Prolonged Antibiotics Versus Prolonged Anticoagulation: A Case Report of Libman-Sacks Endocarditis.

Marantic endocarditis, also known as nonbacterial thrombotic endocarditis (NBTE), Libman-Sacks endocarditis, or verrucous endocarditis, is a rare, non-infectious endocarditis (IE) that primarily affects the aortic and mitral valves. It is often underreported due to its subtle nonspecific presentation and close echocardiographic resemblance to infective endocarditis (IE). Substantial NBTE differentials include cardiac tumors, IE, and prior residual lesions. Echocardiography, clinical evaluation, and other alternative imaging modalities, such as cardiac CT or PET/CT, are essential for comprehensiv

Comparative effectiveness of empirical antibiotic treatments in methicillin-susceptible Staphylococcus aureus infective endocarditis: A post hoc analysis of a prospective French cohort study.

OBJECTIVES: The empirical treatment of infective endocarditis is still debated. The aim of this study was to compare the impact of empirical treatment with antistaphylococcal penicillin (ASP) or cefazolin vs. other treatments in methicillin-susceptible Staphylococcus aureus (MSSA) endocarditis. METHODS: A post hoc analysis of a prospective cohort study of patients hospitalized in a French reference centre with MSSA endocarditis was conducted between 2013 and 2022. The primary outcome was the duration of bacteraemia under treatment. RESULTS: Of the 208 patients included, 101 patients (48.6%)

Infectious endocarditis caused by Elizabethkingia spp.: Two case reports and a literature review.

We report two cases of Elizabethkingia-related infective endocarditis, highlighting key risk factors such as prosthetic valve replacement. These cases underscore the need to consider endocarditis in the setting of persistent or recurrent bacteremia. Diagnosis proved challenging and required multiple imaging modalities, with positron emission tomography (PET) scan providing critical value when echocardiography was non-contributory. Management was complicated by the organism's intrinsic multidrug resistance and the rapid emergence of additional resistance during therapy. These cases emphasize th

Streptococcus bovis bacteraemia: an evaluation of the long-term effect on cardiac outcomes.

INTRODUCTION: Streptococcus bovis can lead to bacteraemia, septicaemia, and ultimately endocarditis. The objective of this study was to evaluate the long-term implications of S. bovis endocarditis on cardiac morbidity and mortality. METHODS: A retrospective cohort study was performed between January 2000 and March 2009 to assess all patients diagnosed with S. bovis bacteraemia from the Belfast Health and Social Care Trust. The primary end-point for cardiac investigations was the presence of endocarditis. Secondary end-points included referral for cardiac surgery and overall mortality. RESULT

A unique case report of mitral valve endocarditis associated with coronary stent infection.

BACKGROUND: Despite increasing use of percutaneous coronary intervention and stenting, septic complications such as coronary stent infections are rare. We report a unique case of mitral valve infective endocarditis and associated coronary stent infection which emerged 6 months after index stent insertion. CASE SUMMARY: A 56-year-old previously healthy man underwent percutaneous coronary intervention and stenting of left circumflex (LCx) coronary artery in the setting of non-ST-segment elevation myocardial infarction. Six months later, he represented with inferior ST-segment elevation myocardi

Abiotrophia defectiva Endocarditis: A Rare Cause with Aggressive Systemic Embolisation and Need of Valve Replacement.

UNLABELLED: Infective endocarditis (IE) is a well-described infectious disease, one with increased morbidity and mortality being the third or fourth most common life-threatening infection syndrome. Abiotrophia defectiva is a non-motile, catalase negative, gram-positive coccus in a chain, which can be isolated from the oral cavity, intestinal, and genitourinary tracts. IE due to this agent is rare and associated with heart valve destruction, congestive heart failure, and high embolisation rates, these being the major mortality causes. We present a case of IE due to this agent, complicated with

[A case of Cefepime encephalopathy, being difficult to distinguish from non-convulsive status epilepticus during the treatment of bacterial meningitis].

A 64-year-old man with fever, appetite loss, and pain in the back of the neck visited our hospital. We diagnosed him as having bacterial meningitis because of pleocytosis of the cerebrospinal fluid, and started treatment with antibiotics. Multiple cerebral infarcts were found on brain MRI. We suspected that the origin of the bacterial meningitis was infective endocarditis, and administered Cefepime and Gentamicin according to the guidelines for treatment of infective endocarditis. Three days later, he became drowsy and had myoclonus and flapping of the extremities. An electroencephalograph sho

Insidious clinical presentation of uncommon infective endocarditis caused by Cardiobacterium species.

Infective endocarditis (IE) is a fatal disease caused by a wide range of pathogens, with a strong prevalence of Gram-positive bacteria. In this paper we describe two peculiar cases of Cardiobacterium spp. endocarditis in young men. These case reports emphasize the insidious clinical presentation of HACEK IE and lead to a brief review of the literature and discussion on this uncommon cause of infective endocarditis. One case is a man with a prior diagnosis of myxomatous mitral valve, presenting with neurological symptoms and a mycotic aneurysm of the right anterior cerebral artery. The second c

Infective Endocarditis Due to Corynebacterium Species: Clinical Features and Antibiotic Resistance.

