The use of disulfiram for alcohol-dependent patients and duration of outpatient treatment.

Despite the fact that alcoholism is a severe public health problem of worldwide proportions, only a limited number of medications is used as coadjuvant treatment. The objectives of this study were to analyse the use of disulfiram for alcohol-dependent patients and the immediate interruption of treatment following medication prescription. This is a transversal study of 810 patients who attended the Alcohol and Drug Research Unit (UNIAD) during the 2000-2006 period. The study showed that both male and female patients who had remained under treatment during the first year used proportionally more

[Is disulfiram (Antabus) indicated in the treatment of offenders with alcohol use disorders?].

BACKGROUND: Research studies and clinical experience provided us with valuable information about the efficacy of disulfiram. Although supervised disulfiram treatment has been shown to improve outcome in court-referred patients too it is obviously rarely used in German forensic hospitals. METHOD: Two cases illustrated the application of disulfiram to constitute an adjunct to treatment of forensic inpatients. RESULTS: Disulfiram can be used as an adjunct for maintenance of abstinence when the relapse risk is due to insufficient impulse control. DISCUSSION: The decision to use disulfiram to as

Adherence to court-ordered disulfiram at fifteen months: a naturalistic study.

Previously, we observed that the presence of a court-mandate doubled adherence to supervised disulfiram treatment over voluntary supervised disulfiram treatment during the initial twelve weeks. To assess persistence, we conducted a naturalistic followup study of adherence at 15 months in 19 voluntary and 17 court-ordered patients from the original groups. Treatment adherence, measured by the percentage of completed disulfiram clinic visits during the study period, was significantly higher in court-ordered subjects: 61.0% vs. 18.2% in the voluntary group (p<.0001). The data suggest that court a

Do ethanol and metronidazole interact to produce a disulfiram-like reaction?

OBJECTIVE: To obtain and evaluate evidence about the supposed disulfiram-like interaction between metronidazole and ethanol. DATA SOURCES: MEDLINE search from January 1964 to June 1999, using the terms metronidazole, ethanol, and drug reaction. DATA SYNTHESIS: The manufacturer's warnings include a disulfiram-like reaction between metronidazole and ethanol. However, review of reports published between 1969 and 1982 produced no convincing evidence that this reaction exists. Six case reports involving eight patients were evaluated. CONCLUSIONS: Four of the eight cases were serious, including o

Disulfiram produces a non-carbon disulfide-dependent schwannopathy in the rat.

Disulfiram is a dithiocarbamate drug used for alcohol aversion therapy that produces a distal sensorimotor peripheral neuropathy in certain individuals. Because carbon disulfide, a disulfiram metabolite, produces a peripheral neuropathy clinically similar to disulfiram, it has been postulated that disulfiram neuropathy results from CS2 release in vivo. The current study evaluated the morphological changes produced by disulfiram and the contribution of CS2-mediated protein cross-linking to disulfiram-induced neuropathy. Male Sprague-Dawley rats were administered 1% w/w disulfiram in their feed

Combined efficacy of acamprosate and disulfiram in the treatment of alcoholism: a controlled study.

This study presents the results of a multicenter investigation of the efficacy of acamprosate in the treatment of patients with chronic or episodic alcohol dependence. One hundred eighteen patients were randomly assigned to either placebo or acamprosate, and both groups were stratified for concomitant voluntary use of disulfiram. Treatment lasted for 360 days, with an additional 360-day follow-up period. The primary efficacy parameters evaluated were: relapse rate and cumulative abstinence duration (CAD). Results were analyzed according to Intention-To-Treat principles using chi2, t, and multi

The effect of disulfiram on egg shell formation in adult Trichuris muris.

The effect of disulfiram on egg shell morphology in the parasitic nematode Trichuris muris was studied in vitro and in vivo. Daily disulfiram treatment of mice infected with T. muris beginning 25 days after infection and continuing for 26 days resulted in the production of malformed eggs by adult female worms in all treated groups. In addition, significantly fewer adult worms were found at necropsy in mice treated with 5.0 or 7.5 mg/kg/day of disulfiram as compared with mice treated with 2.5 mg/kg/day or control mice. Adult worms collected from infected, untreated mice and placed in aerobic cu

Disulfiram-induced hepatitis. Report of four cases and review of the literature.

Liver test abnormalities following disulfiram therapy are common. However, overt disulfiram-induced hepatitis is rare but has a high mortality rate, specially when the etiologic role of disulfiram is not suspected and treatment is not discontinued. We report four cases of disulfiram-induced acute hepatitis with different degrees of severity and review the cases reported in the literature. The clinical spectrum of disulfiram hepatotoxicity ranges from minor elevation of serum aminotransferases to fulminant hepatitis. Although some patients with disulfiram-induced hepatitis have manifestations o

Does consumption of alcohol in the presence of disulfiram cause brain damage?

