Fulminant endogenous panophthalmitis caused by Clostridium septicum infection.

We report an unusual case of fulminant endogenous Clostridium septicum panophthalmitis. A 74-year-old male patient presented with sudden amaurosis in the right eye, which in a few hours, evolved into an orbital cellulitis, endophthalmitis, anterior segment ischemia, and secondary perforation of the eye. A complete diagnostic study, which included cranial and orbital contrast-enhanced computed tomography scan, contrast-enhanced magnetic resonance imaging, blood cultures, and complete blood work, were performed. No causal agent was identified. Clostridium septicum infection caused fulminant gase

A case of delayed oculocardiac reflex induced by an intraorbital foreign body.

A 56-year-old male presented with a history of left periorbital pain, nausea, vomiting, dyspnea, sweating, and pallor. His left eye was injured by a bomb explosion approximately 40 years ago. Evisceration was performed. There were no cardiopulmonary and gastroenterological findings except mild bradycardia (54/min). CT evaluation revealed a 4-cm-sized, sharp, metallic, intraorbital foreign body in the left orbit. After the removal of the foreign body, the symptoms completely resolved with a normal heart rate (69/min). The authors present a case of delayed oculocardiac reflex induced by an old i

A Rare Case of Streptococcus dysgalactiae Panophthalmitis and Concurrent Septic Arthritis from Hematogenous Spread: A Case Report.

INTRODUCTION: Panophthalmitis is a severe ocular infection with significant morbidity, most commonly caused by Staphylococcus aureus or Streptococcus pneumoniae. Streptococcus dysgalactiae is a rare cause of panophthalmitis, and its involvement in concurrent systemic infections is exceedingly uncommon. CASE PRESENTATION: We report a case of S. dysgalactiae panophthalmitis in an elderly male patient, associated with bacteremia and septic arthritis. Despite early antimicrobial therapy, the infection progressed rapidly, ultimately requiring evisceration of the affected eye. CONCLUSION: This cas

Fungal keratitis caused by Podospora austroamericana: A first case report.

We report a case of keratitis caused by a rare fungus Podospora austroamericana. Clinical and microbiological evaluation of the corneal ulcer was done and the treatment outcome was studied. The fungus was grown from the corneal scraping, and it was identified as P. austroamericana based on DNA sequence and analysis of the internal transcribed spacer region. The patient was treated with topical azithromycin, natamycin and voriconazole. Despite maximum medical therapy, the ulcer progressed very rapidly and the patient developed panophthalmitis and evisceration of the eye had to be done. This is

[Anophthalmic conjunctival sac plastic surgery using the modified cul-de-sac method].

AIM: The author refers about the plastic surgery technique of deepening the conjunctival sac in acquired anophthalmos without the orbital implant. The condition without the implant was caused primarily or secondarily after the enucleation or evisceration. The principal of the cul-de-sac technique is the fixation of the lower fornix conjunctiva to the orbital periosteum. MATERIAL AND METHODS: The modification of the original surgery technique applied by the author is from the nineties of the last century. It consists of the use of long-term resorbable suturing material for vascular sutures mad

Management of a blind painful eye.

Debilitating ocular pain poses a significant challenge to the ophthalmologist. When the pain is intractable and the eye has very poor vision and is disfigured, surgical removal of the eye has traditionally been the definitive treatment of choice. Because many people are uncomfortable psychologically with removal of their eye, however painful, and other patients are not good surgical candidates, an alternative to enucleation is sometimes warranted, and injection of a neurolytic substance can often induce long-lasting anesthesia for a blind painful eye. This article reviews a range of options fo

[Endogenous Nocardia asteroides endophthalmitis in a patient with systemic lupus erythematosus].

We report a case of endogenous nocardial endophthalmitis in a 32-year-old man with systemic lupus erythematosus. The patient developed pulmonary and ocular disease while on systemic corticosteroid and cyclophosphamide treatment. The intraocular infection progressed to a scleral fistula, and was treated with pars plana vitrectomy, lensectomy, intravitreous and intravenous antibacterial therapy. The diagnosis of Nocardia asteroides was made by isolation and growth of colonies from samples of a vitreous specimen and bronchioloalveolar aspirates. The eye became phthisical, it was eviscerated, and

[Effectiveness of present-day therapy of purulent lesions of the cornea].

The treatment of 313 patients (317 eyes) with purulent injuries of the cornea was studied. The medication therapy was found to be effective in 173 patients (176 eyes), which made up 56.2%. The purulent process in the cornea was arrested after the therapeutic penetrating and lamellar keratoplasty in 62 patients (63 eyes or 20.1%). 74 patients (74 eyes or 23.7%) were cured through the loss of an organ: enucleation was made for 63 patients, evisceroenucleation--for 8 patients and evisceration--for 3 persons. Before the onset of purulent pathology, all removed eyes were blind due to different dise
