Application of electrolyte analysis of pleural effusion to diagnosis of drowning.

We measured concentrations of sodium (Na), chloride (Cl), calcium (Ca), and magnesium (Mg) in pleural effusion from forensic autopsy cases to examine whether they were useful for a diagnosis of drowning. We analyzed a total of 51 cases (15 seawater drowning, 10 freshwater drowning, and 26 non-drowning), and determined the following reference values. If the concentration of Na or Cl is under 65 mEq/l, a diagnosis of freshwater drowning can be made. If the concentration of Na is higher than 175 mEq/l, or that of Cl is higher than 155 mEq/l, or that of Ca is higher than 16 mg/dl, or that of Mg is

Fatal drowning as a result of an airplane crash--case report.

An ULM (Ultra Light Motorized) plane Aeroprakt 22-L with two men on board dived unexpectedly into the Dworackie lake near the town of Olecko in Warmia and Mazury district, Northeastern Poland. The pilot and the passenger had multiorgan blunt injuries, including subdural and subarachnoid hemorrhage, fractured ribs, fractured thigh (in one of the deceased) and pulmonary contusions as a result of the impact with the lake. The multiorgan injuries and unconsciousness resulting from head trauma prevented the casualties from saving themselves. The underlying cause of the death was the crash into the

An unusual case of homicidal near drowning followed by electrocution.

A young woman was found dead in a bath-tub with her facial orifices under water and current burn marks at the right big toe and at the neck near the hairline. Initial police investigations assumed an accidental electric shock due to a radio falling into the water. The forensic autopsy was followed by extensive histology and immunohistochemistry of the internal organs and skin marks. Intensive vital reactions of a near-drowning were detected that suggested a survival period of at least 20 min. Furthermore, an energy dispersive x-ray spectroscopy analysis on the current burn marks and on possibl

Trends in unintentional drowning: the role of alcohol and medical care.

CONTEXT: During the last few decades, mortality from drowning has decreased in the United States for unknown reasons. It has been hypothesized that this decline may be due to decreased use of alcohol in and around water or improved medical treatment after a submersion. OBJECTIVES: To estimate changes in unintentional mortality due to submersion, estimate trends in drownings attributable to alcohol use, and assess the role of medical care in these mortality trends. DESIGN: A 21-year longitudinal study of case findings, from January 1, 1975, through December 31,1995. SETTING AND PARTICIPANTS:

Drowning deaths in toddlers and preambulatory children in South Australia.

Infants who are preambulatory and toddlers who have only just learned to walk have particular characteristics that give them a unique susceptibility to drowning in certain circumstances. A study of drowning deaths in 32 infants and children <2 years of age in South Australia over a 35-year period from March 1963 to February 1998 was undertaken. The age range was 3 to 24 months (average, 15.4 months), and there was a male:female ratio of 21:11. Drownings occurred in home swimming pools (N = 10); baths (N = 9); waterways (i.e., rivers, irrigation ditches, sea; N = 5); buckets, bins, sinks (N = 4

Care of the pediatric near-drowning victim. A nursing challenge.

In the pediatric near-drowning victim, hypoxia and related pathophysiologic factors affect the pulmonary, neurologic, cardiovascular, gastrointestinal, and renal systems. Other complications arise from the potential for infection; the immobile state; and anxiety and fear related to the hospitalization. Altered coping mechanisms and knowledge deficits related to home care and drowning prevention offer problems that the family must overcome during the recovery of their child from the near-drowning insult. Using this knowledge and the accurate, comprehensive ongoing nursing assessments, the nurse

Swimming pool owners' opinions of strategies for prevention of drowning.

Training in cardiopulmonary resuscitation for pool owners and mandatory placement of a barrier around pools, two commonly suggested strategies for prevention of drowning, will depend, in part, on the support of pool owners to be successfully implemented. To measure this support, an equal probability sample of 795 Sacramento County households with pools was surveyed. An 80% response rate was achieved. A large majority (86%) favored voluntary cardiopulmonary resuscitation training, and a plurality (40%) favored required cardiopulmonary resuscitation certification for pool owners. However, 61% op

Cerebral support for near-drowned children in a temperate environment.

