An investigation into reading epilepsy.

In summary, our main findings were firstly, that faulty visuolingual processing at the level of Brain's visual word schemas, with its intimate connection with the verbal memory pool, is the abnormality that is essential for initiation of spike activity in reading epilepsy. Secondly, visual input is not essential in production of reading epilepsy. Thirdly, we have demonstrated a subconscious voluntary control mechanism operating in our patient. Lastly, we have illustrated alternatives to drug therapy, which, in our patient and in other cases, has not been shown of great value.

Familial Adult Myoclonus Epilepsy: A Non-Coding Repeat Expansion Disorder of Cerebellar-Thalamic-Cortical Loop.

Familial adult myoclonus Epilepsy (FAME) is a non-coding repeat expansion disorder that has been reported under different acronyms and initially linked to four main loci: FAME1 (8q23.3-q24.1), FAME 2 (2p11.1-q12.1), FAME3 (5p15.31-p15.1), and FAME4 (3q26.32-3q28). To date, it is known that the genetic mechanism underlying FAME consists of the expansion of similar non-coding pentanucleotide repeats, TTTCA and TTTTA, in different genes. FAME is characterized by cortical tremor and myoclonus usually manifesting within the second decade of life, and infrequent seizures by the third or fourth decad

[Epilepsy with catamenial pattern].

Catamenial pattern epilepsy is defined as an increase in the frequency of seizures during a specific stage of the menstrual cycle compared to baseline. It has been described that around a third of women with epilepsy have a catamenial pattern. The changes in the seizure pattern would be explained by the influence of catamenial fluctuations, of female gonadal hormones on neuronal excitability. Progesterone through its metabolite allopregnanolone plays a protective role by increasing GABAergic transmission; however, its effect on brain progesterone receptors can increase neuronal excitability. Effect of antiepileptic drug levetiracetam on cochlear function.

BACKGROUND: Levetiracetam (LEV, 5-100 mg/kg) has been shown to prevent audiogenic seizures in a dose-dependent manner. This chemical is known to bind to synaptic vesicle protein 2A and inhibit l-type calcium channels, affecting neurotransmitter release. We hypothesize that the drug prevents audiogenic seizures partially by affecting cochlear neural response. METHODS: To test this hypothesis, rats were given 1000, 500, 50, or 0 mg/kg (saline control) LEV-injection. Distortion product otoacoustic emissions (DPOAE), reflecting outer hair cell (OHC) function, and cochlear compound action potentia

Reading epilepsy: report of five new cases and further considerations on the pathophysiology.

Five new cases of reading epilepsy (RE) are reported. This is an epilepsy syndrome belonging to the group of idiopathic localization-related epilepsies. They all have some interesting features which contribute to the understanding of the pathomechanism and nosology of this specific type of reflex epilepsy. In our first patient the precipitating effect of texts in unknown languages depended upon phonematic intricacy. With our second case, changes of script within the text (Latin to Greek) increased the precipitating effect. The visual aura experiences reported in vague terms by some patients wi

Glutamic acid decarboxylase 65 (GAD65)-mediated autoimmune musicogenic epilepsy in a patient with acute myeloid leukaemia.

We describe a woman in her 30s who presented with progressive cognitive decline and reflex focal seizures triggered by specific auditory stimuli. Serum autoimmune testing demonstrated markedly elevated anti-glutamic acid decarboxylase 65 (GAD65) antibody titres (>2000 IU/mL). A paraneoplastic work-up revealed acute myeloid leukaemia (AML). After comprehensive neurological and haematological evaluation, she was diagnosed with paraneoplastic anti-GAD65-mediated autoimmune epilepsy, consistent with limbic encephalitis. This case is notable as it highlights a rare association between AML and paran

The Moro reflex: insights into the pathophysiology of generalized tonic-clonic seizures and infantile spasms.

The Moro reflex (MR) is a primitive reflex that disappears after the first three months of life. It was described by the Austrian paediatrician Ernst Moro (1874-1951) in 1918, although the earliest visual representation of the MR dates back to the first half of the 14th Century, in a fresco by Ambrogio Lorenzetti (1290-1348). The neural centre underlying the MR is located in the lower part of the brainstem since it can be elicited also in anencephalic infants, as shown by the Austrian neurologist Eduard Gamper (1887-1938) in the first medical description of anencephaly (1926). The MR is due to

Ictal bradyarrhythmias and asystole requiring pacemaker implantation: Combined EEG-ECG analysis of 5 cases.

BACKGROUND: Seizures can lead to cardiac arrhythmias by a number of mechanisms including activation/inhibition of cortical autonomic centers, increase in vagal tone through activation of brainstem reflex centers, and respiratory failure. Ictal asystole (IA) is a potential mechanism underlying sudden unexpected death in epilepsy (SUDEP). We analyzed the clinical features of 5 patients who developed IA requiring pacemaker implantation. METHODS: Patients with ictal arrhythmias were identified from the video-telemetry and ambulatory EEG database at Greater Manchester Neurosciences Centre, as well

Reading epilepsy from the dominant temporo-occipital region.

