Contractile reserve in systemic sclerosis patients as a major predictor of global cardiac impairment and exercise tolerance.

Several studies have evidenced high prevalence of myocardial systolic and diastolic dysfunction among patients with systemic sclerosis (SSc). Exercise echocardiography has shown a diagnostic and prognostic role in identifying early left ventricular (LV) dysfunction in several myocardial pathological settings. The aim of our study was to evaluate early signs of LV impairment under exercise and their correlation to patient's exercise tolerance. Forty-five patients (age 60.4 ± 10.3 years) with SSc and 20 age and sex comparable controls were enrolled in the study. All patients underwent clinical e

Effect of β-blocker cessation on chronotropic incompetence and exercise tolerance in patients with advanced heart failure.

BACKGROUND: Chronotropic incompetence is defined as the inability to reach 80% of heart rate (HR) reserve or 80% of the maximally predicted HR during exercise. The presence of chronotropic incompetence is associated with reduced peak oxygen consumption, and rate-responsive pacing therapy is under investigation to improve exercise capacity in heart failure (HF). However, uncertainty exists about whether chronotropic incompetence and reduced exercise tolerance in HF are attributable to β-blockade. METHODS AND RESULTS: Subjects with HF and receiving long-term β-blocker therapy underwent cardiopu

Resting muscle sympathetic nerve activity, cardiac metaiodobenzylguanidine uptake, and exercise tolerance in patients with left ventricular dysfunction.

BACKGROUND: Sympathetic nerve overactivity and reduced exercise tolerance are characteristic features of patients with heart failure. However, to what extent sympathetic nerve overactivity contributes to limiting exercise tolerance has not been clearly defined. METHODS: Myocardial iodine 123-metaiodobenzylguanidine (MIBG) scintigraphy, muscle sympathetic nerve activity (MSNA), and cardiopulmonary exercise testing were performed within 3 days in 30 patients with left ventricular dysfunction (LVD). Cardiac sympathetic nerve activity was estimated using H/M ratio and washout rate (WR) of 123I-MI

Does an aortopulmonary shunt before repair of tetralogy of Fallot limit exercise tolerance in long-term survivors?

The purpose of this study was to evaluate the impact of an aortopulmonary shunt on exercise capacity in long-term survivors after total repair of tetralogy of Fallot (17.6(2.0) years' follow-up). Submaximal exercise tests, pulmonary function tests, lund diffusion tests for carbon monoxide, two-dimensional and Doppler echocardiography were performed in 12 patients with an aortopulmonary shunt (group A) and in 21 patients (group B) without a shunt before repair. There were no significant differences in two-dimensional and Doppler echocardiographic findings nor in pulmonary function. Group A show

Influence of posture on arm exercise tolerance and physiologic responses in persons with spinal cord injured paraplegia.

This study compared metabolic and cardiopulmonary responses to incremental supine and upright sitting arm crank ergometry (ACE) in nine men with spinal cord injured paraplegia ranging from T1-T5. Both tests consisted of continuous graded ACE from rest to volitional fatigue on a modified electronically braked cycle ergometer with the work rate increased by 8.2 W.min-1. No significant differences (P > 0.05) existed for peak ACE power output (W), oxygen uptake, pulmonary ventilation, respiratory exchange ratio, and O2 pulse between the two tests. Heart rate and O2 pulse responses at six submaxima

[Exercise tolerance in mitral stenosis and chronic obstructive pulmonary disease: evaluation by anaerobic threshold and radionuclide ventriculography].

Serial radionuclide ventriculography was performed using a newly developed "real-time" system, and left ventricular ejection fraction (LVEF), right ventricular ejection fraction (RVEF), stroke volume (SV), and cardiac output (CO) were measured during graded supine exercise in five patients with mitral stenosis (MS), in five patients with chronic obstructive pulmonary disease (COPD) and in five healthy subjects. Simultaneous pulmonary gas exchange analysis permitted determining the anaerobic threshold, which is the point during incremental exercise when lactate begins to accumulate in the blood

Effects and tolerability of exercise therapy modality on cardiorespiratory fitness in lung cancer: a randomized controlled trial.

BACKGROUND: Poor cardiorespiratory fitness (CRF) is a cardinal feature of post-treatment primary lung cancer. The most effective exercise therapy regimen to improve CRF has not been determined. METHODS: In this parallel-group factorial randomized controlled trial, lung cancer survivors with poor CRF (below age-sex sedentary values) were randomly allocated to receive 48 consecutive supervised sessions thrice weekly of (i) aerobic training (AT)-cycle ergometry at 55% to >95% of peak oxygen consumption (VO2 peak); (ii) resistance training (RT)-lower and upper extremity exercises at 50-85% of max

Effect of catheter ablation of the atrioventricular junction on quality of life and exercise tolerance in paroxysmal atrial fibrillation.

The effect of catheter ablation of the atrioventricular junction (AV) and implantation of a rate-adaptive pacemaker on quality of life and exercise capacity was evaluated prospectively in 12 consecutive patients with paroxysmal atrial fibrillation (AF). All patients had been demonstrated to have paroxysmal AF that was refractory to medical therapy over a long period of time (mean 9 +/- 7 years). Patients performed a symptom-limited treadmill exercise test on the day before catheter ablation and 6 weeks after hospital discharge. Quality of life was measured using the physical dimension of the M

Effects of chronic administration of codeine and promethazine on breathlessness and exercise tolerance in patients with chronic airflow obstruction.

