Giant Ependymoma Removal via a Skipped Myelotomy Technique.

Intramedullary ependymomas should be treated with surgical resection. Different surgical techniques are described for these tumors, such as skipped and long-segment myelotomies. A 31-year-old male patient with a giant thoracic spinal cord ependymoma extending from the level of T5 to T10 was operated on with a skipped myelotomy technique. Although the patient had urinary incontinence and muscle weakness in both legs, the patient's complaints were nearly completely resolved in the fourth postoperative month. Operating with the smallest possible myelotomy has given us preferable results; however,

Synchronous presentation of prolactinoma and supratentorial tanycytic ependymoma.

Tanycytic ependymomas mostly occur in the spinal cord and it is the rarest histological subtype of ependymoma. A 29-year-old male was referred from the infertility clinic after serum prolactin levels were found to be elevated. Magnetic resonance imaging (MRI) brain showed an irregular necrotic lesion in the periventricular region of the left parietal lobe which had an intraventricular component and associated perilesional edema. In addition, a sellar mass with suprasellar extension was also found on the MRI. He was started on cabergoline therapy for macroprolactinoma and underwent a left parie

Intra-fourth-ventricular choroid plexus papilloma miming ependymoma.

Choroid plexus papilloma (CPP), a low incidence central nervous system (CNS) tumor, typically develops as an intraventricular neoplasm arising from the epithelium of the choroid plexus. Infratentorial CPP is predominantly clustered in adults with roughly 70% in the fourth ventricle, while supratentorial CPP commonly found in the lateral ventricles, is the most frequent location in children. The clinical and imaging features of CPP are not typical and may induce the misdiagnosis as other types of primary brain tumors. In this paper, we described a fourth-ventricular CPP that was misdiagnosed as

TSC2 alterations in anaplastic ependymoma progression to ependymosarcoma.

Ependymosarcomas are rare, biphasic tumors composed of ependymal and sarcomatous components. Due to their rarity, their biologic basis is not well understood. We report the case of a 38-year-old male with anaplastic ependymoma (WHO grade III) that progressed to ependymosarcoma in less than 2 years after multiple resections, chemoradiotherapy, and anti-PD1 immunotherapy. Next-generation sequencing was performed on both high-grade anaplastic ependymoma and ependymosarcoma samples to detect small base changes, insertions, and deletions in exons and splice junctions from a panel of over 400 genes.

Subcutaneous sacrococcygeal myxopapillary ependymoma misdiagnosed as pilonidal disease.

Ependymomas are neoplasms which arise from the radial glial cells, which many recent studies have proposed are neural stem cells. Extracranial ependymomas are rare. We present the case report and supporting multimedia of a 37-year-old man who presented with a painless intergluteal swelling which was diagnosed clinically as a pilonidal cyst. However, on excision, he was found to have a subcutaneous sacrococcygeal myxopapillary ependymoma based on histological findings. His management and follow-up are presented and discussed. Given the rare nature of this condition, there is a lack of published

Mobile Myxopapillary Ependymoma with Associated Filum Terminale Cyst.

BACKGROUND: Intradural ependymal cysts are benign, fluid-filled cysts usually situated along the ventral surface of the spinal cord. There are previous reports of 19 intradural cysts in the literature, including one cyst of the filum terminale. Here, we report for the first time the presence of a radiographically occult filum terminale cyst associated with a myxopapillary ependymoma. We propose that mobility of the tumor may provide indirect evidence of the presence of a cyst. CASE DESCRIPTION: A 65-year-old male patient presented with a homogenously enhancing ovoid mass measuring 25 mm × 10

MRI Phenotype of RELA-fused Pediatric Supratentorial Ependymoma.

PURPOSE: Epigenetic profiling has recently identified clinically and molecularly distinct subgroups of ependymoma. The 2016 World Health Organization (WHO) classification recognized supratentorial ependymomas (ST-EPN) with REL-associated protein/p65 (RELA) fusion as a clinicopathological entity. These tumors represent 70% of pediatric ST-EPN characterized by recurrent C11orf95-RELA fusion transcripts, which lead to pathological activation of the nuclear factor 'kappa-light-chain-enhancer' of activated B-cells (NF-κB) signaling pathway. Cyclin-dependent kinase inhibitor 2A (CDKN2A) inactivation

Predictors of Functional Outcome after Spinal Ependymoma Resection.

AIM: Spinal ependymomas are among the most common intramedullary neoplasms in both adults and children. While surgical resection is the golden treatment standard, the role chemotherapy and radiotherapy have in patients with spinal ependymomas remains unclear. The aim of this study is to determine the predictors of functional outcome following spinal ependymoma resection to single out patients that may require adjuvant therapy. METHODS: We conducted a retrospective study on patients that underwent spinal ependymoma resection in our institution in a 10-year period. Magnetic resonance imaging of

Tanycytic ependymoma of filum terminale: a case report.

Tanycytic ependymoma is an uncommon but well-recognized variant of ependymoma. Here we report a case of tanycytic ependymoma occurring at the region of filum terminale in a 44-year male who presented with low backache, bilateral lower limb weakness and urinary incontinence. MR imaging in this patient showed a lesion that was composed of solid and cystic components and was suggestive of ependymoma. The filum terminale region is an extremely unusual location for the occurrence of tanycytic ependymoma. To the best of our knowledge this is the third case of tanycytic ependymoma occurring in the fi

[MRI features of 13 cases of intracranial ependymoma].

