[Jejuno-ileal diverticula (Meckel excluded). Apropos of 10 cases].

The authors report ten cases of small bowel diverticulosis revealed by a complication in four cases. Aetiologic, pathological, clinical and therapeutic characteristics of this rare disease are reviewed. Main complications are perforations, intestinal obstruction and hemorrhage. The treatment was the resection of the segment of small intestine involved by complicated diverticula. Resection was followed by anastomosis or temporary enterostomy if there was an acute peritonitis. Uncomplicated diverticulae must be respected and left in place.

[Compression anastomoses in the surgery of transverse colon cancer].

The design and operation of the newly-developed AKA-2 intestine suture appliance for forming circular compression enterostomy are discussed. The advantages offered by application of the device over those of a clip appliance are considered. The results of its application in 86 operations on the transverse colon are presented. The procedure failed in 46% of cases, postoperative lethality rate was 2.3%. The said procedure should be recommended for a wider integration with clinical practice.

[Early adhesive intestinal obstruction].

In the paper, on the basis of 54 observations the incidence of early adhesive intestinal obstruction, its diagnosis and operative treatment are discussed. In case of jejunal overfilling a modified I. D. Zhitnjuk enterostomy is recommended, the lessens the danger of the operative wound contamination, accelerates an insertion of a drainage tube in the intestinal lumen and provides an opportunity to accomplish continuous suction both during the process of intestinal drainage and in the postoperative period.

Role of the enterostomal therapy nurse in ostomy patient rehabilitation.

Enterostomal therapy (ET) nurses specialize in the management of patients with urinary and fecal diversions, draining wounds and fistulas, fecal and urinary incontinence, and chronic wounds such as pressure ulcers and vascular ulcers. ET nurses have much to offer in the management of patients with cancer. Such nurses play a major role in the rehabilitation of patients undergoing fecal or urinary diversions. Preoperative services include: counseling regarding planned surgical procedure, the impact of an ostomy on the patient's life, and the basics of ostomy management; sexual counseling; and st

Management of biliary atresia in Nigeria: the ongoing challenge.

BACKGROUND: In developed countries, the outlook for patients with biliary atresia has improved remarkably owing to early referral, good diagnostic facilities and the availability of liver transplantation. In Nigeria and most of sub-Saharan Africa, there is little information on the current outlook for these children. METHODS: A retrospective review of 14 patients with biliary atresia managed from 1991 to 2004 at Ahmadu Bello University Hospital, Zaria. RESULTS: Six girls and eight boys presented at between 6 and 24 weeks of age (median 16). Seven presented to another hospital with persistent

Surgical strategy for the management of biliary injury in laparoscopic cholecystectomy.

BACKGROUND/AIMS: The incidence of biliary injury during laparoscopic cholecystectomy remains high and several complications resulting from injuries have recently been reported. The aim of this study is to elucidate the surgical strategy for the management of biliary injury during laparoscopic cholecystectomy. METHODOLOGY: Ten patients with biliary injury during laparoscopic cholecystectomy are retrospectively reviewed. RESULTS: Second operations as initial repair were performed in five patients in our institute. Duct-to-duct anastomosis for one and duct-enterostomies for two were performed i

[Rare anorectal malformations. Intermediate-type anal agenesis with a rectocutaneous fistula].

A case of anal agenesia with recto-cutaneous fistula is presented. The recto-cutaneous fistula in an intermediate or high anomaly seldom noticed so that it is not included in the common classifications. The surgical approach performed by us was that described by Mollard. The anterior perineal pull-through, under proximal protective enterostomy, allow to reduce the rate of postoperative complications and sequences and obtain--as in our patient--a normal continence and sphincteric function with an excellent esthetic outcome.

Septic Shock Due to Chyme Recycling in an Extremely Low-Birth-Weight Infant.

Chyme recycling (CR) is an effective therapeutic approach that involves reintroducing bowel contents from the proximal to the distal enterostomy limb and is recognized for its ability to reduce or prevent enterostomy-related complications. Although the benefits of CR are well documented, reports on its potential adverse events remain limited. We report the case of an extremely low-birth-weight infant (ELBWI) who developed septic shock caused by Klebsiella pneumoniae immediately after the first CR. The male infant, born at 22 weeks' gestation via emergency cesarean section, had a birth weight o

An 18-year experience in total colonic aganglionosis: from staged operations to primary laparoscopic endorectal pull-through.

