We evaluated the usefulness of case-finding instruments for identifying patients with major depression or dysthymia in primary care settings using English language literature from Medline, a specialized trials registry and bibliographies of selected papers. Studies were done in primary care settings with unselected patients and compared case-finding instruments with accepted diagnostic criterion standards for major depression were selected. A total of 16 case-finding instruments were assessed in 38 studies. More than 32,000 patients received screening with a case-finding instrument; approximat

Depressive personality disorder (DPD) is being considered for inclusion in future editions of the Diagnostic and Statistical Manual of Mental Disorders (DSM). However, there is substantial conceptual and empirical overlap between DPD and dysthymic disorder (Dysthymia) criteria, suggesting that these two constructs may not be distinct. Confirmatory factor analysis of the DPD traits and dysthymia symptoms in a large, nonclinical sample (N = 368) indicated that a two-factor model was a better fit than a one-factor model. However, binary diagnostic analysis revealed that over half of the individua

A prospective follow-up of depressed elderly patients is described. The subjects, 91 men and 173 women aged 60 years or over, were diagnosed as depressed in an epidemiological study using the DSM-III criteria. One hundred ninety-nine patients suffered from dysthymic disorder. The mean duration of the follow-up was 14.9 +/- 4.3 months. Outcome was not associated with sex and was good in 41% of the cases. In men, poor outcome was associated with low social participation, low frequency of visiting contacts, and poor insight into the depressive disorder. In women, poor outcome was associated with

There are five categories of psychiatric disorders in DSM-III that embrace depressive moods: adjustment disorder with depressed mood (group 1), bipolar depression (group 2), major depression (group 3), dysthymic disorder (group 4), and atypical depression (group 5). A large sample of patients seen in a metropolitan university psychiatric referral center, with these categories as primary diagnoses in axis I, constitute the subjects studied (N = 2988). The study includes a comparison of the cross-sectional clinical properties of these patients, including an inventory of psychopathological sympto

In this paper we review the research literature on depressive personality. We begin with a brief discussion of the historical antecedents of the current debate, noting the long-standing uncertainty about the relation of this construct to both major mood disorders and normal temperament. Then we examine the DSM-IV Appendix B construct of depressive personality disorder, in particular its controversial overlap with dysthymic disorder. This overlap is discussed within the construct validation criteria proposed by Robins and Guze (1970), highlighting recent developments and responding to criticism

The pivotal role of rumination in relation to other coping strategies was assessed in chronically depressed (dysthymic disorder) individuals versus nondepressed controls. Individuals with dysthymia demonstrated elevated use of rumination and other emotion-focused strategies (emotional expression, emotional containment, self- and other-blame). Among patients with dysthymia, rumination was linked to this limited array of emotion-focused efforts and diminished use of cognitive disengagement, whereas among controls, rumination was correlated with a broad constellation of problem- and emotion-focus

The present study aimed to investigate the prevalence of Axis I disorders in adult inhalant-dependent patients in comparison to other substance-dependent patients and subjects without substance use disorders. The inhalant-dependent group consisted of 83 male inpatient and outpatient adults diagnosed according to DSM-IV criteria. This group was compared with 74 other substance-dependent patients and with 70 subjects without alcohol and substance use disorder diagnoses. Ninety-three percent of the inhalant dependents had a lifetime history of at least one type of comorbid Axis I disorder, while

The antidepressant efficacy of fluoxetine in major depression has been briefly reviewed. A brief outline of dose selection, therapeutic onset, and pharmacokinetics of fluoxetine were made. The potential use of the drug in management of various psychiatric conditions has been examined. These include obsessive-compulsive disorder and related variances, anorexia nervosa, bulimia nervosa, Tourette's syndrome, and trichotillomania. The suggested use of fluoxetine in pain relief in certain diabetics, premenstrual syndrome, and migraine headache were assessed. The reports on the use of fluoxetine in

BACKGROUND: Goethe was one of the most creative poets, scientists and statesmen ever existing. Since the age of fourteen, he suffered from severe mood swings. His descriptions of feelings, emotions, and mental states related to temperamental and poetic melancholy, depressive episodes, dysthymic phases, and creativity are unique in respect to their phenomenological precision and richness. Furthermore, his (self-) therapeutic strategies and his self transformation in literature remain interesting until today for psychopathology, psychotherapy and creativity research.
METHODS: Goethe's self-asse