After blocking aldehyde dehydrogenase activity with disulfiram in rats, a single injection of ethanol or a single day of inhaling ethanol vapor resulted in neuronal degeneration in the cerebral cortex and elsewhere that was detected by a specific silver stain and by electron microscopy. Treatment of rats with alcohol alone and with disulfiram alone did not cause neuronal degeneration. The minimum blood alcohol levels and disulfiram doses administered to the rats which caused neural degeneration are not distant from those values described in a clinical case of abuse of disulfiram.

A comparative review of the pharmacological and toxicological properties of disulfiram and calcium carbimide.

Disulfiram and calcium carbimide are widely used alcohol deterrents. Their safe use, however, requires a knowledge of their pharmacology, toxicity, and interactions with alcohol and other drugs. The absorption, metabolism, and elimination of these compounds are described, and elimination of these compounds are described, as is the mechanism of their reaction with ethanol. The effects and limitations of the interaction between ethanol and disulfiram and calcium carbimide are discussed. Four areas of concern regarding toxicity are discussed, including medical complications of the reactions with

Disulfiram-like reaction with ornidazole: A case report of a rare interaction.

A disulfiram-like reaction (DLR) associated with ornidazole is a rare but important adverse drug-alcohol interaction. We report a 44-year-old female with no known comorbidities or drug allergies who presented to the emergency department with palpitations, nausea, vomiting, facial flushing, and acral hyperemia. Her symptoms began approximately 30 min after consuming two glasses of raki while receiving ornidazole for a pilonidal cyst. On examination, she was found to be hypotensive and tachycardic. Laboratory results were within normal limits. Her symptoms improved significantly with supportive

Evaluation of disulfiram in experimental trichinellosis through biochemical and histological analysis.

Trichinellosis treatment is complex and must be tailored to disease progression. No single drug is fully effective across all stages. Management includes antiparasitics, inflammation control, pain relief, symptomatic care, and rehabilitation for chronic cases. Disulfiram (DSF) has been recognized to have anti-parasitic effects and several medicinal uses. Consequently, the aim of the study was to investigate the efficacy of DSF in the treatment of the intestinal and muscular phases of trichinellosis in mice compared with albendazole (ABZ). Mice were divided into five groups: negative control; p

Repurposing disulfiram in a promising combination therapy for cryptosporidiosis in immunocompromised mice.

Cryptosporidiosis remains a major health concern, particularly in immunocompromised individuals, due to limited effective treatment options. Nitazoxanide (NTZ) is currently the only FDA-approved drug for cryptosporidiosis, yet its efficacy is significantly reduced in immunosuppressed hosts. This study evaluated the therapeutic potential of disulfiram, an FDA-approved drug for alcoholism, alone and in combination with NTZ, against Cryptosporidium infection in immunocompromised mice. Forty immunosuppressed Swiss albino mice were divided into five equal groups: uninfected controls, infected untre

Antimicrobial pharmacodynamics of vancomycin and disulfiram (Antabuse®) in Staphylococcus aureus.

INTRODUCTION: Intravenous vancomycin (VAN) is the primary treatment for systemic infections due to methicillin-resistant Staphylococcus aureus (MRSA). Pharmacokinetic/pharmacodynamic target (PK/PD) indices for VAN therapies are more difficult to achieve for MRSA isolates with a minimum inhibitory concentration (MIC) greater than 1 µg mL-1. This research investigated the in vitro antimicrobial PD interaction of disulfiram (DSF) with VAN as a potential adjuvant therapy for infections due to these bacteria. METHODS: The antimicrobial interaction was assessed by differential analysis using checke

Disulfiram's journey from rubber vulcanization to T-cell activation.

Disulfiram, a drug prescribed for the treatment of alcohol use disorders for more than 60 years, has recently been repurposed for cancer treatment. New work in The EMBO Journal now describes a disulfiram role in immunotherapy of cancer, involving direct binding to Lck to mediate activation of tumor-infiltrating T cells.

Recognition of a disulfiram ethanol reaction in the emergency department is not always straightforward.

OBJECTIVES: Disulfiram is an adjunct in the treatment of alcohol use disorders, but case reports indicate that disulfiram ethanol reactions are not always recognized in the emergency department. Our first aim is to remind of this risk with two case reports of life-threatening reactions not immediately considered by the emergency physician. The second aim is to estimate the probability that a disulfiram reaction goes unrecognized with the use of a retrospective study of patients admitted to the emergency department. METHODS: Clinical files of patients admitted between October 1, 2010 and Septe

Malignant Catatonia and Neuroleptic Malignant Syndrome in Relation to Disulfiram Overdose.

Disulfiram is a widely used drug in the management of alcohol dependence syndrome as an aversive agent. Although a drug of high efficacy, it has a large number of side effects. Disulfiram-induced catatonia is a known rare side effect of the drug and herein we report a case of what appeared to be the sequential development of malignant catatonia and neuroleptic malignant syndrome in a patient with a diagnosis of alcohol dependence syndrome and co-morbid paranoid schizophrenia following disulfiram overdose. Clinicians need to be vigilant on the emergence of such rare side effects.
