Prevention of bacterial endocarditis. Recommendations by the American Heart Association.

OBJECTIVE: To update recommendations issued by the American Heart Association last published in 1990 for the prevention of bacterial endocarditis in individuals at risk for this disease. PARTICIPANTS: An ad hoc writing group appointed by the American Heart Association for their expertise in endocarditis and treatment with liaison members representing the American Dental Association, the Infectious Diseases Society of America, the American Academy of Pediatrics, and the American Society for Gastrointestinal Endoscopy. EVIDENCE: The recommendations in this article reflect analyses of relevant

[A case of ruptured aneurysm of the sinus of Valsalva with bacterial endocarditis].

A 21-year-old male with ruptured aneurysm of sinus of Valsalva caused by bacterial endocarditis is presented. The patient complained of palpitation and a attack of fever. Continuous murmur was heard with a thrill at the third intercostal space of the left sternal border. On plain chest x-ray film pulmonary congestion and increase in pulmonary flow were observed. Cardiac catheterization showed step up of oxygen contents at the level of right ventricle and left to right shunt of 36.5%. Aortography demonstrated the aneurysm of the sinus of Valsalva ruptured to right ventricle. The operation was p

Surgical techniques for treatment of bacterial endocarditis of the mitral valve.

Bacterial endocarditis of the mitral valve appears to be much less common than bacterial endocarditis of the aortic valve. One of the main etiologic factors is the presence of degenerative lesions of the mitral apparatus, ballooning or mitral floppy valve. The surgical anatomy of the lesions is described: vegetations, perforations, rupture of chordae tendinae, abscess of the mitral ring observed in the isolated mitral endocarditis, mitral-aortic dislocation, abscesses and aneurysms of the mitral-aortic fibrosa and jet lesions on the anterior mitral leaflet. In the isolated primitive mitral inf

Chemoprophylaxis for bacterial endocarditis--a survey of current practice in London.

1820 medical and dental practitioners were circulated with a questionnaire regarding their use of antibiotics to prevent bacterial endocarditis. The response rate was 48.2%. Analysis of results suggests that antibiotics are believed to prevent endocarditis and are in common use for this purpose. However, the regimes used differ markedly from published current recommendations. Different antibiotics that are often inactive against the likely infecting organisms are used or, where recommended antibiotics are used, the doses are often inadequate. The timing of administration of prophylaxis also us

[Value of echocardiography in the preoperative diagnosis of acute bacterial endocarditis].

Open-heart surgery was performed in 26 of 56 patients with acute bacterial endocarditis seen in three years. Non-controllable infection, cardiac failure or embolism were the indications for operation. In all instances pre-operative invasive angiographic diagnosis was dispensed with, indications being based entirely upon clinical findings plus the results of M-mode or cross-sectional echocardiography. In 22 of the 26 patients the pre-operative echocardiographic diagnosis coincided with the intra-operative one. In the other four patients the pre-operative echocardiographic findings were incomple

Bacterial endocarditis due to Listeria monocytogenes in a pregnant diabetic.

A case of bacterial endocarditis due to Listeria monocytogenes in a pregnant, Class D diabetic patient is presented. The importance of obtaining proper cultures and instituting appropriate antibiotic therapy promptly is emphasized. A favorable outcome was achieved in spite of the combined risk to the fetus of maternal diabetes and listeria endocarditis.

Echocardiographic diagnosis of bacterial endocarditis of the mitral valve in a neonate.

A full-term normal neonate had acute staphylococcal endocarditis. Serial echocardiograms demonstrated abnormal echos in the region of the posterior mitral valve leaflet and aortic outflow tract that were subsequently proved to be a bacterial vegetation. This represents, we believe, the first case reported of the echocardiographic diagnosis of bacterial endocarditis in a neonate. There was progressive functional impairment of the mitral valve despite appropriate antibiotic therapy. At necropsy, a 1-cm vegetation was found attached to, and eroding through, the posterior mitral valve leaflet. The

[Etiology of bacterial endocarditis in patients with congenital heart defects].

In 90 patients with various congenital heart diseases complicated by bacterial endocarditis multiple inoculations of the blood were practised. In 14 patients microbial cultures were planted postmortem with vegetation on the endocardium, valves and also from the thrombi in the cardiac chambers and from the spleen. In 42.3 per cent of the cases a growth of microorganisms was recorded; a relation between the frequency of the microbial growth and the severity of endocarditis could be then noted. Among the planted microorganisms it was Staphylococcus albus that prevailed and this prompted the autho

[Prognosis and treatment of bacterial endocarditis. Review of 70 cases].

The long term results of medical and surgical treatments have been reviewed in 70 cases examined during the last 8 years. In 24 of them, bacterial endocarditis developed after valvular replacement. Antibiotic treatment was administered during at least 6 weeks. Determinations of the minimum inhibitory concentration of the antibiotic and of the bactericidal power of the serum were considered as the most important laboratory controls. In the post-operative cases with prosthetic valves, numerous pathogenic organisms were found, including gram (minus) bacilli and monilia, which may partly account f

Echography and phonography of acute aortic regurgitation in bacterial endocarditis.

Since management of acute aortic regurgitation in bacterial endocarditis is enhanced by early recognition, echocardiography and phonocardiography were evaluated in three such cases documented by catheterization and surgery without positive blood cultures and compared to echophonograms of 34 patients with aortic regurgitation of other origins. Endocarditis manifested distinctive, thickened, irregular aortic leaflet echoes with normal systolic excursion and mitral echopreclosure with anterior leaflet fluttering. Mitral preclosure resulted in mid- or end-diastolic crescendo murmur accompanied by

Prepapillary metastatic abscess in a case of subacute bacterial endocarditis.

The case of a 49-year-old woman is described in which the presenting symptom of subacute bacterial endocarditis was the rare condition of a prepapillary abscess. There was a rapid decrease in visual acuity in the patient's left eye 10 days after the onset of an influenza-like episode, and ophthalmological examination revealed a pus-containing formation covering the optic disc, star-figure exudates in the macula, and some retinal hemorrhages with white centers. These findings in a patient known to have old valvular stenosis indicated a diagnosis of subacute bacterial endocarditis which was conf

Saccular aneurysm caused by bacterial endocarditis in a syphilitic aorta.

Robinson, W.M., McMillan, I.K.R., and Johnson, A. M. (1973). Thorax, 529-532. Saccular aneurysm caused by bacterial endocarditis in a syphilitic aorta. The rapid development of a saccular aneurysm during successfully treated bacterial endocarditis, in an ascending aorta previously affected by syphilis which had been treated with adequate chemotherapy 15 years previously, is described. Although bacterial endocarditis does occasionally complicate syphilitic aortic valve disease, the literature does not contain any report of resulting aneurysm development. The investigation and successful surgica

External coronary artery compression due to prosthetic valve bacterial endocarditis.

Acute coronary syndromes in the setting of infective endocarditis may be the result of coronary compression secondary to periannular aortic valve complications, coronary embolism, obstruction of the coronary ostium due to a large vegetation, coronary atherosclerosis, and severe aortic insufficiency. External coronary artery compression as a result of infective endocarditis is a rare and lethal finding with few reported cases available in the medical literature. We present a rare occurrence of an acute coronary syndrome occurring in the setting of a bioprosthetic aortic valve abscess in which t
