PURPOSE: The precise location of the endolymphatic sac (ES) may be difficult during surgical approaches. This morphometric study aimed to determine the exact location of ES in adult human cadavers for the management of pathologies such as Meniere's disease.
METHODS: Twenty temporal bones of 10 adult cadavers (mean age: 70 ± 13.40 years, range: 45-92 years; sex: 4 males and 6 females) fixed with 10% formalin were bilaterally dissected to obtain numeric data about the location of ES.
RESULTS: Distances of ES to the posterior semicircular canal (PSC), Donaldson line (DL), sigmoid sinus (SS) and

BACKGROUND: Ménière's disease (MD) is a chronic inner ear disorder with a multifactorial etiology. Decreased visualization of the endolymphatic duct (ED) and sac (ES) is thought to be associated with MD, although controversy exists about whether this finding is specific to MD. Recent literature has revealed that two distinct ES pathologies, developmental hypoplasia and epithelial degeneration, can be distinguished in MD using the angular trajectory of the vestibular aqueduct (ATVA) or ED-ES system as a radiographic surrogate marker. It has been suggested that these two subtypes are associated

BACKGROUND: Ménière's disease is an inner ear disorder causing recurrent vertigo, hearing loss, and tinnitus. Diagnosis is based on the variability of the symptoms over time and absence of radiological abnormalities. Medical therapy is effective only in a small percentage of patients. Surgical strategies remain controversial. In this article, we revisit a surgical technique neglected over the years: endolymphatic sac surgery.
METHODS: Fifty-four patients affected by Ménière's disease underwent endolymphatic duct and sac decompression. According to the American Academy of Otolaryngology-Head a

The immunohistochemical expression pattern and the physiological role of transient receptor potential vanilloid (TRPV) 4 in the endolymphatic sac were investigated. TRPV4 was expressed predominantly in the apical membrane of mitochondria-rich cells, and cell volume regulation by TRPV4 was observed in a tissue culture of the rat endolymphatic sac. TRPV4 was also present in the endolymphatic sacs of patients with vestibular schwannoma and with Ménière's disease. TRPV4 is assumed to play a role as an osmoreceptor in cell and fluid volume regulation in the human endolymphatic sac.

Patients with an endolymphatic sac tumor (ELST) typically present with palsy of cranial nerves VII and/or VIII; other presenting symptoms include hearing loss, otalgia, occipital headaches, cranial nerve palsies, vertigo, gait ataxia, tinnitus, and otorrhea. ELSTs are extremely vascular, and they can invade and destroy temporal bone. Because of these characteristics, they are often mistaken for glomus tumors of the skull base. We describe the clinical presentation, evaluation, and management of ELSTs based on our review of the limited literature and our experience with 3 adults who presented t

In this retrospective case series and literature review, we demonstrate that temporal bone lesions that obstruct the endolymphatic sac or duct can cause symptoms of Ménière's disease. This finding is likely attributable to endolymphatic hydrops; initially, such cases typically masquerade as Ménière's disease. Between July 1995 and April 2002, a total of 379 patients were treated for an initial diagnosis of Ménière's disease at our institution. Among this group, 3 patients were found to have an obstructing lesion of the endolymphatic sac or duct that we felt was causally related to their Ménièr

SEM and immunoglobulin staining of the intradural portion of the endolymphatic sac (ES) were investigated. All 15 normal subjects, from fetuses to an 80-year-old adult, showed well arranged epithelial cells. In Menière's disease (all 15 patients) the ES showed various types degeneration of the epithelial cells. Inner ear deafness experimentally produced by kanamycin injection showed a similar degeneration of the ES. IgG of the ES in Menière's disease showed moderately evident deposits compared with normal subjects. However, these findings were also found to the same degree as IgG deposits in a

Hyaluronan (HA) 0.5, 1, 1.9 and 4% was instilled into the round window niche in mice. Six hours after the 1.9% HA and 4, 6 and 8 h after the 4% HA deposition there was a significant increase in the proportion of granule-containing light epithelial cells in the endolymphatic sac. This indicated an increase in secretory activity. This activity could be a response to lowered intralabyrinthine pressure due to the osmotic capacity of HA. Secretion of high molecular substances can serve to keep the sac lumen patent and/or to regulate labyrinthine volume and pressure.