It has been reported that short-term treatment with relatively high doses of opiates or promethazine causes improvements in dyspnoea and exercise tolerance in patients with chronic airflow obstruction (CAO). This study was designed to determine whether initial benefits were sustained during chronic administration of codeine or promethazine and to compare the two drugs in terms of their efficacy and possible mechanisms of action. Eleven patients with stable CAO were entered into a double-blind, randomized cross-over trial in which codeine (30 mg four times daily) or promethazine (25 mg four tim

[Predictors of exercise tolerance during early rehabilitation after endovascular revascularization in elderly patients with acute coronary heart disease.].

Exercise tolerance, determined in the early stages of cardiac rehabilitation during exercise tests upon discharge from the hospital, is an important functional indicator of the cardiovascular system in patients with coronary heart disease after revascularization operations. Exercise tolerance should be taken into account when planning and evaluating the effectiveness of rehabilitation measures at all stages of treatment. 126 elderly patients with acute coronary syndrome were examined. Emergency angioplasty and coronary artery stenting were performed. Examination, treatment, and inpatient rehab

Functional Capacity and Exercise Tolerance in Residential Care Home Residents with Treated Arterial Hypertension and Coronary Heart Disease.

Background: Older adults commonly experience a progressive decline in functional capacity and exercise tolerance, particularly among residents of residential care homes. Advanced age is associated with a higher prevalence of coronary heart disease (CHD) and arterial hypertension, both of which further reduce physical activity levels. Evidence indicates that regular, structured physical activity can improve physical functioning and cardiovascular parameters. This study aimed to assess functional capacity and exercise tolerance in residential care home residents with treated arterial hypertensio

Cost-effectiveness of stress echocardiography and exercise tolerance tests as screening in asymptomatic adults before starting physical activity.

AIMS: Previous studies on exercise tolerance screening in asymptomatic individuals before starting physical activity were not cost-effective due to low specificity. However, given progress in diagnosing and treating coronary artery disease (CAD), a reevaluation of this approach is justified. We aimed to examine whether stress echocardiography (SE) would be cost-effective. METHODS AND RESULTS: The study was conducted on asymptomatic individuals with no known coronary disease. The decision tree had two arms: in one arm, the subjects underwent stress echocardiography (SE) as a screening test bef

Echocardiographic correlation of right ventricular-Pulmonary artery coupling to exercise tolerance in congenital heart disease-associated pulmonary hypertension.

BACKGROUND: Pulmonary arterial hypertension (PAH) is a chronic, progressive disease affecting the pulmonary vasculature, with a high mortality rate. Patients with PAH due to congenital heart disease (CHD) (World Health Organization Group 1) exhibit significantly reduced exercise tolerance compared to other PAH groups. Right ventricular (RV) dysfunction is a commonly used independent prognostic parameter in PAH. This study aimed to investigate an echocardiographic-derived parameter of RV-pulmonary artery (PA) coupling to exercise tolerance in PAH-CHD. SUBJECTS AND METHODS: This cross-sectional

Improved pulmonary function and exercise tolerance despite persistent pulmonary fibrosis over 1 year after severe COVID-19 infection.

We conducted a prospective single-centre cohort study of 104 multi-ethnic severe COVID-19 survivors from the first wave of the pandemic 15 months after hospitalisation. Of those who were assessed at 4 and 15 months, improvement of ground glass opacities correlated with worsened fibrotic reticulations. Despite a high prevalence of fibrotic patterns (64%), pulmonary function, grip strength, 6 min walk distance and frailty normalised. Overall, dyspnoea, cough and exhaustion did not improve and were not correlated with pulmonary function or radiographic fibrosis at 15 months, suggesting non-respir

Isosorbide DiNitrate Effect on Hemodynamic Profile, Liver Stiffness, and Exercise Tolerance in Fontan Circulation (The NEET Clinical Trial).

After Fontan operation, decreased venous capacitance and venoconstriction are adaptive mechanisms to maintain venous return and cardiac output. The consequent higher venous pressure may adversely impact end-organ function, exercise capacity and result in worse clinical outcomes. This pilot study evaluated the safety and effect of isosorbide dinitrate (ISDN), a venodilator, on exercise capacity, peripheral venous pressure (PVP), and liver stiffness in patients with Fontan circulation. In this prospective single-arm trial, 15 individuals with Fontan circulation were evaluated at baseline and aft

Cardiopulmonary and Muscular Interactions: Potential Implications for Exercise (In)tolerance in Symptomatic Smokers Without Chronic Obstructive Pulmonary Disease.

Smoking and physical inactivity are important preventable causes of disability and early death worldwide. Reduced exercise tolerance has been described in smokers, even in those who do not fulfill the extant physiological criteria for chronic obstructive pulmonary disease (COPD) and are not particularly sedentary. In this context, it is widely accepted that exercise capacity depends on complex cardio-pulmonary interactions which support oxygen (O2) delivery to muscle mitochondria. Although peripheral muscular factors, O2 transport disturbances (including the effects of increased carboxyhemoglo
