[Possible mechanisms of action of electroconvulsive therapy].

Electroconvulsive therapy (ECT) remains one of the most effective biological treatment methods in psychiatry. The development of convulsive treatment methods was based on a theory of its mechanism of action, namely the presumed biological antagonism between schizophrenia and epilepsy. Later studies did not confirm this antagonistic diseases theory, but intensive research started to clarify ECT's mechanism of action. In early studies on ECT, attention was drawn to the anticonvulsant effect , its impact on cerebral circulation and the change of permeability of the blood-brain barrier. Later rese

Electroconvulsive therapy and women: An international survey.

858 electroconvulsive therapy (ECT) recipients, from 44 countries, responded to an online survey. In keeping with previous studies, the majority (73%) were women. Most of the psychiatrists giving ECT (81%) were men. Women patients were less likely than men to report improved mood following ECT. Consistent with previous smaller studies, women patients also reported worse outcomes than men for multiple adverse effects, including anterograde and retrograde memory loss, and for how "harmful" ECT was in general. Even fewer women (15%) than men (29%) said they would want to have ECT again. Implicati

Electroconvulsive Therapy and Hyperventilation: A Narrative Review.

Electroconvulsive therapy (ECT) is a non-pharmacological biological treatment method used to treat major depression, bipolar disorder, schizophrenia, catatonia, and some other psychiatric conditions. Despite its high effectiveness, it is often used when other methods, such as pharmacotherapy and psychotherapy, fail to improve treatment outcomes. The refinement of this particular therapy may increase the popularity of this method, and among the currently studied therapy modifiers is protocolised hyperventilation. Hyperventilation is implemented to improve ventilation and gas exchange, reduce sh

Effects of different indications on electroconvulsive therapy.

OBJECTIVE: The objective of the study is to evaluate how electroconvulsive therapy (ECT) affects treatment-resistant depression, bipolar and schizophrenic patient groups, and suicide attempt histories and to evaluate the relationship between treatment variables and patient outcomes. METHOD: In a retrospective cohort study at the inpatient psychiatry clinic of Çam and Sakura City Hospital between January, 2021, and February, 2023, 103 patients receiving ECT were analyzed. They were categorized into two groups according to indications that suicide risk (n = 76) and resistance to pharmacotherapy

[Electroconvulsive therapy in people with intellectual disability].

Electroconvulsive therapy (ECT) is a highly effective treatment option for severe mental illness. Although people with intellectual disability (ID) have similar prevalence rates of mental disorders in comparison to the general population their access to ECT remains challenging. A systematic literature review was carried out on treatment with ECT in patients with ID and a case report on a patient with ID who underwent ECT is presented, to highlight a typical clinical routine. A total of 100 articles with 208 different case reports were retrieved. In summary, the results underline the effectiven

Electroconvulsive Therapy for the Treatment of Somatic Delusions.

Somatic delusions occur in various psychiatric disorders and are associated with higher mortality and lower quality of life. In this case report, we present a 68-year-old man with the diagnosis of schizoaffective disorder, bipolar type with associated somatic delusions, and auditory hallucinations. His somatic delusions were alleviated by the 20th ECT treatment with additional clinical improvements in his speech, thought processes, and judgment. This case report supports the utilization of ECT for patients with schizoaffective disorder and somatic delusions.

Outcome prediction of electroconvulsive therapy for depression.

INTRODUCTION: We developed and tested a Bayesian network(BN) model to predict ECT remission for depression, with non-response as a secondary outcome. METHODS: We performed a systematic literature search on clinically available predictors. We combined these predictors with variables from a dataset of clinical ECT trajectories (performed in the University Medical Center Utrecht) to create priors and train the BN. Temporal validation was performed in an independent sample. RESULTS: The systematic literature search yielded three meta-analyses, which provided prior knowledge on outcome predictors

The Use of Electroconvulsive Therapy on Death Row.

Despite high rates of mental illness among incarcerated people in the United States, use of electroconvulsive therapy (ECT) remains limited in jails and prisons. There are some published guidelines regarding the provision of mental health care, including ECT, in U.S. correctional facilities, but little attention has been paid to the use of ECT for individuals sentenced to death. This article examines ECT within the context of the death penalty, including court consideration of ECT in capital cases and historic uses of ECT to facilitate execution of people on death row. Given the unique clinica

Electroconvulsive Therapy in a Patient With a Liver Transplant.

Depression is common in patients after liver transplantation and, when severe, can threaten both viability of the graft and the patient's life. Electroconvulsive therapy (ECT) is a safe and highly effective therapy for severe depression, but there have been very few reports of its use in liver transplant recipients. We describe a patient who had undergone a successful orthotopic liver transplant 26 years previously, who developed severe major depression. The patient tolerated an index course of ECT well and responded robustly. There were no complications pertinent to the graft or otherwise. Li

Electroconvulsive Therapy as a Treatment for Somatization Disorder.

OBJECTIVE: To review the published literature over the last 10 years for the efficacy of electroconvulsive therapy (ECT) in refractory somatization disorder. DATA SOURCES: A comprehensive evidence search of the published literature in the last 10 years (2010-2020) was conducted using the search terms ECT, electroconvulsive therapy, efficacy, effectiveness, use of ECT, chronic pain, somatoform disorders, somatoform pain, somatic symptom disorder, and somatization disorder. The review was limited to articles written in the English language. Databases searched included PsycInfo, MEDLINE, PubMed,

Challenges and pitfalls in anesthesia for electroconvulsive therapy.

Electroconvulsive therapy (ECT) refers to the application of electricity to the patients' scalp to treat psychiatric disorders, most notably, treatment-resistant depression. It is a safe, effective, and evidence-based therapy that is performed with general anesthesia. Muscle relaxation is used to prevent injuries related to the tonic-clonic seizure caused by ECT. Hypnotics are administered to induce amnesia and unconsciousness, so that, patients do not experience the period of muscle relaxation, while the generalized seizure is left unnoticed. For the anesthesiologist, ECT is associated with t

Insight and capacity to consent to electroconvulsive therapy.

OBJECTIVE: To critically examine a recent decision of the Victorian Supreme Court that found that the Mental Health Tribunal and the Victorian Civil and Administrative Tribunal erred in the application of the capacity test in the Mental Health Act 2014 (Vic) and that compulsory electroconvulsive therapy would infringe upon the human rights of two patients who had no insight into their chronic schizophrenia. CONCLUSIONS: After considering the concepts of insight and capacity to consent to treatment, the paper concludes that the decision in NJE and PBU v Mental Health Tribunal [2018] VSC 564 is

Electroconvulsive therapy in a patient with moyamoya syndrome.

We report on a 30-year-old woman diagnosed with moyamoya syndrome resulting from sickle cell disease who developed catatonia and was successfully treated with electroconvulsive therapy (ECT). Neuroimaging revealed severe tandem narrowing of the left internal carotid artery with diminished cerebral blood flow, moderate narrowing of the right supraclinoid aspect of the right internal carotid artery, and associated numerous lenticulostriate collaterals bilaterally, consistent with moyamoya. The patient presented with mutism; posturing; immobility; stupor; withdrawal; refusal to eat, drink, or spe
