[Airway obstruction and massive subcutaneous emphysema].

Airway obstruction caused by massive subcutaneous emphysema (MSE) in the chest and neck region is a rare complication to tension pneumothorax. We describe the condition in a 91-year-old male in whom rapidly progressing MSE caused airway obstruction. As a result of progressing airway obstruction acute intubation was performed prior to completion of tube thoracostomy resulting in immediate improvement of the condition. This case demonstrates that MSE can lead to acute airway obstruction. Rapid intubation can in this situation be required to prevent asphyxiation.

Extensive subcutaneous emphysema resembling necrotizing fasciitis.

Necrotizing fasciitis is an aggressive, invasive soft tissue infection. Because it can rapidly progress to patient instability, prompt diagnosis followed by urgent debridement is critical to decreasing mortality. Despite the importance of early diagnosis, necrotizing fasciitis remains a clinical diagnosis, with little evidence in the literature regarding the effectiveness of diagnostic tools or criteria. Common clinical findings are nonspecific, including pain, blistering, crepitus, and swelling with or without fever and a known infection source. This article describes a patient who was transfe

Asymptomatic spontaneous cervical and mediastinal emphysema.

We report our experience with four patients presenting with spontaneous pneumomediastinum (SPM) within a 3-month period. The patients (three male and one female, aged 15-17 years) were hospitalised with SPM. All patients were kept under observation, successfully treated and followed up for several years, with no recurrences reported. Two patients had histories of asthma, while the other two developed SPM during sporting activities. SPM carries the possibility of being latent. For symptoms such as chest pain without evidence of pneumothorax in young people, it is necessary to always cons

Camouflaging Facial Emphysema: a new syndrome.

Camouflaging Facial Emphysema, as is defined in this paper, is the result of a simple technique used by the patient to deform his face in order to prevent recognition at a police identity parade. The patient performs two punctures in the mucosa at the rear of the upper lip and, after several Valsalva manoeuvres, manages to deform his face in less than 15 min by inducing subcutaneous facial emphysema. The examination shows an accumulation of air in the face, with no laterocervical, mediastinal or thoracic affectations. The swelling is primarily observed in the eyelids and the orbital and zygoma

Spontaneous orbital subcutaneous emphysema after sneezing.

Orbital subcutaneous emphysema develops when air enters the surrounding soft tissue. This occurs as a result facial bone trauma, iatrogenic dental and otolaryngeal procedures, and gas-producing infectious microorganisms. Case reports regarding this phenomenon after sneezing are very uncommon. Although orbital subcutaneous emphysema is not a true emergency, it can be distressful to patients. This case serves to bring awareness to emergency department physicians regarding the possibility of a nontraumatic orbital subcutaneous emphysema and its related complications.

Spontaneous pneumomediastinum presenting with retropharyngeal emphysema.

Spontaneous pneumomediastinum is a rare condition that may follow increases in intrathoracic pressure. Patients may present with symptoms isolated to the head and neck. Although this condition is usually benign, there are other potentially fatal conditions, which often present similarly and clinically are difficult to differentiate from this entity. Despite this condition being described in the thoracic and medical literature, very little is found in the otolaryngological literature. This article reports 4 cases in which presenting symptoms were limited to the head and neck and otolaryngologis

Emphysema lung tissue gene expression profiling.

Emphysema occurs in a subgroup of patients with chronic obstructive pulmonary disease and patients with the genetic defect of alpha(1)-antitrypsin deficiency who have a smoking history of many years' duration. Emphysema is generally the result of a chronic and progressive destruction of the alveolar structures, which is believed to be driven by chronic inflammation, infections, oxidative stress, and an imbalance of protease and antiprotease activity. Here, we use microarray technology to characterize the gene expression profile of lung tissue samples obtained from patients with advanced emphys

Subcutaneous emphysema after restorative dental treatment.

Subcutaneous emphysema is an uncommon phenomenon in dentistry, usually occurring with the use of air-driven, high-speed handpieces during dental and oral surgery, operative, endodontic, or periodontal treatment. Air is forced into a surgical wound or subepithelial laceration in the oral cavity, dissecting through the different layers of tissue fasciae, and usually creating a unilateral enlargement of the facial and/or submandibular regions. This occurs with or without crepitus, pain, and airway obstruction. Treatment usually consists of antibiotic and mild analgesic therapy, close observation,

[Congenital unilateral pulmonary emphysema (Macleod's syndrome)].

The author analyzes the experience with diagnosis and surgical treatment of 5 patients with one-side emphysema of the lung (McLeod syndrome). The leading symptoms of the disease were respiratory and cardiovascular disorders, their degree being dependent on the stage of compensation of the emphysema, the degree of displacement of the mediastinum organs, disturbance of bronchial passability, compression of the contralateral lung and concomitant diseases. The clinical picture and findings obtained by complex X-ray and angiocontrast methods of examination are considered to be the leading factors i

Surgical management of congenital lobar emphysema.

Here in nine patients with congenital lobar emphysema who had been treated surgically in the previous 10 years are reported. The ages of the patients at diagnosis ranged from 26 days to 11 months. The six patients whose symptoms started in the neonatal period had more severe dyspnea, cyanosis and respiratory distress. Tube thoracostomy was performed in two of three patients who had been misdiagnosed initially. The affected side was the left upper lobe in five patients, the right upper lobe in three, and the right middle and upper lobes in one patient. Lobectomy was performed in all cases. Dysp

[Subcutaneous emphysema in gastrointestinal tract perforation].

Perforation of the gastrointestinal tract can lead to retroperitoneal abscess formation. Clinical signs and symptoms are often non-characteristic. In undetected cases the first sign of perforation may be subcutaneous emphysema. The place where this first appears correlates with the localization of the perforation. The pathogenetic mechanisms and clinical features are discussed, with reference to a case report.