The case histories of three children who were admitted to Modbury Hospital after immersion accidents in the northeastern suburbs of Adelaide during the period from November, 1978, to March, 1979, are presented. Hypothermia of moderate degree was found in one child. A Canadian treatment regimen is reviewed, and a plea made for a more vigorous management of the near-drowned victims of immersion accidents.

Prehospital and Resuscitation Factors Associated With Favorable Pediatric Drowning Outcomes.

OBJECTIVES: Drowning is a leading cause of unintentional injury death in children in the United States. There is a need to identify prehospital factors that affect drowning outcomes. We sought to study prehospital and resuscitation factors associated with favorable neurological outcomes at hospital discharge among pediatric drowning patients. METHODS: This was a retrospective study of drowning patients younger than 18 years in an urban area from 2010 to 2017. Submersion, prehospital, and patient data were obtained from hospital, Emergency Medical Services, and fatality records. The outcome wa

Neuroimaging patterns of brain injury in children following near-drowning.

BACKGROUND: Near-drowning is a prevalent cause of hypoxic ischemic injury in children and young adults. Despite its clinical significance, there is a lack of studies examining the brain magnetic resonance imaging (MRI) findings associated with near-drowning incidents. PURPOSE: The aim of this study is to identify patterns of imaging findings on brain MRI scans of children who have experienced near-drowning. METHODS: This retrospective study included children who experienced near-drowning incidents and had brain MRI scans available for review between November 2000 and September 2023. Abnormal

The utility of drowning site inference through metagenomic diatom analysis.

The diatom test is one of the methods used to diagnose drowning in forensic autopsies. Metagenomic diatom analysis may reveal where a drowning occurred. We evaluated whether metagenomic diatom analysis could be used to infer waters, watersheds, and geographic locations using 166 water samples from 64 locations (freshwater: 55; seawater: 9). Principal component analysis (PCA) in all samples revealed no specific clusters for waters or watersheds. In one river, the three samples at the same site generally tended to be in close clusters, but there were some cases where the three sites were far fro

A case of acquired thrombotic thrombocytopenic purpura following near-drowning.

A 19-year-old woman with a history of asthma presented with acute confusion following a near-drowning event 2 weeks prior to admission. She was found to have severe thrombocytopenia and microangiopathic hemolytic anemia (MAHA). The treatment for thrombotic thrombocytopenic purpura (TTP) was started on the day of admission due to high clinical suspicion. Subsequent workup confirmed a diagnosis of TTP with no clear etiology except the near-drowning incident. TTP following a near-drowning event has never been reported in the literature. Furthermore, she developed refractory TTP that required rein

Near-drowning: new perspectives for human hypoxic acute kidney injury.

Concepts regarding hypoxic acute kidney injury (AKI) are derived from widely used warm ischemia-reflow (WIR) models, characterized by extensive proximal tubular injury and associated with profound inflammation. However, there is ample clinical and experimental data indicating that hypoxic AKI may develop without total cessation of renal blood flow, with a different injury pattern that principally affects medullary thick limbs in the outer medulla. This injury pattern likely reflects an imbalance between blood and oxygen supply and oxygen expenditure, principally for tubular transport. Experime

Comparison of clinical characteristics of intentional vs accidental drowning patients.

PURPOSES: Drowning may happen by accident or as a method of committing suicide. The aim of this study was to determine some characteristics of drowning patients who committed intentionally. METHODS: A retrospective review was performed on 462 patients who visited the emergency department complaining of drowning between January 1998 and October 2011. Of these patients, we only included the patients for whom the cause could be identified. Age, sex, cause, time of drowning, season, mechanism, cardiopulmonary resuscitation performance, body temperature, alcohol ingestion, history of previous suic
