Cervical Spine Pathology Increases the Risk of Rotator Cuff Tear: A Population-Based Cohort Study.

BACKGROUND: Patients with cervical radiculopathy typically present with shoulder pain and weakness; these symptoms are similar to those of rotator cuff disease. Studies investigating cervical spine pathology (CSP) as an independent risk factor for rotator cuff tear (RCT) are lacking in the literature. PURPOSE: To investigate the risk of RCT among patients with CSP who have undergone cervical diskectomy (CD) and to determine whether CD reduces this risk. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: The authors queried the Taiwan National Health Insurance Research Database for p

Feasibility of a resorbable anterior cervical graft containment plate.

In this article we review the background of anterior cervical plating for one- and two-level diskectomy for degenerative disease and provide background justification for the design and testing of a cervical plate composed of a resorbable material. The design of the plate is discussed with special reference to modifications of implant design and implant tools compared with metallic plates that are necessary because of the different mechanical properties of the less rigid material. Our cadaveric and animal in vivo testing methodologies are described and a novel testing method for reliably quanti

Cervical Disk Arthroplasty Failure in a Patient with Klippel-Feil Syndrome: A Case Report.

CASE: A 26-year-old female with Klippel-Feil syndrome (KFS) type III underwent cervical disk arthroplasty (CDA) at C5-6 for radiculopathy unresponsive to conservative care. Initial improvement was followed by recurrence of symptoms despite no implant failure or adjacent segment disease. At 11 months, she underwent revision to anterior cervical diskectomy and fusion (ACDF), resulting in full symptom resolution. CONCLUSION: This case illustrates early CDA failure in a patient with congenital cervical fusion, successfully revised to ACDF. It highlights the uncertain outcomes of motion-preserving

Postoperative intractable leg pain caused by dislocation of drainage tube.

BACKGROUND: A wide variety of conditions can cause recurrent postoperative lumbar radiculopathy. CASE DESCRIPTION: A 49-year-old female developed sudden recurrent postoperative right leg pain after a right-sided L5S1 microdiskectomy for a herniated disc. Emergent magnetic resonance and computed tomography studies demonstrated migration of the drainage tube into the right L5S1 lateral recess compromising the S1 nerve root. Following drain removal, the patient's right reg pain immediately resolved. CONCLUSION: Migration of a lumbar wound drain into the operated lateral recess following a lumba

Definitive diagnosis of cerebrospinal fluid leak into the pleural space using 111In-DTPA cisternography.

A 58-year-old woman with a calcified disk extrusion causing severe spinal stenosis underwent T8 to T9 diskectomy and spinal fusion. A postoperative pseudomeningocele was treated with lumbar drain and fibrin glue. Performed for persistent right pleural effusion, CT myelogram failed to show communication between the cerebrospinal fluid (CSF) and pleural space--even on 2-hour delayed images. Subsequent 111In-DTPA cisternogram clearly demonstrated CSF leakage into the right pleural space at 2 hours, and surgical repair yielded good results. Radionuclide cisternography is a highly useful method to

[One cases of esophageal cervical spondylosis with pharyngeal foreign body sensation].

The patient was treated as pharyngeal foreign body sensation for six months. Laryngeal endoscopy: one about 1 cm x 2 cm x 2 cm, hard, smooth bulge in hypopharyngeal wall at the plane of epiglottis valley. The cervical MRI showed that the C3-C4 disc herniation and degeneration of the C3-C4 vertebrae. Cervical CT showed the C3-C4 disc osteophyte formation and forward bulge. After orthopedic consultation, the patient was diagnosed as esophageal cervical spondylosis. C3-C4 diskectomy and vertebral interbody bone grafted with plate fixation were undertaken. At postoperational day 2, the pharyngeal

Cervical total disk replacement: complications and avoidance.

Anterior cervical diskectomy and fusion for neurologic deficits, radicular arm pain, and neck pain refractory to conservative management are successful. The approach and procedure were first described in 1955 and have become the anterior cervical standard of care for orthopedic surgeons and neurosurgeons. Advancements and innovations have addressed disease processes of the cervical spine with motion-preserving technology. The possibility of obtaining anterior cervical decompression while maintaining adjacent segment motion led to the advent of cervical total disk replacement. The Food and Drug

Surgical management of cervical radiculopathy. Indication, techniques, and results.

Cervical radiculopathy can be surgically approached either posteriorly or anteriorly and the anterior approach has been described with or without fusion. The choice of approach and technique must be based upon anatomic, pathophysiologic, and biomechanical principles in addition to the familiarity of the surgeon with the procedures. The authors discuss the use of the posterior approach for lateral soft-disk disease because it minimizes disruption of soft and bony tissues and does not markedly disrupt the biomechanics of the cervical spine. The anterior approach is preferred for radiculopathy in

Factors Associated With Failure of Symptomatic Relief After Single-Level Posterior Cervical Foraminotomy for Unilateral Radiculopathy.

