STUDY DESIGN: Occlusion of the infrarenal abdominal aorta was applied to adult rabbits, which induced spinal cord ischemia, followed by disocclusion and reperfusion. Cortical somatosensory- and motor-evoked potentials were monitored continuously up to 24 hours and correlated to hind limb motor and sensory status.
OBJECTIVES: To investigate cortical somatosensory-, and motor-evoked potentials in the rabbit model of spinal cord ischemia and reperfusion injury, especially their changes during reperfusion and their relationship to hind limb motor and sensory function.

The excitability cycles of the N1-P1-N2 waveforms of the scalp-recorded somatosensory evoked potential (SEP) and of the long-latency, cortical loop reflex electromyographic (EMG) activity were studied in two patients with cortical reflex myoclonus. Long-latency cortical loop reflex EMG activity in the thenar muscles and giant SEPs occurred following median nerve stimulation. The excitability cycle of the EMG paralleled that of the SEP. There was an initial period of attenuation of SEP and EMG amplitude at interstimulus intervals (ISIs) of less than 40 ms followed by a period of amplitude enhan

In 28 healthy subjects the peripheral nervous system was stimulated with brief high-voltage condenser discharges over the vertebral column. Latencies were correlated with the length of the extremity. The following normative data were obtained: M. biceps brachii 5.2 ms (SD 0.37 ms), M. abductor pollicis 14.2 ms (SD 0.86 ms), M. tibialis anterior 16.2 ms (SD 1.0 ms), M. extensor digitorum brevis 25.0 (SD 1.3 ms). Comparison of these latencies with those obtained by F-wave studies support the idea that motor roots are stimulated by this method.

Amyotrophic lateral sclerosis (ALS) diagnosis relies on signs of progressive damage to both lower motoneuron (LMN), given by clinical examination and electromyography (EMG), and upper motoneuron (UMN), given by clinical examination only. Recognition of UMN involvement, however, is still difficult, so that diagnostic delay often remains too long. Shortening the time to clinical and genetic diagnosis is essential in order to provide accurate information to patients and families, avoid time-consuming investigations and for appropriate care management. This study investigates whether combined pate

BACKGROUND CONTEXT: Transcranial electrical stimulation motor-evoked potentials (Tc-MEPs) are the current trend and are important in preventing intraoperative neurological deficits. Posttetanic Tc-MEPs (p-MEP) can augment the amplitudes of compound muscle active potentials (CMAPs), especially in the case of insufficient conventional Tc-MEPs (c-MEP).
PURPOSE: To retrospectively investigate pre- and intraoperative factors necessitating p-MEP monitoring and to examine changes in the success rates of baseline Tc-MEP monitoring before and after tetanic stimulation in patients with such factors.

Spinal evoked potentials, sciatic neurogenic motor evoked potentials, and somatosensory evoked potentials were recorded before and after overdistraction of the spinal cord, and compared with the clinical status of 14 pigs. The sciatic neurogenic motor evoked potential consisted of two components: fast and slow. The fast component was more sensitive and associated to a greater degree with motor function in wake-up tests than the slow component somatosensory evoked potential and spinal evoked potential. Furthermore, the loss of only the fast component in the initial status allowed the possibilit

Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) syndrome is a complex and infrequently encountered mitochondrial cytopathy. Patients with MELAS often present with multi-systemic manifestations, making their anesthetic management particularly challenging. In this case report, we describe in detail our anesthetic approach for a 19-year-old male with confirmed MELAS linked to an m.3243A>G mutation. The patient had been diagnosed with MELAS at age 12 following a stroke-like episode and presented with progressive spinal deformities. He exhibited a 70° thoracic spi

OBJECTIVE: Deep brain stimulation (DBS) modulates neural elements in the target region but can cause involuntary muscle contractions through unintended internal capsule activation. We characterized upper and lower limb motor evoked potentials (MEPs) produced by DBS to determine how stimulation location and orientation affect responses.
METHODS: Low-frequency DBS was delivered in the globus pallidus internus (GPi, 17 hemispheres) and subthalamic nucleus (STN, 8 hemispheres). MEPs were identified by stimulus-triggered averaging of surface electromyography (sEMG) from nine contralateral muscles.

