Hormonal and metabolic modulation through nutrition: towards a primary prevention of breast cancer.

Breast cancer (BC) is a polygenic and multifactorial disease for which estrogens have been recognized as the main risk factor, and for which lifestyle plays a key role. Previous epidemiologic cancer research performed in Uruguayan population delimited its dietary and anthropometric profiles. Recognizing the difficulty for universalizing a nutritional basis for prevention due to different eating patterns among regions and countries, we summarize the existent knowledge linking nutrition, estrogens, metabolism and BC. As an attempt towards primary prevention of BC, we present recommendations main

Behaviour change interventions for the control and elimination of schistosomiasis: A systematic review of evidence from low- and middle-income countries.

BACKGROUND: For the last two decades, schistosomiasis control efforts have focussed on preventive treatment. The disease, however, still affects over 200 million people worldwide. Behaviour change (BC) interventions can strengthen control by interrupting transmission through modifying exposure behaviour (water contact) or transmission practices (open urination/defaecation); or through fostering treatment seeking or acceptance. This review examines these interventions to assess their effectiveness in modifying risk practices and affecting epidemiological trends. METHODOLOGY/PRINCIPAL FINDINGS:

Epidemiology of spinal muscular atrophy in Aotearoa-New Zealand.

BACKGROUND: The advent of three effective disease modifying therapies for SMA has highlighted the need to understand the epidemiology of spinal muscular atrophy (SMA) and its disability impact. OBJECTIVE: We aimed to establish the nationwide incidence and prevalence of SMA in Aotearoa-New Zealand, and to estimate the patients' disability and the impact of this on health resource utilisation. METHODS: We used multiple sources to identify patients with SMA and verified the diagnosis, disabilities and resources utilisation by review of the individual patient notes and genetic results. The four

Post-disappearance scenarios: policy implications following the potential disappearance of B/Yamagata lineage influenza viruses.

The COVID-19 pandemic and related preventive measures reduced influenza virus circulation, notably causing the disappearance of the B/Yamagata lineage of influenza viruses. In this Perspective, we discuss the implications that this development may have for global influenza epidemiology, and the adjustments that may need to be implemented concerning surveillance strategies and practices, laboratory safety protocols, and influenza vaccine formulations. The disappearance of the B/Yamagata lineage might indeed alter the dynamics of the influenza disease burden (although in a way that is difficult

Metabolomic Profiles and Heart Failure Risk in Black Adults: Insights From the Jackson Heart Study.

BACKGROUND: Heart failure (HF) is a heterogeneous disease characterized by significant metabolic disturbances; however, the breadth of metabolic dysfunction before the onset of overt disease is not well understood. The purpose of this study was to determine the association of circulating metabolites with incident HF to uncover novel metabolic pathways to disease. METHODS: We performed targeted plasma metabolomic profiling in a deeply phenotyped group of Black adults from the JHS (Jackson Heart Study; n=2199). We related metabolites associated with incident HF to established etiological mechan

Nutrition and Atherosclerosis.

Cardiovascular disease (CVD) is a universal problem in modern society. Atherosclerosis is the leading cause of CVD resulting in high rate of mortality in the population. Nutrition science has focused on the role of essential nutrients in preventing deficiencies, at the present time, the nutritional strategies are crucial to promote health and intervene with these global noncommunicable diseases. In many cases, diet is a major driving force, which is much easier to change and follow than other factors. It is important to establish that the first strategy to treat atherosclerosis is to modify li

The relationship between depression, daytime napping, daytime dysfunction, and snoring in 0.5 million Chinese populations: exploring the effects of socio-economic status and age.

BACKGROUND: Daytime napping has been postulated as both a protective and a risk factor for depression in previous studies. In addition to these conflicting results, research gaps also exist with regard to controlling confounding bias between daytime napping and depression and examining the potential association within the Chinese population. To facilitate the prevention and diagnosis of depression, this study aims to provide insight into the association of daytime napping and depression in 0.5 million Chinese adults by fully controlling confounders, and further examine the modifying effects of

Performance of seven carbapenemase detection assays in Pseudomonas aeruginosa across different epidemiological settings: a multicenter cross-sectional study.

The detection of carbapenemases in Pseudomonas aeruginosa remains challenging due to a great variety of other resistance mechanisms, and most laboratories, therefore, do not test for them. This study aimed to comparatively evaluate seven phenotypic carbapenemase detection assays across three epidemiological settings. A total of 320 P. aeruginosa isolates from three German centers with varying carbapenemase prevalences (5.8%-51.4%), including 113 carbapenemase-producing isolates carrying VIM-2 (n = 58), NDM-1 (n = 19), and GIM-1 (n = 16), underwent whole-genome sequencing as reference to assess

DHA in Alzheimer's Disease: The translational gap between mechanistic complexity and clinical inconsistency.

Alzheimer's disease (AD) is a neurodegenerative disorder characterized by progressive cognitive dysfunction. Docosahexaenoic acid (DHA), the most abundant ω-3 polyunsaturated fatty acid (PUFA) in the brain, is essential for neuronal structure and function. Although epidemiological studies generally associate higher DHA intake or blood levels with better cognitive outcomes, clinical findings remain inconsistent, partly because DHA intake is closely linked to healthier dietary patterns and lifestyles. This review summarizes the role of DHA in AD from mechanistic, epidemiological, and clinical pe

Effect Modifiers of the Association Between Physical Activities and Subsequent Low Back Pain Flares.