BACKGROUND AND OBJECTIVES: Posterior cervical foraminotomy (PCF) is a motion-sparing treatment for cervical radiculopathy with rates of symptomatic pain relief similar to anterior cervical diskectomy in patients with persistent symptoms despite nonsurgical management. Although the operation is effective in well-selected patients, some who undergo PCF do not show significant symptomatic improvement, and objective radiographic parameters to identify these patients would assist with operative planning. We sought to determine the radiographic, demographic, and operative factors that are associated

Incidence of C5 Palsy and Recovery Rate After Cervical Spine Surgery: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: C5 nerve palsy is a debilitating complication of cervical spine surgery that can occur after certain approaches. To our knowledge, no previous systematic reviews have compared the rates of C5 nerve palsy after different cervical approaches, identifying the rate at which this complication becomes permanent. We aimed to compare the rates of C5 palsy after different cervical spine surgery approaches and the rates of recovery when C5 palsy was present. METHODS: A literature search of the PubMed and Embase databases from their inception to July 1, 2023, was completed to

[Sciatica caused by avulsion of the vertebral limbus in children].

PURPOSE OF THE STUDY: Sciatica is very rare in children. We report a particular from due to traumatic avulsion of the posterior vertebral limbus. The purpose of this study was to give the principal clinical and radiological aspects of this entity and to discuss the treatment which should be different from that of common adult disk herniation. MATERIALS: From 1980 to 1992, 10 children were examined for a sciatica (7 boys and 3 girls aged from 10 years 1 month to 15 years 2 months). Sciatica appeared in all cases during sport activities and sometimes after a sport accident. Lumbar spine stiffne

Cervical Disk Arthroplasty.

Anterior cervical diskectomy and fusion has been and remains the benchmark surgical management of cervical degenerative disk disease. However, an increased use of cervical disk arthroplasty (CDA) has been found in the past few years. The purported benefits of CDA included preserved motion, less adjacent-level degeneration, and less morbidity. Short-term results from randomized control trials clearly showed noninferiority of CDA compared with fusion. With long-term comparison data becoming available, results are equivalent and superior in many metrics compared, favoring CDA. Concerns remain reg

Controversies in the Management of Cervical Spine Conditions in Elite Athletes.

Cervical spine injuries in elite athletes can have detrimental consequences, which makes return to play for professional athletes after cervical spine injury controversial. Although most athletes can return to sport under some circumstances, such as single-level anterior cervical diskectomy and fusion for cervical disk herniation, return to play after cervical disk arthroplasty and multilevel fusion for cervical disk herniation remains controversial. Allowing athletes to return to play after a finding of cervical stenosis and in the incidence of pseudarthrosis remains unclear. This review prov

Technique for CT guided microendoscopic dissection of the spine.

The indications for percutaneous nucleotomy techniques are limited to disk herniation without dislocated free fragments. In cases with free fragments we have developed a new technique for percutaneous sequestrectomy. Under CT and fluoroscopic guidance spinal endoscopy was performed. In the past, percutaneous diskectomies were X-ray-guided with or without endoscopy. In this paper we describe the combination of CT-scanning, fluoroscopy and microendoscopy for guidance of nucleotomes, lasers, microendoscopes and micro-instruments. The technique was performed on 20 patients (17 outpatients) with ch

[Biomechanical relation of the intervertebral disk, intervertebral foramen and vertebral canal: an experimental study on autopsy specimens].

For the purpose of observing the biomechanical relationship among intervertebral disk (D), intervertebral foramen (F) and vertebral canal (C), 11 sets of functional unit of lumbar spine from fresh cadavers were used as specimens in this experiment. The data showed that the DFC were closely related in dimensional changes with different loadings, a phenomenon we call it DFC variation reaction. It was shown that the loading axes in the disk and in the inter-articular joints worked together biomechanically as an isosceles triangle to accommodate the stresses applied. The mechanism of the relapsed

Lumbar Spine Posttherapeutic Imaging.

Management of patients after lumbar spine surgery or interventional radiology can be complex, and postoperative imaging patterns are often poorly understood by nonspecialized radiologists. This article focuses on postoperative imaging features of the lumbar spine in five clinical settings (with corresponding interventions): vertebral osteoporotic fractures (percutaneous vertebroplasty and vertebral augmentation), lumbar disk herniation (surgical diskectomy and percutaneous interventional radiology), lumbar spinal stenosis (surgical decompression), lumbar spondylolisthesis (surgical decompressi