The morphometric examinations of the endolymphatic saccule epithelium in Ménière's disease were performed. The squamous epithelium prevailed in lower and cuboid and cylindrical in upper part of the saccule. In Ménière's disease the cellular nuclei are more elongated. The mean height of the epithelial cells are lower of 2,236 microns than the cells in normal hearing person. The morphometric method of examination permits to follow the supposition that in Ménière's disease there are the degeneration of the epithelial cells and subsequently the disturbances in endolymphatic resorption.

Two types of abnormality in the endolymphatic sac acquired at shunt surgery are presented. One was deep brown pigmentation in the subepithelial connective tissue. The pigment did not react to Prussian blue, thus ruling out hemosiderin. The other, in seven out of eight specimens, was the perisaccular fibrosis. The fibrotic foci in four cases were assumed, from the findings under ordinary illumination and polarized light microscopy, to be reorganized scar tissue.

OBJECTIVE: Lymphoid neogenesis seems to play an important role in the persistence of chronic inflammation and has been shown in various disorders characterized by chronic inflammation including rheumatoid arthritis. Arthritis of Behçet's disease is characterized by non-erosive arthritis in which the disease course is considered to be subacute and self limiting. However, molecular mechanisms underlying those features of Behçet's arthritis have not been defined yet. In order to determine the contribution of lymphoid neogenesis in the disease course of Behçet's arthritis, we investigated the syno

We present a giant tumor of the skull base compressing the brain in a 40-years-old man. The tumor was policystic at imaging. Its histopathology, immunohistochemical profile and long evolution suggest an endolymphatic sac tumor (ELST), a rare case of neoplasia. Since the patient had multiple otolaryngological procedures in his medical history, a possible traumatic pathogenesis could be suspected. On the other way, some immunohistochemical aspects found in our case may imply a histogenesis divergent from that currently accepted. This could be from either the organ of Corti or some local cells th

Large vestibular aqueduct syndrome (LVAS) is a congenital disorder characterized by progressive or fluctuating sensorineural hearing loss of unknown etiology. Serial MR examinations were performed before and after the development of hearing loss in two patients with LVAS. The signal and volume of the enlarged endolymphatic sac (EES) vary even in ears with stable hearing. In ears with fluctuating hearing, changes in EES signals were observed in only one of two patients. The finding that the EES volume and signal intensity vary dynamically independently of hearing is important for future researc

The foregoing discussion has attempted to substantiate and clarify the six propositions outlined at the outset in light of the author's personal experience. In sum, the following points are emphasized: Employment of ESS for treatment of intractable Meniere's disease in cases where the sac is definitively delineated, the real sac lumen entered, the sac's integrity preserved, and sac enlargement technique is employed has an approximate 90% chance of achieving complete or substantial control of vertigo in the short run (2-3 years). Although there seems to be little likelihood of pushing the short

The purpose of the present study was to evaluate the long-term auditory results in the ears of patients suffering from unilateral Ménière's disease (Md) who had undergone retrolabyrinthine vestibular neurotomy (RVN) associated with endolymphatic sac (ES) surgery. Retrospective evaluation was performed on 45 patients with unilateral Md who had undergone RVN between 1982-1997. All patients had previously been treated with medical therapy for at least 6 months without showing any response. The forms of ES surgery performed were as follows: simple ES decompression (ESD) in 15 patients, chronic end

BACKGROUND: Endolymphatic sac tumors (ELSTs) are rare, locally aggressive tumors often associated with von Hippel-Lindau disease. Due to their location and hypervascularity, complete surgical excision is challenging, and recurrence is common. Gamma knife radiosurgery (GKRS) has emerged as a potential noninvasive treatment modality, particularly in recurrent or inoperable cases.
METHODS: A retrospective analysis of five patients with radiologically diagnosed ELSTs treated with GKRS at our institution between 2012 and 2023 was performed. Patients who had received either upfront or adjuvant GKRS