OBJECTIVE: Many authors have reported discontinuation symptoms associated with selective serotonin reuptake inhibitors (SSRIs). The aim of this study was to investigate the incidence and characteristics of the discontinuation syndrome in patients who stopped treatment with the SSRIs paroxetine and fluoxetine under the usual conditions of clinical practice, and to identify clinical predictors of the syndrome.
METHODS: Ninety-seven outpatients who received an initial diagnosis of dysthymic disorder, who responded to >or=8 weeks treatment with paroxetine (n = 52) or fluoxetine (n = 45), and who

Working in close cooperation with the clinician in charge, the human resource department, and line managers, the occupational physician plays a key role in employees' healthy adaptation to the workplace. In the workplace, it is not sufficient for an employee's depressive symptoms to disappear; it is also necessary that the employee be able to function according to job demands. It is becoming increasing difficult to distinguish between depression and related disorders that manifest depressive symptoms, such as adjustment disorder and personality issues. Using a complex case of dysthymia in the

BACKGROUND: Many drug treatments have been proposed for the treatment of dysthymia, but with so many potential comparisons it is not possible at the present time to determine which is the treatment of choice. There is a need to know whether the different classes of antidepressants have similar efficacy. In addition, the tolerability of treatments may be even more important, since dysthymia is a chronic condition characterised by less severe symptoms than major depression.
OBJECTIVES: To conduct a systematic review of all randomised controlled trials comparing two or more active drug treatment

Dysthymia is characterized by long-lasting periods of lowered mood. Epidemiological studies in the USA and Europe have demonstrated that the prevalence of dysthymia is at least 3% of the general population. Its pervasive occurrence makes dysthymia a public health problem worldwide. One feature of this disorder is its co-occurrence with medical and neurological disorders. A World Health Organization meeting on dysthymia in neurological disorders was held in Geneva, 1-3 July 1996 to address this topic. Some of the major goals of this meeting were to clarify the definition of dysthymia in the pre

Pimozide overdose is uncommon, with limited published cases worldwide. Its long elimination half-life predisposes to delayed and prolonged toxicity, including extrapyramidal symptoms (EPS) that may mimic neuroleptic malignant syndrome (NMS). Herein, we report a case of a 33-year-old female with dysthymic disorder, recurrent depression, and obsessive-compulsive disorder who presented with rigidity, mutism, psychomotor retardation, and reduced responsiveness for 4 days. She had intentionally ingested around 20-30 pimozide tablets 4 days prior. Initial suspicion of catatonia was revised after the

Depression is a common occurrence among epileptic patients and constitutes, along with anxiety disorders, the most frequent psychiatric condition in these patients. The relationship between depression and epilepsy is two-directional, because patients with major depression also have a higher frequency of epilepsy. In epileptic patients, depressive disorders can present as unipolar, bipolar, or dysthymic disorders. More characteristically, however, they present as an atypical depression, which can often go unrecognized for long periods of time. In the diagnostic evaluation of these patients, cli

The incidence of violent crimes committed by youthful offenders in the United States is increasing. In this report, 150 inmates in a prison for youths (ages 14 to 24 years) who were treated by a psychiatrist were compared with 150 control subjects on several parameters including those who had committed violent crimes (V) versus those who had committed nonviolent crimes (NV). There were more similarities than differences between those who had committed V versus NV crimes. There was no statistically significant difference between the V and NV groups in most diagnostic categories, the Beck Hopele

The xanthurenic acid (XA) flow in 24-hour urine following L-tryptophan loading was determined in a control group and in depressive patients divided into two DSM-III categories: minor depression (dysthymic disorder, atypical depression, adaptation disorder with depressive mood), and major depression. The XA flow is not significantly (p = 0.15) different in these categories of depression and in the control group. The number of patients with an abnormal XA flow (cut-off value greater than or equal to 106.8 mu mol/24 hours) is not significantly different among these groups (p = 0.40). The XA flow
