BACKGROUND: Contralateral delayed endolymphatic hydrops (CDEH) is a clinical entity characterized by fluctuating low frequency hearing loss and/or vertigo, mimicking Ménière's disease (MD), that manifests after the appearance of severe non-hydropic hearing loss (NHHL) at the other ear.
OBJECTIVES: to describe the clinical features and the course of 57 patients affected by CDEH.
METHOD: this is a retrospective study; 57 patients affected by CDEH, out of 1065 patients seen in the same period and affected by MD, were subjected to otoscopy, PTA threshold evaluation, impedance testing, ABR, resea

INTRODUCTION: Middle ear application of gentamicin is a common medical treatment for uncontrolled Ménière's disease. The objective of the study was to evaluate the impact of endolymphatic hydrops on inner ear delivery.
METHODS: Perilymph gentamicin concentrations and correlation with endolymphatic hydrops in an animal model were assessed. A group of 24 guinea pigs was submitted to surgical obstruction of the endolymphatic sac and duct of the right ear. Gentamicin was applied either to the right ear's round window niche or through a transtympanic injection. Perilymph specimens were collected a

The present investigation was specifically designed to evaluate the ability of the glycerol test combined with pure-tone audiometry, distortion-product otoacoustic emissions (DPOAEs) to diagnose endolymphatic hydrops early and to identify patients who may evolve toward Meniere's disease. This investigation consisted of 43 consecutive patients with daily episodes of tinnitus with duration exceeding few hours or with continuous tinnitus who received no treatment. These were the unique symptoms reported by the patients. Each patient underwent glycerol testing measured with conventional pure tone

The classical guinea pig model for Meniere's disease, in which endolymphatic hydrops was achieved by destruction of the endolymphatic sac and obliteration of the endolymphatic duct, is a non-physiological profound model with shortcomings in relation to Meniere's disease as seen in patients. We developed a more subtle animal model; the two-phase endolymphatic hydrops. This model is based on a combination of chronic endolymphatic sac dysfunction, induced by slight destruction of the most distal part of the endolymphatic sac, and acute stress-induced endolymph production by stimulation of the Na/

The tympanic membrane was perforated (n = 19) or a tube was inserted into the middle ear through the wall of the tympanic bulla (n = 6) immediately after blockage of the endolymphatic duct in guinea pigs. Total cochlear endolymph volume and volumes in each cochlear turn were determined from serial sections of the temporal bones and volume changes were analyzed statistically by comparison with hydropic controls without treatment (n = 12). Results showed that both middle ear ventilation procedures significantly reduced the subsequent development of endolymphatic hydrops. This inhibition of hydro

BACKGROUND: A new method of diagnosis of endolymphatic hydrops by recording low-tone masked evoked otoacoustic emissions (TEOAE) is presented.
METHODS: A short acoustic stimulus and a masker tone of 30 Hz are applied in an adjustable phase relation simultaneously to the same ear. In the normal hearing ear the masker shows little influence on the TEOAE at 0 degrees, whereas the suppression at phase 270 degrees (maximal rarefaction at the eardrum) is nearly complete. However, in cases of endolymphatic hydrops this masking effect is reduced or absent, indicating impaired mobility of the basilar

The Mondini deformity of the inner ear is usually associated with a large vestibular aqueduct and endolymphatic sac. The authors present a case with a hypoplastic sac and endolymphatic hydrops, which are presumed to be the cause of the Meniere's syndrome symptoms that occurred in mid-life.

This study attempted to develop a test which would be specific for endolymphatic hydrops. It is based upon the hypothesis that increased pressure in the scala media will alter the stiffness of the basilar membrane and hence increase the speed of the travelling wave. Auditory brainstem-derived responses were used to measure the basilar membrane travelling wave velocity in normals and to take the equivalent measures in Meniere's patients. Ten normally hearing subjects and 20 Meniere's patients were tested. The data for the Meniere's group showed that the travelling wave velocities were within no

Localizing the ear responsible for vertigo attacks may be exceedingly difficult in patients with delayed endolymphatic hydrops, vestibular Ménière's disease or bilateral Ménière's disease. This has important clinical implications when planning operative treatment. We present a case of delayed endolymphatic hydrops to exemplify these difficulties and demonstrate the localizing value of recovery nystagmus. We stress the importance when possible of monitoring patients with endolymphatic hydrops for recovery nystagmus during acute vertigo attacks. Monitoring should include direct eye observation a

A temporal bone specimen demonstrating endolymphatic hydrops 13 years after fenestration of the lateral semicircular canal is presented. Fibro-osseous tissue extending from the lateral semicircular canal and reaching the vestibule produced fixation of the membranous wall of the utricle to the bony wall. Fixation and retraction of the utricule appears to have resulted in a permanently open utriculo-endolymphatic valve leaflet. Similar findings of fibro-osseous changes arising from the area of the crus commune and an open valve were found in a Meniere's disease specimen. The implications of thes

BACKGROUND: Hydropic Ear Disease (Menière) is one of the most common inner ear disorders and one of the most common causes of vertigo attacks. The underlying pathology is a distension of the endolymphatic space of the inner ear, termed endolymphatic hydrops. However, the unequivocal morphologic confirmation of ELH has been restricted to post-mortem histologic analysis until 2007, when the first clinical MR imaging report demonstrated ELH in living patients with Menière's disease at 3 T combined with intratympanic application of contrast. Imaging techniques have since then evolved further. Howe

BACKGROUND AND PURPOSE: Gadopiclenol is a next-generation macrocyclic gadolinium-based contrast agent (GBCA) distinguished by its high T1 relaxivity and kinetic stability. It was developed to address the clinical need for reduced gadolinium dosing while maintaining high diagnostic accuracy, thereby minimizing potential long-term risks associated with gadolinium retention. Although various neuroradiology applications have been explored, the potential benefits of gadopiclenol's increased T1 relaxivity have not been investigated for the purpose of evaluating endolymphatic hydrops (EH) using delay

OBJECTIVE: In this study, we aimed to summarize magnetic resonance imaging (MRI) findings of perilymphatic enhancement (PE) and endolymphatic hydrops (EH) of the inner ear, which are associated with vestibular and cochlear symptoms.
METHODS: We analyzed data on ears with definite Meniere's disease (MD), sensorineural hearing loss (SNHL), vertigo, and listening difficulties (LiD) from 508 ears of 254 patients who underwent contrast-enhanced 3-Tesla MRI between April 2021 and March 2023. We evaluated the degree of endolymphatic hydrops (EH), signal intensity ratios (SIRs) between the basal turn

Objective: We used volumetric three-dimensional (3D) analysis to quantitatively evaluate the extent of endolymphatic hydrops (EH) in the entire inner ear. We tested for correlations between the planimetric and volumetric measurements, to identify their advantages and disadvantages. Methods: HYDROPS2-Mi2 EH images were acquired for 32 ears (16 patients): 16 ipsilateral ears of MD patients (MD-ears) and 16 contralateral ears. Three-T MR unit with a 32-channel phased-array coil/the contrast agent to fill the perilymphatic space and the HYDROPS2-Mi2 sequence. We calculated the EH% [(endolymph)/(en

HYPOTHESIS: Triple semicircular canal occlusion will eliminate rotatory stimulation to the vestibular peripheral system (as it blocks endolymphatic fluid movement) and therefore release rotatory vertigo attack. This surgery is safe in ears with endolymphatic hydrops.
BACKGROUND: Semicircular canal occlusion has been used as an alternative treatment of intractable benign paroxysmal positional vertigo with varied success. Triple semicircular canal occlusion in animal models blocks the responses of the semicircular canals to rotation and spares cochleae and the otolithic apparatus. This result s

Guinea pigs placed outside or inside a pressure chamber and exposed to 49.2 cm H2O overpressure continuously for 24 h immediately after blockage of the endolymphatic duct showed no significant difference in the magnitude of endolymphatic hydrops when compared to controls, although there was a slight indication of a volume decrease in the outside-treatment group and an increase in the inside-treatment group. A pressure increase of 49.2 cm H2O in the external auditory canal for 1 h twice a day for 2 weeks outside the chamber significantly inhibited the development of hydrops. The latter result s

The use of electrocochleography (ECochG) in the identification of endolymphatic hydrops in Menière's disease, using either trans-tympanic or extra-tympanic recording techniques, has become very popular. The presence of an enhanced summating potential (SP) component is considered to be a diagnostic indicator of hydrops. The response evoked by a click stimulus is widely used in this investigation. The aim of this study was to assess the value of tone-pip stimuli for measurement of the SP component in patients with suspected Menière's disease. Extra-tympanic ECochG was recorded from a surface ele
