New minimally invasive discectomy technique through the interlaminar space using a percutaneous endoscope.

INTRODUCTION: The serial dilating technique used to access herniated discs at the L5-S1 space using percutaneous endoscopic discectomy (PED) via an 8 mm skin incision can possibly injure the S1 nerve root. In this paper, we describe in detail a new surgical procedure to safely access the disc and to avoid the nerve root damage. This small-incision endoscopic technique, small-incision microendoscopic discectomy (sMED), mimics microendoscopic discectomy and applies PED. MATERIALS AND SURGICAL TECHNIQUE: The sMED approach is similar to the well-established microendoscopic discectomy technique. T

Comparison of novel double-needle puncture technique to the conventional TESSYS technique in percutaneous endoscopic lumbar discectomy: enhanced efficiency and reduced radiation exposure.

BACKGROUND: Percutaneous endoscopic lumbar discectomy (PELD) is an effective minimally invasive treatment for lumbar disc herniation (LDH). Establishing a precise working channel is critical for success but is technically demanding with the conventional transforaminal endoscopic spine system (TESSYS) technique, often requiring multiple fluoroscopic attempts. OBJECTIVE: This randomized controlled trial aimed to compare the efficacy and safety of a novel double-needle puncture technique against the conventional TESSYS technique for PELD in treating L4-L5 single-segment LDH. METHODS: Sixty patien

A Postoperative Phenomenon of Percutaneous Endoscopic Lumbar Discectomy: Rebound Pain.

OBJECTIVE: After percutaneous endoscopic lumbar discectomy (PELD), most patients with lumbar disc herniation (LDH) experience relief from the typical symptoms of low back and leg pain. However, for a small number of patients, these symptoms are relieved immediately after surgery but aggravated soon after, and then relieved after short-term full rest or conservative treatment. The aim of the study was to demonstrate this short-term recurrent phenomenon, termed rebound pain. METHODS: A retrospective study was conducted on 144 patients who underwent single-segment PELD from May 2017 to June 2020

A new technique for low back pain in lumbar disc herniation: percutaneous endoscopic lumbar discectomy combined with sinuvertebral nerve ablation.

BACKGROUND: Percutaneous endoscopic lumbar discectomy (PELD) has demonstrated efficacy in alleviating leg pain among patients with lumbar disc herniation. Nonetheless, residual back pain persists as a troubling issue for surgeons following the procedure. In the treatment of discogenic back pain, sinuvertebral nerve radiofrequency ablation has shown promising results. Nevertheless, the potential benefit of simultaneously implementing sinuvertebral nerve radiofrequency ablation during PELD surgery to address residual back pain has not been thoroughly investigated in current literature. METHODS:
