A functional immune-based platform for donor-recipient matching in faecal microbiota transplantation for inflammatory bowel disease.

BACKGROUND: Faecal microbiota transplantation (FMT) shows variable efficacy in inflammatory bowel disease (IBD), and current donor selection strategies rely primarily on microbiome characteristics, while host immune responses to donor microbiota remain largely unexplored. Here, we investigated whether recipient-specific immune responses to donor microbiota could be leveraged to develop a personalised donor-recipient matching strategy for FMT in IBD. METHODS: We developed a proof-of-concept (POC) assay, termed Gut Microbiota-Leukocyte Reaction (GMLR), to assess immune compatibility between don

Preventable death: children on the transplant waiting list.

Children, especially those under 5 years of age, have the highest death rate on the transplant waiting list compared to any other age range. This article discusses the concept, supported by OPTN data, that there is an age range of small pediatric donors, which are almost exclusively transplanted into small pediatric transplant candidates. Allocation policies that allow broader sharing of small pediatric donors into small pediatric candidates are likely to decrease death rates of children on the waiting list. As well, although the number of pediatric deceased donors continues to decline, improv

[Organ conservation and transportation modalities].

ORGAN CONSERVATION AND TRANSPORTATION MODALITIES. Organ preservation in transplantation is an essential step in the graft journey between the donor and the recipient. The modalities of preservation have become a major element in this process given the evolution of donors in terms of age and associated comorbidities. This situation has led to the evolution of preservation in terms of the composition of solutions and perfusion technologies for all organs. Several concepts have thus emerged with extracellular type composition and the contribution of new molecules such as high molecular weight pol

Attitudes and disclosure decisions of Finnish parents with children conceived using donor sperm.

STUDY QUESTION: What are disclosure intentions and experiences of heterosexual parents with children born after assisted donor insemination (DI) or IVF with donor sperm (dIVF)? SUMMARY ANSWER: Only 16.5% of Finnish DI/dIVF heterosexual parents had told their child of his/her origin; 18% of all children above 3 years of age had received the information. Parents with older children were more unwilling to tell or were more uncertain regarding what to do than parents with younger children. WHAT IS KNOWN ALREADY: In general, 10-35% of parents of DI offspring are willing to inform their child abou

A standardised model for stool banking for faecal microbiota transplantation: a consensus report from a multidisciplinary UEG working group.

BACKGROUND: Faecal microbiota transplantation is an emerging therapeutic option, particularly for the treatment of recurrent Clostridioides difficile infection. Stool banks that organise recruitment and screening of faeces donors are being embedded within the regulatory frameworks described in the European Union Tissue and Cells Directive and the technical guide to the quality and safety of tissue and cells for human application, published by the European Council. OBJECTIVE: Several European and international consensus statements concerning faecal microbiota transplantation have been issued.

Electronic data base systems support the evaluation of male infertility factors, example cryptorchidism.

BACKGROUND: A new data base system was applied to analyse our patient group with two aims: (a) to analyze the effects of former cryptorchidism on the fertility of OUR infertility patients in comparison with the data of the literature and (b) to evaluate this system in a clinical study. MATERIALS AND METHODS: Using the electronic data base Winsperm 2000, 1,648 infertility patients, 79 patients with testicular cancer and 201 healthy semen donors were examined. RESULTS: A history of cryptorchidism, treated at 6.8 +/- 3.3 years of life, was found in 10.1% of our infertility patient group. The ro

Amniotic Fluid Embolism in Donor Egg Twin Pregnancy: A Clinical Challenge in Critical Care.

Amniotic fluid embolism (AFE) is a rare but life-threatening obstetric emergency marked by respiratory distress, severe hypotension, and coagulopathy with haemorrhage. This case involves a 45-year-old nulliparous woman with a dichorionic diamniotic twin pregnancy via in vitro fertilisation (IVF) with a donor egg. At 36 weeks' gestation, the patient developed sudden respiratory distress following the detection of persistent maternal tachycardia. Cardiotocography (CTG) revealed foetal distress, necessitating an emergency caesarean section, which resulted in the delivery of two live neonates. Her

Risk Factors of Ectopic Pregnancy in Women Undergoing In Vitro Fertilization and Embryo Transfer (IVF-ET).

OBJECTIVE: To identify risk factors associated with ectopic pregnancy (EP) in women undergoing assisted reproductive technology (ART). MATERIAL AND METHOD: A retrospective study was conducted among pregnant women after in vitro fertilization and embryo transfer (IVF-ET) in an assisted conception unit of a university hospital between 1994 and 2013. The factors evaluated included the use of donor oocyte, use of intracytoplasmic sperm injection (ICSI), stage of embryo, number of embryos transferred, and fresh or frozen-thawed cycle. RESULTS: One thousand three hundred eighty five pregnancies we

Strategies to Achieve Nontattooed Flap Transfer Where Decorative Tattoos Are Found at Workhorse Donor Sites.

BACKGROUND: Tattooing in the general population has become increasingly popular in recent years. Tattoo ink may contain carcinogenic chemicals, and long-term health risks remain a concern. When any proposed surgical intervention violates a tattooed area, such as where a tattoo is located in a planned donor site of a workhorse flap, the physical, cosmetic, and psychological effects of both recipient and donor sites on a patient must always be considered. AIM AND OBJECTIVES: In this study, we aimed to develop alternative options for microsurgeons when faced with planned donor sites for workhors

Salivary Microbiota From a Periodontitis Donor Is Associated With Altered IgA-Related Immune Features in Germ-Free Mice.

AIM: The oral microbiota has been implicated in mucosal immune regulation; however, its causal impact on salivary gland-associated immune features remains incompletely understood. In this proof-of-concept study, germ-free (GF) mice were colonised with salivary microbiota derived from either a periodontitis donor or a periodontally healthy donor, and IgA-associated immune readouts were evaluated across multiple compartments. METHODS: GF mice were orally colonised with saliva-derived microbiota from a periodontitis patient or a periodontally healthy donor (or PBS). Total IgA in saliva, serum an

[Transplantation today].

Purpose of this review is, to highlight key issues in transplantation medicine today and to summarise basic information on current state of perfused organ transplantation. There is special emphasis on ethical and legal considerations with regard to living donor transplantation, brain death concept, donor consent and organ allocation. Transplantation frequencies and the impact of whole organ transplantation on the treatment concept for patients with end stage organ disease is heavily dependent on donor and organ availability. Organ donor rate of 25 to 30 donors per million inhabitants per year

Livebirth after uterus transplantation from a deceased donor in a recipient with uterine infertility.

BACKGROUND: Uterus transplantation from live donors became a reality to treat infertility following a successful Swedish 2014 series, inspiring uterus transplantation centres and programmes worldwide. However, no case of livebirth via deceased donor uterus has, to our knowledge, been successfully achieved, raising doubts about its feasibility and viability, including whether the womb remains viable after prolonged ischaemia. METHODS: In September, 2016, a 32-year-old woman with congenital uterine absence (Mayer-Rokitansky-Küster-Hauser [MRKH] syndrome) underwent uterine transplantation in Hos

Transferring more than one embryo simultaneously is justifiable in most patients.

Elective single embryo transfer (eSET) was first introduced to IVF in 1999, and its subsequent integration into mainstream reproductive endocrinology and infertility has been hugely consequential. It can be viewed as the first (among many since) 'add-ons' to IVF that has significantly and adversely affected how IVF is practised, resulting in astonishing declines in live birth rates after fresh non-donor IVF cycles worldwide. We propose that, like most 'add-ons' to IVF over recent years, the almost universal use of eSET worldwide lacks proper validation of its underlying hypothesis and is based

A normal baby is possible despite twins following a single embryo transfer even if one twin is genetically defective.

PURPOSE: To demonstrate that it is important to consider all possibilities when confronted with a decision to terminate a pregnancy because it is likely to be chromosomally abnormal. MATERIALS AND METHODS: A cell free DNA test was performed on a woman with dichorionic diamniotic twins that followed the transfer of a single embryo. RESULTS: The cell free DNA test was consistent with trisomy 21. Two perinatologists favored identical twins and thus suggested termination since both twins would be trisomy 21 and the couple did not want to raise a child with Down's syndrome. Our group suggested th

Knowledge and perceptions of blood donors of the Western Cape Blood Services, South Africa, toward vaginal sample donation for biobanking.

INTRODUCTION: Depletion of Lactobacillus species and an overgrowth of anaerobes in the vaginal tract bacterial vaginosis (BV)], is associated with non-optimal reproductive health outcomes, and increased susceptibility to sexually transmitted infections (STIs). BV is currently treated with antibiotics, although these provide suboptimal cure levels and high recurrence rates. Vaginal microbiota transplantation (VMT), the transfer of vaginal fluid from healthy donors with an optimal vaginal microbiota to a recipient with BV, has been proposed as an alternative treatment strategy. METHODS: Here, w

Comparison of isosorbide mononitrate (Mono Mack) and misoprostol (Cytotec) for cervical ripening in the first trimester missed abortion.

OBJECTIVE: The objective of this study was to compare NO-donor isosorbide mononitrate to misoprostol, both applied as vaginal tablets for cervical ripening prior to first trimester curettage in patients with missed abortion. MATERIALS AND METHODS: Thirty women with missed abortion were assigned after a random list to be treated either with 200 microg gemeprost (Cytotec, Pfizer, Germany) or with 40 mg isosorbide mononitrate for cervical priming at least 3 h before curettage. Vaginal bleeding or the intracervical presence of products of conception was documented. The largest number of Hegar's d

Toward a family-building approach to donor insemination.

Donor insemination (DI) is the oldest and simplest method of "treating" male infertility. The medical profession, prospective parents, legislators, and society in general are increasingly recognizing that providing DI services involves far more than facilitation of conception. This paper explores a multidisciplinary family-building approach to DI as an alternative to a treatment-of-infertility approach, and describes the need to provide patients with a broader and deeper preparation for this special type of parenthood. The short-term medical ease of the procedure is contrasted with the complex

Gamete donors' expectations and experiences of contact with their donor offspring.

STUDY QUESTION: What are the expectations and experiences of anonymous gamete donors about contact with their donor offspring? SUMMARY ANSWER: Rather than consistently wanting to remain distant from their donor offspring, donors' expectations and experiences of contact with donor offspring ranged from none to a close personal relationship. WHAT IS KNOWN ALREADY: Donor conception is part of assisted reproduction in many countries, but little is known about its continuing influence on gamete donors' lives. MOET Efficiency in a Spanish Herd of Japanese Black Heifers and Analysis of Environmental and Metabolic Determinants.

Multiple ovulation and embryo transfer (MOET) systems have been intensively implemented in Japanese Black cattle in Japan and to create Japanese Black herds out of these areas. Environmental conditions influence MOET efficiency. Thus, we describe results of 137 in vivo, non-surgical embryo flushings performed between 2016-2020, in a full-blood Japanese Black herd kept in Spain and the possible effects of heat, year, bull, donor genetic value, and metabolic condition. Additionally, 687 embryo transfers were studied for conception rate (CR) and recipient related factors. A total of 71.3% of viab
