. METHODS: This study analyzed Medicare fee-for-service (FFS) beneficiaries aged ≥65 receiving care in SNFs located in flooded ZIP codes in New York, New Jersey, and Connecticut (October 2012). A 20% Medicare FFS sample was linked to Minimum Data Set assessments, LTCFocus, Care Compare, and American Community Survey data. Flooding exposure was defined using 2012 U.S. Geological Survey flood shapefiles. Follow-up extended 5 years.

The role of a regional trauma system in the management of a mass disaster: an analysis of the Keystone, Colorado, chairlift accident.

On December 14, 1985, the Teller chairlift at the Keystone, Colorado, ski area collapsed, throwing 60 of the 372 people aboard to the ground from heights up to 50 feet. Initial triage and management of the victims was carried out by the local ski patrol, the on-duty physician at the area's Snake River Health Services Clinic, and by volunteer physicians and nurses present at the scene. Thirty-three people required immediate evacuation to hospitals, most of them being transported 75 miles by helicopter air ambulance to level I and II trauma centers in the Denver metropolitan area. Eighteen of th

TRAINING PROGRAM FOR NURSING STAFF REGARDING VIRAL HEMORRHAGIC FEVERS IN A MILITARY HOSPITAL.

Viral hemorrhagic fevers (VHFs) refer to a group of illnesses caused by several distinct families of viruses. In general, the term "viral hemorrhagic fever" is used to describe a severe multisystem syndrome (multisystem in that multiple organ systems in the bpdy are affected). Characteristically, the overall vascular system is damaged, and the body's ability to regulate itself is impaired. These symptoms are often accompanied by hemorrhage (bleeding); however, the bleeding is it rarely life-threatening. [Expert consensus on the clinical application of continuous glucose monitoring in critically ill patients (2026 edition)].

Glycemic fluctuations are highly prevalent among critically ill patients. Glycemic abnormalities constitute an independent risk factor for adverse outcomes, and their effective identification and management are essential to improving clinical prognosis in this population. Continuous glucose monitoring (CGM) technology, which continuously and in real time captures interstitial fluid glucose concentrations, provides multidimensional glycemic metrics and offers a novel technical approach to overcoming the inherent limitations of conventional intermittent glucose monitoring, thereby enabling refin

[Medical students in hospital emergency preparedness in the wake of the COVID-19 pandemic: a qualitative analysis of the ESCAPE study].

BACKGROUND: Hospitals should plan a staffing surge capacity to prepare for a new pandemic or disaster. As medical students are a possible surge capacity resource, this study investigated whether including students in surge capacity plans is desirable and under which conditions it is feasible. METHODS: We performed two retrospective multicentre observational trials in 2023 by enrolling medical students from 20 German faculties and leadership from critical areas of inpatient care at two university hospitals. We investigated the integration of medical students in the COVID-19 effort, assessed th

Primary care occupational, physical, and respiratory therapy role adaptation in the first year of the COVID-19 pandemic.

BACKGROUND: Occupational, physical and respiratory therapists are relatively new to primary care settings, and thus their roles are still emerging. The COVID-19 pandemic was a time of abrupt changes in professional roles. Professional role adaptations are integral to the ability of health care teams to respond to day-to-day care delivery challenges, such as the current physician and nurse shortage, as well as disaster situations. This study explored the role adaptation of occupational, physical, and respiratory therapists in Canadian primary care settings throughout the first year of the COVID

Soaring summer temperatures, systematic urban and political violence, unreliable infrastructure-power outages, water shortages, sporadic transportation and interruption of other basic services-plus the illness, death and economic straits wrought by COVID-19, are what Haitians awake to every day. On the morning of August 14, 2021, they also woke to the earth in the throes of violent, lethal convulsions caused by a 7.2-magnitude earthquake, along the same fault line responsible for the devastating 2010 disaster and stronger still. Surgery in the air--evacuating Finnish tsunami victims from Thailand.

In connection with the Asian tsunami disaster on December 26, 2004, a specially equipped Finnair B-757 airplane capable of evacuating badly injured patients was remodeled into an ambulance airplane. The vehicle could take up to 22 severely injured or ill patients and intensive care and limited surgical procedures could be provided to the patients. The plane was manned with a civilian medical team of 37 physicians and nurses. The plane left for Thailand to evacuate the most severely injured Finnish citizens within 10 hours of the evacuation decision. A total of 14 patients including 4 criticall

Hurricane Katrina and perinatal health.

We review the literature on the effects of Hurricane Katrina on perinatal health, and providing data from our own research on pregnant and postpartum women. After Katrina, obstetric, prenatal, and neonatal care was compromised in the short term, but increases in adverse birth outcomes such as preterm birth, low birthweight, and maternal complications were mostly limited to highly exposed women. Both pregnant and postpartum women had rates of post-traumatic stress disorder similar to, or lower than, others exposed to Katrina, and rates of depression similar to other pregnant and postpartum popu

Individual and environmental risk factors for post-traumatic stress among hospital nurses after the 2024 Noto Peninsula earthquake in Japan.

Although post-earthquake psychological distress arises from a complex interplay of personal vulnerabilities and environmental stressors, the pathways by which these factors interact remain underexplored. We surveyed 327 hospital nurses in Nanao City, Japan, approximately eight months after the magnitude-7.5 2024 Noto Peninsula earthquake; 224 complete responses were analyzed. Participants completed the Impact of Event Scale-Revised and a bespoke questionnaire assessing age, location during the earthquake, seven binary stress factors (home damage, relocation, community damage, change in co-resi

The effect of clothes on sphygmomanometric blood pressure measurement in hypertensive patients.

AIMS: To test whether there is any difference between blood pressure readings with patients wearing clothes under the manometer's cuff and not wearing clothes. BACKGROUND: The few studies published on this subject have shown that blood pressure measurements give similar results whether the patients' arm is covered by clothing or not. However, it has not been clarified whether this is also true in hypertensive patients. METHOD: Blood pressure was measured on non-sleeved arm, sleeved arm and again on non-sleeved arm in 258 hypertensive patients using a mercury-filled column sphygmomanometer. T

Are Saudi Arabian hospitals prepared for the threat of biological weapons?

The use of biological weapons has been recorded repeatedly in history. Until recently, biological terrorism had been little discussed or written about. However, events over the past 12 to 18 months have made it clear that likely perpetrators already envisage every possible scenario. Nations and dissident groups exist that have both the motivation and access to utilize biological weapons. In April 1994, a Russian biological weapons expert presented the conclusions of the Russian experts as to the agents most likely to be used: smallpox, anthrax, and plague. Health care workers in the Kingdom of