BACKGROUND: Reading epilepsy is a rare form of epilepsy, classified among idiopathic, age- and localisation-related (partial) epilepsies as a reflex epilepsy syndrome. Seizures usually consist of myoclonic jerks restricted to the jaw. However, distinct ictal features including visual symptoms and paroxysmal a- or dyslexia are described in some patients. The anatomical substrate of ictogenesis in reading epilepsy remains poorly understood. METHODS: The authors report here the case of a primary reading epilepsy for which ictal semiology was characterised by visual symptoms and dyslexia, investi

Autoimmune Encephalitis and Musicogenic Epilepsy: A Case of GAD65 Antibody-Associated Seizure.

Musicogenic epilepsy (ME) is a rare form of reflex epilepsy with a prevalence of 1 in 10,000,000. Recent research suggests a potential link between ME and autoimmune encephalitis, particularly involving glutamic acid decarboxylase 65-isoform (GAD65) antibodies. A 48-year-old female presented with a one-year history of music-triggered seizures. Her episodes were characterized by an initial aura followed by unresponsiveness and oral automatisms. Electroencephalography revealed abnormalities in the left anterior temporal lobe and temporal leads. Laboratory studies showed positive anti-GAD65 antib

Combined variants in reading epilepsy; coexisting anterior and posterior variants camouflaged as heat cramps where the patient finds his own diagnosis searching the internet.

Reading epilepsy is a form of reflex-induced seizures. Two entities are postulated as part of a clinical spectrum; one anterior variant with jaw jerks and orofacial myoclonia and another posterior variant with visual symptoms and alexia or dyslexia. We present a case with suggestible evidence of both conditions coexisting within the same patient, a finding that, to our knowledge, has not been previously reported. The diagnosis in this specific case was contributed to by the patient searching the internet.

Drawing-induced seizures.

In a review of the literature on triggering mechanisms in reflex epilepsy, we found no reported instance of drawing as the provoking factor. Recently we studied the case of a 17-year-old man with myoclonic seizures. These seizures were most effectively triggered by drawing, although occasionally they occurred spontaneously and could also be evoked by other activities requiring concentration. The results of a neurological examination were normal, and family history was unremarkable. Treatment with primidone and methsuximide has been very effective.

Use of a Dynamic Genetic Testing Approach for Childhood-Onset Epilepsy.

IMPORTANCE: Although genetic testing is important for bringing precision medicine to children with epilepsy, it is unclear what genetic testing strategy is best in maximizing diagnostic yield. OBJECTIVES: To evaluate the diagnostic yield of an exome-based gene panel for childhood epilepsy and discuss the value of follow-up testing. DESIGN, SETTING, AND PARTICIPANTS: A case series study was conducted on data from clinical genetic testing at Children's Hospital of Philadelphia was conducted from September 26, 2016, to January 8, 2018. Initial testing targeted 100 curated epilepsy genes for seq

Cortical reflex negative myoclonus.

Three patients with progressive myoclonic epilepsy (PME), two of them clinically manifesting only negative myoclonus and the other manifesting both positive and negative myoclonus, were electrophysiologically investigated, and compared with two other patients with PME presenting with only positive myoclonus. Electric stimulation of the median nerve during sustained active wrist extension in the three patients with negative myoclonus often elicited a short lapse of the posture in the stimulated hand associated with a silent period in the muscle discharge with or without being preceded by an abr

Intractable sneezing: case report and literature review.

Intractable sneezing is rare; the 12th case is reported here. Various causative factors have been identified in the literature; these include psychologic problems, cervical lymphadenitis, epilepsy, and allergy. The sneeze reflex consists of two phases, nasal and respiratory, which are mediated by cranial nerves V and VII and by brain-stem respiratory centers. Hypotheses that have been proposed to explain the multiple causation of sneezing are the concept of the trigeminal system as a central neuronal pool, optic-trigeminal summation, and parasympathetic generalization.

The genetic audiogenic seizure hamster from Salamanca: The GASH:Sal.

The hamster has been previously described as a paroxysmal dystonia model, but our strain is currently recognized as a model of audiogenic seizures (AGS). The original first epileptic hamster appeared spontaneously at the University of Valladolid, where it was known as the GPG:Vall line, and was transferred to the University of Salamanca where a new strain was developed, named GASH:Sal. By testing auditory brainstem responses, the GASH:Sal exhibits elevated auditory thresholds that indicate a hearing impairment. Moreover, amplified fragment length polymorphism analysis distinguished genetic dif

Identification of a monogenic locus (jams1) causing juvenile audiogenic seizures in mice.

Epilepsy is a debilitating disease with a strong genetic component. Positional cloning has identified a few genes for rare monogenic epilepsy syndromes; however, the genetics of common human epilepsies are too complex to be analyzed easily by current techniques. Mouse models of epilepsy can further this analysis by eliminating genetic background heterogeneity and enabling the production of sufficient numbers of offspring. Here, we report that Black Swiss mice have a heretofore unrecognized specific susceptibility to audiogenic seizures. These seizures are characterized by wild running, loss of
