Evaluation of vertical deviations secondary to anatomical abnormalities.

Vertical strabismus secondary to anatomical abnormalities includes orbital anomalies leading to rotation of the extraocular muscle cone, converting the horizontal rectus muscles from adductors and abductors to elevators and depressors. Craniofacial syndromes also may include anomalous or absent muscles, simulating paretic strabismus. Abnormal musculature is also a cause of vertical strabismus. Included in this category is Brown syndrome and Duane retraction syndrome. Unusual strabismus that does not fit known patterns may be caused by anatomical anomalies. High axial myopia can lead to the "he

Duane syndrome with prominent oculo-auricular phenomenon.

Duane syndrome is a congenital cranial dysinnervation disorder involving absent or anomalous innervation of the lateral and medial rectus muscles that is sometimes associated with other manifestations of dysinnervation. We describe a patient with right esotropic Duane syndrome with a long-standing retroauricular tugging sensation in right gaze who was noted to have prominent ipsilateral oculo-auricular phenomenon, representing either abnormal enhancement of existing innervation or an uncommon dysinnervation. After successful strabismus surgery the tugging sensation improved but the phenomenon

[Epidemiological and clinical aspects of Stilling-Turk-Duane syndrome].

BASIS: In a retrospective study on 155 patients with Stilling-Turk-Duane's syndrome, we compare our results with those described in the literature. Particular focus was based on the different clinical entities observed. PATIENTS AND METHODS: Patients were selected from 8000 charts of cases of strabismus seen in the Ophthalmological Clinic of the CHU in Nantes between 1980 and 1995. An oculomotor assessment carried out for all of these patients allowed us to determine the clinical type according to Malbran's classification. Patients were classified on the basis of family history of oculomotor

[Diagnosis and treatment of Stilling-Türk-Duane syndrome (clinical electromyographic and electrooculographic studies)].

The results of clinical, EMG and EOG examinations in 42 patients with Duane's syndrome are analyzed and some characteristic clinical examples are presented. The clinical analysis of motoric and sensoric disturbances in Duane's syndrome reveals elements of concomitant squint in ocular statics. Dynamically, there are marked differences with regard to concomitant and paralytic squint. A number of interesting observations are reported relating to the permanent tonic innervation of the lateral rectus muscle, polymorphic EOG dyssynergia in saccadic and smooth pursuit movements and the progression of

Wildervanck's syndrome--unilateral Mondini dysplasia identified by computed tomography.

We present a case of the Wildervanck (cervico-oculo-acoustic) syndrome exhibiting congenital deafness. Klippel-Feil anomaly and lateral rectus palsy with enophthalmos. Audiometry indicated a predominantly conductive loss which, because of masking difficulties, was assumed to be bilateral: an erroneous assumption supported by results of conventional petrous bone tomography which failed to demonstrate any abnormality of the inner ears. Computed tomography (CT), however, revealed a severe Mondini dysplasia of one ear, a condition which must be assumed to be associated with severe sensorineural he

Isolated atlantal stenosis in a patient with idiopathic growth hormone deficiency, and Klippel-Feil and Duane's syndromes.

OBJECTIVE AND IMPORTANCE: Isolated atlantal stenosis is very rare with less than ten cases reported, mostly in adult Asians; however, this pathology should be recognized by the neurosurgeon. An unusual case of this rare clinical entity is reported in conjunction with multiple other anomalies. CLINICAL PRESENTATION: We report a Caucasian girl with symptomatic stenosis of the first cervical vertebrae who presented with episodes of loss of tone with subsequent falling, facial cyanosis, urinary incontinence, hand weakness, and difficulties with swallowing. This patient also had the diagnoses of D

Human genetic disorders of axon guidance.

This article reviews symptoms and signs of aberrant axon connectivity in humans, and summarizes major human genetic disorders that result, or have been proposed to result, from defective axon guidance. These include corpus callosum agenesis, L1 syndrome, Joubert syndrome and related disorders, horizontal gaze palsy with progressive scoliosis, Kallmann syndrome, albinism, congenital fibrosis of the extraocular muscles type 1, Duane retraction syndrome, and pontine tegmental cap dysplasia. Genes mutated in these disorders can encode axon growth cone ligands and receptors, downstream signaling mo

Magnetic resonance imaging of innervational and extraocular muscle abnormalities in Duane-radial ray syndrome.

PURPOSE: The authors used magnetic resonance imaging (MRI) to study extraocular muscles (EOMs) and nerves in Duane-radial ray (Okihiro) syndrome (DRRS) caused by mutations in the transcription factor SALL4. METHODS: The authors examined four male and two female affected members of a pedigree previously reported to cosegregate DRRS and a heterozygous SALL4 mutation. Coronal T1-weighted magnetic resonance images of the orbits and heavily T2-weighted images in the plane of the cranial nerves were obtained in four subjects. MRI findings were correlated with motility examinations and published nor

The ECEL1-related strabismus phenotype is consistent with congenital cranial dysinnervation disorder.

BACKGROUND: Congenital cranial dysinnervation disorders (CCDDs) are phenotypes of congenital incomitant strabismus and/or ptosis related to orbital dysinnervation. CCDDs have been associated with dominant or recessive monogenic mutations in at least 7 different genes (CHN1, SALL4, HOXA1, KIF21A, PHOX2A, TUBB3, ROBO3) that cause phenotypes such as Duane retraction syndrome, congenital fibrosis of the extraocular muscles, and horizontal gaze palsy with progressive scoliosis. Recently, arthrogryposis with or without strabismus has been shown to be caused by recessive mutations in ECEL1, a gene li

Isolated lateral rectus recession with Y splitting versus anchoring of the lateral rectus muscle in patients with exotropic Duane syndrome.