BACKGROUND: Traditionally, total colonic aganglionosis (TCA) was managed with enterostomy +/- pull-through. Since 1998, primary laparoscopic endorectal pull-through (PLEP) was offered to patients with TCA in our unit. A retrospective study was conducted and early results reviewed. METHOD: Patient data were collected retrospectively. Before 1998, enterostomy followed by open Duhamel pull-through was performed. From 1998, patients were assessed to receive staged operation or PLEP. RESULTS: From June 1990 to May 2007, 10 patients (6 males) were treated. All presented in the neonatal period. Sev

Primary anastomosis in necrotizing enterocolitis: the first option to consider.

INTRODUCTION: Necrotizing enterocolitis (NEC) is the most frequent gastrointestinal emergency in preterm newborns. Thirty percent of all cases will require surgical intervention. Following resection of the involved segment, most patients will undergo a diverting enterostomy. OBJECTIVE: To describe the safety and effectiveness of primary anastomosis in patients with complicated NEC. METHODS: This study was a retrospective chart review. The study participants were obtained from both public and private health systems between December 2004 and December 2009 in Santiago, Chile. The inclusion crit

Use of the appendix stump in the treatment of meconium ileus.

Three neonates with meconium ileus who failed to respond to non-operative measures were successfully treated by appendicectomy and irrigation with Gastrografin into the small bowel via the appendix stump. The meconium was emptied out and the stump ligated. This method avoids enterotomy, enterostomies or resection of bowel. The post-operative course is simplified and hospital stay decreased. Removal of the appendix precludes future disease of this organ which may be problematic in patients with cystic fibrosis.

Eccrine syringofibroadenoma arising in peristomal skin: a report of two cases.

Eccrine syringofibroadenoma (ESFA) is a benign neoplasm arising from the intraepidermal portion of eccrine ducts. It is characterized by a distinctive histologic pattern of epithelial cells arranged in anastomosing cords surrounded by a fibrovascular stroma. Approximately 50 cases of ESFA have been reported, and in recent years the lesion has been described occurring in association with other skin conditions. We report 2 cases of ESFA arising in abdominal skin adjacent to enterostomy sites.

Oesophagoplasty in the treatment of oesophageal stricture.

Two children with oesophageal stricture due to corrosive oesophagitis and retained foreign-body respectively, were treated with resection and primary oesophageal reconstruction. Both were complicated by anastomotic leaks which healed after adequate drainage with temporary enterostomy feeding. The long-term results have remained excellent without dysphagia or reflux. The technique of primary repair is recommended for short-length stricture of the oesophagus as an alternative to reconstruction by gastric or colonic tube interposition.

[Gastrointestinal surgery and gastroenterology. XI. Stomas and stoma surgery].

Stomas are an essential part of gastrointestinal surgery. Indications for stoma construction are faecal diversion from a distal diseased bowel segment, prevention of an intestinal anastomosis in intra-abdominal sepsis, and faecal incontinence. Pre- and postoperative counselling and nursing care is essential for a good functional outcome. Following stoma construction, complications such as dermatitis, retraction, prolapse, stenosis and parastomal hernia occur in 30-60% of cases. Thirty percent of stomas need surgical re-intervention in the first 10 years. For diversion of a distal anastomosis,

The clinical application of the Malone antegrade colonic enema.

Continent, catheterizable enterostomies (Malone stomas) have been formed in 25 patients. A reversed appendicocecostomy was used for the majority, although experience has led to modifications of this procedure. Antegrade enemas are regularly administered via these stomas in the treatment of fecal incontinence and intractable constipation. There have been only two important surgical complications, and the technique has proved to be highly successful. Follow-up of the patients has shown that individual requirements can be met with variations in the administration of the enemas.

Lateral pararectal versus transrectal stoma placement for prevention of parastomal herniation.

BACKGROUND: A parastomal hernia is defined as an incisional hernia related to a stoma, and belongs to the most common stoma-related complications. Many factors, which are considered to influence the incidence of parastomal herniation, have been investigated. However, it remains unclear whether the enterostomy should be placed through, or lateral to the rectus abdominis muscle, in order to prevent parastomal herniation and other important stoma complications. OBJECTIVES: To assess if there is a difference regarding the incidence of parastomal herniation and other stoma complications, such as i
