The assessment of balance and dizziness in the TBI patient.

OBJECTIVES: All degrees of traumatic brain injury (TBI) are associated with balance dysfunction and/or dizziness. The clinician assessing and managing patients with TBI should become familiar with vestibular and non-vestibular causes of dizziness and imbalance, and be able to perform screening tests to determine when referral to a vestibular specialist is warranted. This chapter outlines the clinical pathways to be followed in history-taking, physical examination, and assessment. INTRODUCTION: Dizziness, vertigo, balance dysfunction and gait ataxia can have their origin in the vestibular syst

Medication-related dizziness in the older adult.

With increased medication use among the older adult population, adverse drug events and polypharmacy can be significant causes of dizziness in the elderly. The evidence evaluated in this review is helpful in clinical practice but requires an additional detailed investigation into the agents discussed to understand the risk/benefit ratio associated with medications. Examples of medications highly associated with dizziness in older adults and discussed in this review include cardiovascular and central nervous system agents. Several other medication classes associated with dizziness are among the

The evidence base for the evaluation and management of dizziness.

OBJECTIVES: Dizziness presentations pose many clinical challenges. The objective of this study is to broadly summarize the evidence base that supports clinical decisions in dizziness presentations. METHODS: MEDLINE (1966 to September 2007), Web of Science and The Cochrane Library were searched for articles with clinical relevance on topics concerning dizziness. Additional sources were also searched for clinical practice guidelines. The following information was abstracted from each article: year of publication, journal type, type of article and the topics of the article. RESULTS: Of nearly 3

Effect of vestibular rehabilitation on dizziness in the elderly.

OBJECTIVES: Dizziness in the elderly is relatively common, but only a few studies are available. The purpose of this study was to analyze the effect of vestibular rehabilitation on dizziness in elderly patients. MATERIALS AND METHODS: A total of 240 patients older than 70 years with dizziness who visited the dizziness center of a tertiary care university hospital from January 2000 to January 2004 were studied. The patients' charts were retrospectively reviewed. Thorough otolaryngologic and neurotologic evaluations and vestibular function testing were performed in every case to determine the s

[The therapy of patients with dizziness and balance disorders].

A new computerized method is proposed for correction and inhibition of unfavorable illusory (dizziness), vestibular-oculomotor (nystagmus) and vestibular-postural (balance) which allows to teach a subject to block generalization of an afferent signal to effector mechanisms of the central nervous system by developing a fixation reflex, employing a delayed feedback. Two ways of using this method were employed depending on the kind of stimulation programs aimed at inducing abnormal responses of a certain sensory modality (visual or vestibular). The study involved 24 patients with vestibulopathies

Isolated vertigo and dizziness of vascular origin.

INTRODUCTION: Vertigo of vascular origin is usually limited to migraine, transient ischemic attacks, and ischemic or hemorrhagic stroke. Excluding migraines, focal neurologic deficits usually allow a fast identification of the impending neural damage and permit a rapid transfer to a stroke team. Few authors have considered that a vascular origin should be thought of in cases of positional vertigo or vertigo not associated with neurologic signs (ie, isolated vertigo) in patients with vascular disease risk factors. OBJECTIVES: (1) To present a case series of vertigo and dizziness without vertig

Recurrent dizzy spells: all in the head!

Hypoglycaemia is an uncommon manifestation of hypopituitarism in non-diabetic adults. A 47-year-old woman with Cushing's disease was treated with bilateral adrenalectomy and pituitary radiotherapy following unsuccessful trans-sphenoidal surgery. Twenty years later, she presented with a tonic-clonic seizure and recurrent symptoms suggestive of spontaneous hypoglycaemia. Her symptoms continued despite manipulation of her glucocorticoid replacement therapy. Recurrent severe hypoglycaemia resolved after identification and treatment of her concomitant growth hormone deficiency. In patients with hyp

Management of chronic dizziness in elderly people.

Dizziness is a frequent complaint in elderly people, and is a broad term used to explain various abnormal sensations related to the perception of the body's relationship to space. Classically, four subtypes are described: vertigo, pre-syncopal lightheadedness, disequilibrium, and other dizziness. Dizziness is often a chronic complaint in elderly people and may lead to dramatic worsened functional and psychosocial outcomes. Dizziness should be approached as a symptom and as a syndrome. In fact, physicians should exclude potential curable causes of dizziness, considering dizziness as a symptom o

A survey of dizziness management in General Practice.

There is little information in the current literature about the management of dizziness in Primary Care (General Practice). An attempt was made to evaluate this by carrying out a postal survey of General Practitioners' (GPs) treatment methods in four Health Authorities in England and Wales. The survey revealed that eight out of every 1000 individuals are likely to consult their GP on account of dizziness in a 12-month period. The symptom is most common in the 60-80 years' age group, with 'labyrinthitis' being the commonest diagnosis. In the majority of cases (74.9 per cent) there was an improv

[Statistical observation of vertigo and dizziness patients].

Subjects visiting the Department of Otolaryngology at Suita Municipal Hospital reporting vertigo or dizziness numbered 664 women and 343 men from April 1999 to December 2000. As a city hospital, we play an important role in the diagnosis and treatment of acute vertigo or dizziness. The frequency of diagnosis of these cases was divided almost equally into 5 groups: (1) benign paroxysmal positional vertigo (BPPV) 23%; (2) suspected BPPV, 18%; (3) peripheral vestibular disorders other than BPPV, 22%; (4) disorders other than peripheral origin, 18%; and (5) undiagnosed, 19%. Based on our results,

Effects of vestibular rehabilitation on dizziness and imbalance.

Vestibular rehabilitation is a specific approach to physical therapy aimed at reducing dizziness and imbalance by facilitating central nervous system compensation for peripheral vestibular dysfunction. This article reports preliminary results of studies concerning the relative effectiveness of vestibular rehabilitation, general conditioning exercises, and vestibular suppressant medication on dizziness and imbalance in patients with chronic vestibular symptoms of at least 6 months duration. Patients with positional and/or movement-related dizziness and abnormal posturography were randomly assig

The dizzy patient. A practical approach to management.

Dizziness can generally be divided into true vertigo and pseudovertigo (giddiness or light-headedness). The most common causes of pseudovertigo are hyperventilation, orthostatic hypotension, and multisensory deficits of older patients. Of the many types of true vertigo, only a few are caused by serious structural disorders of the brainstem, and these can usually be recognized by their temporal profile and concomitant symptoms and signs. Most cases of vertigo are caused by peripheral vestibular disorders that are self-limiting. Treatment is directed toward control of the acute autonomic symptom

A study of vertigo and dizziness in the premenstrual period.

Twenty-five patients with nausea, vomiting and vertigo or dizziness in the premenstrual period were examined. In the week before menstruation, four of them had spontaneous nystagmus, six had positional nystagmus, and nine of them showed abnormal caloric responses. Most of them showed no abnormality during the week of menstruation. The caloric-induced responses were 'labyrinthine' in type in seven cases, and 'central' in type in two cases, thus denoting that cerebral oedema is not the only factor which may produce vertigo in the premenstrual period; as suggested by the present study, labyrinthi

Dizziness and falling in elderly psychiatric outpatients.

The authors examined 100 psychiatric patients who were 60 years old and older for orthostatic hypotension and symptoms of dizziness and falling. Almost 40% of the patients complained of dizziness and falling, although only 27% had systolic orthostatic hypotension. Drug treatment, particularly the combination of tricyclics with other orthostatic hypotension-inducing drugs, was the most important factor accounting for the dizziness and falling. Underlying medical illness, particularly heart disease, also correlated significantly with the patients' symptoms.

Symposium. ENT for nonspecialists. The dizzy patient.

Dizziness or vertigo may be a perplexing situation to both the patient and the physician, The most important diagnostic factor is a careful history; a dizziness questionaire may be helpful. An otorhinolaryngologic and neurologic evaluation is necessary, and additional consultations may be in order, including vestibular testing and other laboratory studies, The etiology still may not be apparent, and the patient may need to be treated symptomatically and reevaluated periodically.