PURPOSE: To compare the outcome of two surgical procedures used to treat up- and downshoots in Duane retraction syndrome associated with exotropia. METHODS: The medical records of patients with exotropic Duane retraction syndrome seen at a single tertiary care facility from 2009 to 2012 were retrospectively reviewed. Patients were divided into two groups, one comprising patients treated with Y splitting and recession of the lateral rectus muscle; the other, patients treated with anchoring of the lateral rectus muscle to the lateral palpebral ligament. Surgical outcomes were compared in terms

Hearing loss in the cervico-oculo-acoustic (Wildervanck) syndrome.

A 22-year-old woman with mixed hearing loss, Klippel-Feil anomalad (fused cervical vertebrae), and bilateral abducens palsy with retracted bulb (Duane's syndrome) is classified as having the cervico-oculo-acoustic syndrome or Wildervanck syndrome. In this syndrome, the unilateral or bilateral congenital hearing loss is usually severe and of neurosensory origin. To our knowledge, this is the third case of this syndrome with the classical triad and a mixed hearing loss. It is the second case in which an exploratory tympanotomy was performed. An ankylosis of the stapes was found. Stapedectomy and

Cervico-oculo-Acoustic syndrome in a male with consanguineous parents.

BACKGROUND: The cervico-oculo-acoustic syndrome comprises Klippel-Feil anomaly, sensorineural deafness and Duane's retraction syndrome. Polygenic, autosomal dominant, and X-linked inheritance have been hypothesized. The disorder has rarely been reported in males. CASE REPORT: A 42-year-old male, born of consanguineous parents, presented with Duane's syndrome, mixed hearing loss, C2-C3 fusion, neck stiffness, and right facial palsy. A variety of cardiac, neurological and urogenital anomalies occurred in his relatives. The electro-oculographic studies showed impaired abduction and adduction of

Characteristics of Okihiro syndrome.

PURPOSE: To describe Okihiro syndrome, a syndrome consisting of Duane's syndrome, radial ray anomaly, and sensorineural deafness. METHODS: The incidence of anomalies in 41 individuals from 9 affected families was assessed by literature review and patient examination, and the frequency of the characteristics occurring together was determined. RESULTS: Of all the individuals with at least one of the above characteristics, only 12% had all three characteristics. However, 44% had both Duane's syndrome and radial ray anomaly, whereas sensorineural deafness alone occurred in only 17% of patients.

Duane syndrome and congenital upper-limb anomalies. A familial occurrence.

We report a family in which five members in three generations have been afflicted with Duane syndrome. Four of the five members also have congenital hypoplasia of the thenar eminence. One also was afflicted with Hirschsprung disease and another was born deaf. A sixth member, who does not have Duane syndrome, is afflicted with a more extensive malformation of the upper extremities and unilateral deafness. We present a discussion of Duane syndrome and its association with other congenital anomalies. Although some members of this family presented in this report show features that are similar to t

Developmental neurogenetics and neuro-ophthalmology.

The field of developmental neurogenetics has burgeoned over the past decade. Through the combined efforts of developmental biologists, geneticists, and clinicians, genetic defects resulting in neuro-ophthalmic disorders such as holoprosencephaly, microphthalmia, dominant optic atrophy, and optic nerve colobomas have been identified and characterized at the molecular level. Experimental studies in model organisms are continuing to identify novel genes critical for ocular and central nervous system development. Mutations in some of these genes have revealed a spectrum of pathology similar to tha

Loss of MAFB Function in Humans and Mice Causes Duane Syndrome, Aberrant Extraocular Muscle Innervation, and Inner-Ear Defects.

Duane retraction syndrome (DRS) is a congenital eye-movement disorder defined by limited outward gaze and retraction of the eye on attempted inward gaze. Here, we report on three heterozygous loss-of-function MAFB mutations causing DRS and a dominant-negative MAFB mutation causing DRS and deafness. Using genotype-phenotype correlations in humans and Mafb-knockout mice, we propose a threshold model for variable loss of MAFB function. Postmortem studies of DRS have reported abducens nerve hypoplasia and aberrant innervation of the lateral rectus muscle by the oculomotor nerve. Our studies in mic

Update on Congenital Cranial Dysinnervation Disorders (CCDDs).

Congenital cranial dysinnervation disorders (CCDDs) are a group of rare, nonprogressive conditions characterized by abnormal development of the cranial motor nerves and variable ocular motility deficits, ptosis, incomitant strabismus, and facial palsy. Advances in genetics and neuroimaging have revealed that these disorders result from defects in neuronal differentiation or axon guidance of the cranial motor neurons. Duane retraction syndrome, the most common CCDD, results from the absence of the abducens nerve and innervation of the lateral rectus by oculomotor nerve axons; causative genes in

Congenital trigeminal anaesthesia: a rare preventable cause of visual loss in children.

Congenital trigeminal anaesthesia (CTA) is a rare condition characterised by a congenital deficit involving all or part of the sensory component of the trigeminal nerve in children. It is a heterogeneous condition that can present in isolation or is associated with congenital abnormalities affecting the mesoderm, ectoderm and/or brainstem. The authors report a case of a 4-year-old girl who presented with reduced visual acuity, painless bilateral keratitis and painless non-healing lesions on the face, who was confirmed to have CTA on detailed neurophysiological investigations. She also had asso