BACKGROUND: Corynebacterium species are often dismissed as contaminants in blood cultures, but they can also cause infective endocarditis (IE), which is a severe condition. Antibiotic resistance of corynebacteria is increasing making treatment challenging. Reports on IE caused by Corynebacterium species are scarce and more knowledge is needed. METHODS: Cases of IE caused by Corynebacterium species were identified through the Swedish Registry of Infective Endocarditis. Isolates were collected for species redetermination by matrix-assisted laser desorption ionization-time of flight and for anti

Fulminant infective endocarditis with toxin-negative Corynebacterium diphtheriae in people with substance use experiencing homelessness, England, 2024 to 2025.

Between July 2024 and January 2025, five male patients in their early 20s to early 50s were confirmed with infective endocarditis associated with non-toxigenic Corynebacterium diphtheriae in England. Three were known to have experienced homelessness. All five used non-intravenous recreational drugs. Disease progression was rapid, four patients required surgical intervention, one died. Whole-genome sequencing and multilocus sequence type (MLST) analysis identified four individuals as ST559. Clinicians and substance use services have been alerted and enhanced surveillance implemented. A prevalen

[Infectious endocarditis in a drug addict].

PURPOSE: To compare two groups of patients with infective endocarditis, the drug addicts and non-drug addicts. We attempted to set particularities among the various aspects that involve the patient with endocarditis, due to the concurrent chronic use of cocaine intravenously. METHODS: Twenty nine patients, group B, whose clinical diagnose was compatible with infective endocarditis, with risk factor of parenteral toxicomania by cocaine were treated at Institute "Dante Pazzanese de Cardiologia" and Hospital "Emilio Ribas" in São Paulo, from 1984 to 1990. The data obtained for etiological agents

Cardiac Device-Related Infective Endocarditis Presenting With Suspected Embolic Stroke in a Patient With a Legacy Implantable Cardioverter-Defibrillator: A Case Report.

Cardiac device-related infective endocarditis can evade early recognition when embolic neurologic symptoms obscure the underlying cardiac source. We report the case of an 87-year-old man with a cardiac resynchronization therapy defibrillator (CRT-D) that was not labeled as MRI-conditional. He presented after an unwitnessed fall with acute dysarthria, facial asymmetry, and asymmetric extremity weakness, with a National Institutes of Health Stroke Scale score of 10 during the acute stroke evaluation. Initial cardiovascular examination revealed no murmur, and noncontrast CT of the head showed no

Reintervention after aortic root replacement with allograft, xenograft, and stented bioprosthetic valves.

BACKGROUND: There has been a significant increase in the utilization of non-mechanical valves in the aortic position over time. However, details in reinterventions after aortic root replacement (ARR) with non-mechanical prosthesis were limited in the literature, despite the potential importance of reinterventions in the lifetime management of aortic valve disease. METHODS: This is a single-center retrospective study, identifying all patients who underwent ARR with allograft, xenografts, and stented bioprosthetic valved conduit from 2010 to 2020. Cumulative incidence was plotted for aortic val

Nonbacterial thrombotic endocarditis as the initial presentation of prostate cancer- a case report.

Nonbacterial thrombotic endocarditis (NBTE) refers to noninfectious vegetations of the heart valves. It is commonly associated with malignancy and autoimmune diseases like systemic lupus erythematosus, Rheumatoid arthritis. Herein, we present Non-bacterial thrombotic endocarditis and Disseminated intravascular coagulation as the initial manifestations of prostate cancer. A 50-yearold gentleman, known case of hypertension and diabetes, presented with a history of recurrent ischaemic strokes, STEMI and a recent diagnosis of infective endocarditis. He had been taking antibiotics for the past 20 d

Adherence characteristics of coagulase-negative staphylococci isolated from patients with infective endocarditis.

Coagulase-negative staphylococci isolated from patients with endocarditis were divided according to whether the infection was of native or of prosthetic valves and was acquired either in the community or in hospital. Comparisons were made with strains from intravenous line-associated bacteraemias. All strains were examined by direct and indirect adherence tests. Line-associated bacteraemia strains were more likely to produce slime and were more hydrophilic but were less likely to attach HEp2 tissue culture cells than were endocarditis strains, and almost equally likely to adhere to plastic and

[Ten questions about infective endocarditis].

Infective endocarditis (IE) is an infective process involving the endocardium and, more frequently, the native heart valves, valvular prostheses and cardiac implantable electronic devices. IE can manifest with various non-specific symptoms making the diagnosis challenging. This condition is associated with high in-hospital and long-term mortality. Therefore, it is particularly important to prevent it by implementing an adequate antibiotic prophylaxis especially in patients at high risk undergoing invasive procedures. Moreover, it is pivotal to promptly diagnose IE, detect the presence of local