OBJECTIVE: To compare the intraoperative blink reflex (BR) and facial corticobulbar motor evoked potentials (FCoMEPs) in skull base surgery, and to assess their respective and combined value in predicting postoperative facial nerve outcome.
METHODS: 80 consecutive patients undergoing skull base surgery were monitored using ipsilateral FCoMEPs and bilateral BR recordings. BR was particularly assessed during stages in which FCoMEPs were limited by stimulus-induced movement. Facial nerve function was evaluated in the immediate postoperative period and at 6 months using the House-Brackmann (HB) s

Intraoperative monitoring of motor-evoked potentials (MEPs) is important for preventing serious ischemic complications during aneurysm surgery following subarachnoid hemorrhage (SAH). Reduced intraoperative MEP amplitude indicates corticospinal tract neuronal damage, which should be avoided during surgical maneuvers. We aimed to investigate whether the MEP amplitude immediately before surgery can serve as both a control value for intraoperative MEP and an indicator of the pathophysiology of cortical excitability and inhibitory-facilitatory circuits immediately after SAH and neurological functi

Tetanic stimulation of the peripheral nerve, immediately prior to conducting transcranial electrical stimulation motor evoked potential (TES-MEP), increases MEP amplitudes in both innervated and uninnervated muscles by the stimulated peripheral nerve; this is known as the remote augmentation of MEPs. Nevertheless, the mechanisms underlying the remote augmentation of MEPs remain unclear. Although one hypothesis was that remote augmentation of MEPs results from increased motoneuronal excitability at the spinal cord level, the effect of spinal anterior horn cells has not yet been investigated. INTRODUCTION: Neuromuscular blockade suppresses transcranial electrical motor evoked potential (TceMEP) amplitude and is usually avoided during TceMEP monitoring. In this randomized controlled trial, we investigated whether rocuronium-induced suppression of TceMEP amplitude could be reversed by sugammadex in patients undergoing spine surgery.
METHODS: Seventy-six patients undergoing spinal surgery were randomly allocated into sugammadex and control groups. In the sugammadex group, a rocuronium infusion was titrated to maintain moderate neuromuscular blockade (2 twitches on train-of-four) unti

PURPOSE: The incidence of bite injuries associated with transcranial electrical stimulation motor-evoked potentials monitoring reportedly ranges from 0.13 to 0.19%. However, in clinical practice, bite injuries appear to occur more frequently than previously reported. Our aim was to identify the incidence of and perioperative risk factors associated with bite injuries caused by transcranial electrical stimulation motor-evoked potential monitoring.
METHODS: Patients who underwent elective surgery with transcranial electrical stimulation motor-evoked potential monitoring at a single tertiary hos

Background Nonhuman primates (NHP) may provide the most adequate (in terms of neuroanatomy and neurophysiology) model of spinal cord injury (SCI) for testing regenerative therapies, but bioethical considerations exclude their use in severe SCI. New Method A reproducible model of SCI at the lower thoracic level has been developed in Rhesus macaques. The model comprises surgical resection of 25% of the spinal cord in the projection of the dorsal funiculus and dorsolateral corticospinal pathways, controlled via registration of intraoperative evoked potentials (EPs). The animals were evaluated usi

Motor evoked potentials (MEPs) are commonly used to prevent neurological injury when operating in close proximity to the motor cortex or corticospinal pathway. We report a novel application of the NIM® nerve monitoring system (Medtronic@ NIM response 3.0) for intraoperative direct cortical (dc)-MEPs monitoring. A 69-year-old female patient presented with a 4month history of progressive left hemiparesis resulting from a large right sided posterior frontal meningioma that abutted and compressed the motor cortex. Motor cortical mapping and MEPs were indicated. The patient was anesthetized and mai

The present study prospectively investigated the value of free-running electromyography (EMG) and transcranial motor-evoked potentials (TcMEP) during anterior cervical microdiscectomy and fusion for cervical disc herniation with radiculopathy. Thirty-eight patients were eligible for inclusion in the study. Pain was assessed using the visual analogue scale (VAS) score. Quality of life results were scored according to Odom's criteria in the postoperative period. Patients with at least a 41% increase in TcMEP amplitude had an excellent postoperative outcome. When the increase of TcMEP amplitude w

OBJECTIVES: Spinal cord injury (SCI) causes anorectal problems, whose pathophysiology remains poorly characterized. A comprehensive method of evaluating spino-anorectal function is lacking. The aim of this study was to investigate the neuropathophysiology of bowel dysfunction in SCI by evaluating motor-evoked potentials (MEP) of anus and rectum following transspinal magnetic stimulation and anorectal physiology.
METHODS: Translumbar and transsacral magnetic stimulations, anorectal manometry, and pudendal nerve terminal motor latency (PNTML) were performed in 39 subjects with SCI and anorectal