STUDY DESIGN: Prospective case-crossover study. OBJECTIVE: This study evaluated whether concurrent actions, work-relatedness, and analgesic use modify the associations between physical activities and subsequent LBP flares. SUMMARY OF BACKGROUND DATA: Prior longitudinal case-crossover studies found transient risks of various activities on LBP flares. These studies have focused on the main effects of exposures for LBP. However, little is known about contextual factors that could modify activity-flare relationships. METHODS: This was a secondary analysis of a 1-year prospective case-crossover

Smoking (active and passive) and breast cancer: epidemiologic evidence up to June 2001.

The first generation of studies evaluating the association between exposure to tobacco smoke and breast cancer merely compared active to nonactive smokers, with varying degrees of detail in the definition of active smoking. With rare exceptions, studies of this kind failed to show an effect of smoking on breast cancer risk. However, such analysis is probably insufficient. The most recent reports on the smoking-breast cancer connection have two characteristics. Some have separated women exposed to passive smoking from those nonexposed to either active or passive smoke. Other reports have focuse

Entertainment activities and the risk of multiple sclerosis: A Mendelian randomization analysis.

Modifying environmental and lifestyle factors may have the potential to prevent and ameliorate multiple sclerosis. Further elucidating the etiology of multiple sclerosis and proposing actionable prevention measures are of significant importance, but establishing causality in epidemiological data can be challenging. This study employed a two-sample Mendelian randomization analysis to evaluate the causal effect of entertainment activity factors on the risk of multiple sclerosis. Publicly accessible summary statistics derived from genome-wide association studies were utilized to assess 14 modifia

Structural diversification of vitamin D using microbial biotransformations.

Vitamin D deficiencies are linked to multiple human diseases. Optimizing its synthesis, physicochemical properties, and delivery systems while minimizing side effects is of clinical relevance and is of great medical and industrial interest. Biotechnological techniques may render new modified forms of vitamin D that may exhibit improved absorption, stability, or targeted physiological effects. Novel modified vitamin D derivatives hold promise for developing future therapeutic approaches and addressing specific health concerns related to vitamin D deficiency or impaired metabolism, such as avoid

Sexual health and function in liver disease.

Sex is a central aspect of human life and is significantly impacted by chronic illness. Cirrhosis, due to its unique pathophysiology and the side effects of common therapies, serves as a paradigmatic example, being associated with very high rates of sexual dysfunction in both men and women. Liver transplantation can modify certain hormonal and pathophysiological aspects related to sexual dysfunction, but complete recovery occurs in only a relatively small percentage of patients. This review examines the pathophysiology, epidemiology, and management of sexual and reproductive dysfunction in pat

Leflunomide: a manageable safety profile.

The safety profile of leflunomide in the treatment of rheumatoid arthritis has been well documented in clinical trials, postmarketing surveillance, and epidemiological studies. Both postmarketing surveillance and epidemiological study results are consistent with the safety profile of leflunomide reported in clinical trials, and no increased risk was observed with leflunomide, compared with other disease modifying antirheumatic drugs; these studies have allowed a more precise representation of the incidence of adverse events occurring with leflunomide under normal conditions of care. A meta-analysis of alcohol drinking and cancer risk.

To evaluate the strength of the evidence provided by the epidemiological literature on the association between alcohol consumption and the risk of 18 neoplasms, we performed a search of the epidemiological literature from 1966 to 2000 using several bibliographic databases. Meta-regression models were fitted considering linear and non-linear effects of alcohol intake. The effects of characteristics of the studies, of selected covariates (tobacco) and of the gender of individuals included in the studies, were also investigated as putative sources of heterogeneity of the estimates. A total of 235

Cognitive Impairment Across the Spectrum of Chronic Kidney Disease: Clinical Characteristics and Implications for Practice-Is There a Role for Medical Nutritional Therapy?

Cognitive impairment is increasingly recognized as a frequent and clinically significant complication of chronic kidney disease (CKD). Across the CKD spectrum, patients commonly exhibit a cognitive profile dominated by executive dysfunction and slowed processing speed, reflecting cumulative vascular, metabolic, and systemic burden. Cognitive impairment is not merely an epiphenomenon of aging but a modifier of clinical trajectory, with implications for treatment adherence, decision-making capacity, dialysis planning, transplantation, and mortality risk. This narrative review synthesizes current

The associations of zinc and GRIN2B genetic polymorphisms with the risk of dyslexia.

BACKGROUND: Zinc is an essential trace element and a number of studies have identified the importance of zinc in neurodevelopment in children. However, epidemiologic data on the associations of zinc and genetic susceptibility with the risk of dyslexia are limited. We aimed to investigate whether genetic polymorphisms in GRIN2B genes modify the association between zinc levels and dyslexia risk. METHODS: A case-control study of 240 dyslexic children and 230 healthy controls was conducted in Wuhan, Hangzhou and Jining city in China from April 2017 to April 2018. Zinc concentrations in urine samp