OBJECTIVE: To improve the accuracy of qualitative diagnosis of intracranial ependymomas by analyzing their magnetic resonance (MR) images. METHODS: The MR images of 13 cases of intracranial ependymomas confirmed by pathological examinations were investigated retrospectively. RESULTS: In the 10 cases of ventricle ependymomas, the tumors were identified in the fourth ventricle in 3 cases and in the lateral ventricle in 7 cases. The T(1)-weighted MR images presented isointense or hypointense signals and T(2)-weighted images showed inhomogeneous hyperintense ones. No edema occurred around these

Mixed ependymoma-neuroendocrine tumor of the lateral ventricle.

A histologically unique glioneuronal neoplasm occupying the lateral ventricle of a child was immunohistochemically and ultrastructurally characterized. Its principal component exhibited the characteristic features of ependymoma, whereas a minor population of neuroendocrine cells, occurring singly and in small clusters, lay scattered throughout the ependymoma component. Yet another unusual finding was the presence of numerous elastic fibers within the extracellular matrix. This tumor is considered to represent a true mixed neoplasm consisting of ependymal and neuroendocrine elements.

Intracranial ependymoma in children: analysis of prognostic factors.

Between 1955 and 1986, 25 children (aged 2 weeks to 15 years) were treated for intracranial ependymoma at M.D. Anderson Cancer Center. Nine patients had supratentorial primaries (5 high-grade, 4 low-grade), and 16 had infratentorial primaries (9 high-grade, 7 low-grade). Five patients had gross complete resection and 20 had incomplete resection. Seven patients received craniospinal irradiation (25-36 Gy to the neuro-axis, 45-55 Gy to tumor bed), 12 received local field irradiation (29-60 Gy, median 50 Gy). Five infants had adjuvant chemotherapy without radiotherapy, and 6 children had post-rad

Posterior Fossa Group A Ependymoma in Trisomy 21.

Brain tumors are the most common solid tumors in children. However, the risk of central nervous system tumors appears reduced in individuals with trisomy 21, particularly for tumors of nervous tissue. We present the first reported case of posterior fossa group A (PFA) ependymoma in a patient with trisomy 21. Including this case, only four cases of ependymoma in trisomy 21 have been reported. Similar to medulloblastoma, ependymoma may occur at a markedly reduced frequency in trisomy 21. Investigating the protective role of trisomy 21 in ependymoma development may provide new insights into its p

Intradural extramedullary ventral thoracic spinal ependymoma: illustrative case.

BACKGROUND: Spinal cord ependymomas are typically intramedullary and rarely present as intradural extramedullary (IDEM) tumors. Here, the authors report a case of a long-segment thoracic IDEM ependymoma that was surgically treated, along with a review of the existing literature on this uncommon presentation. OBSERVATIONS: A 27-year-old female presented with a 1-year history of low back pain. The condition was initially mild and managed conservatively; however, 6 weeks prior to presentation, she developed progressive weakness in both lower limbs. Spinal MRI revealed a large IDEM mass along the

Optimizing outcomes in intracranial ependymoma: a contemporary review.

Intracranial ependymomas are glial tumors arising from the ependymal lining of the ventricular system, most commonly affecting young children (median age: 5 years), though they can occur across all age groups. Typically located in the posterior fossa, they account for fewer than 10% of pediatric central nervous system neoplasms and show a slight male predominance. Clinical symptoms vary by location, with posterior fossa tumors often causing hydrocephalus-related signs, and supratentorial lesions presenting with seizures or focal deficits. The 2021 WHO CNS5 classification integrates histologic,

Dorsal cervical approach for recurrent intradural anaplastic ependymoma.

The video demonstrates an operative approach to a recurrent cervical anaplastic ependymoma. MYCN-amplified anaplastic ependymomas are locally aggressive, recurrent, and have a high risk of iatrogenic injury. In this case, the patient presented with local, aggressive tumor expansion, arachnoid adhesions, and pial invasion ventral to the spinal cord. Subcapsular decompression minimized cord retraction from a dorsal approach. Removal of the tumor capsule was guided by bipolar stimulation paired with neuromonitoring. Local gross-total resection was achieved, and the patient had a postoperative imp

Imaging features to distinguish posterior fossa ependymoma subgroups.

OBJECTIVES: Posterior fossa ependymoma group A (EPN_PFA) and group B (EPN_PFB) can be distinguished by their DNA methylation and give rise to different prognoses. We compared the MRI characteristics of EPN_PFA and EPN_PFB at presentation. METHODS: Preoperative imaging of 68 patients with posterior fossa ependymoma from two centers was reviewed by three independent readers, blinded for histomolecular grouping. [Ependymoma in WHO Classification of Tumours 5th Edition].

Ependymoma is a rare central nervous system tumor that occurs in all age groups and is one of the most common childhood malignant brain tumors. Until the 3rd edition of the WHO Classification of Brain Tumours, ependymoma was classified based solely on histologic findings, but advances in molecular analysis led to the incorporation of molecular information into the diagnosis of ependymoma for the first time in the 2016 WHO Classification, 4th edition. However, the low correlation between tumor grade and survival prognosis remained a problem. Unlike other malignant brain tumors, ependymoma is ge
